Kate did the programming work for the MAR and assembled a team from Pharmacy Services, Nursing Services, and Internal Auditing. Members of the project team provided feedback on the software, made suggestions related to user training, and worked on an implementation schedule. Both team members and outside observers recognized the difficulty in developing and implementing the MAR in one year. Nurses antagonism toward the project surfaced after their suggestions and recommendations were rejected without explanation. The MAR project was also affected when a management shakeup occurred during the project development phase. The MAR was implemented and immediately failed. By 10:00 a.m. on the first day of the implementation, users complained that the system did not work and was too complicated. After hearing nurses complaints, physicians expressed concern about patient safety. At 1:30 p.m., Central Hospitals computer system crashed, and the MAR project was suspended. Case Study—Memorial Health System CPOE Implementation
Memorial Health System is an eight-hospital integrated health care system in the Midwestern United States. The health system has two downtown flagship tertiary care hospitals, each licensed for more than 700 beds, located in the two major metropolitan areas served by the system. The remaining six hospitals are community-based facilities, ranging in size from 200 to 400 beds. These hospitals are located in the suburban and rural areas served by Memorial Health System.
Four years ago, the system’s board of directors approved a multi-million-dollar initiative to install an enterprise-wide clinician provider order entry (CPOE) system intended to dramatically reduce medical errors. Today, the system is far from fully implemented, and, in fact, has been removed from all but one of the two tertiary care facilities, where it remains in pilot adopter status.
At the time, the board approved the CPOE initiative, the project was championed by Fred Dryer, the CEO, and was closely supported by Joe Roberts, the chief information officer (CIO) of the health system. Even during its proposal and evaluation by the board, the project was considered controversial by some of the health system’s stakeholders. For example, many of its physicians, who are community-based independent providers, were adamantly opposed to the CPOE system. They worried that their workload would increase because CPOE systems replace verbal orders with computer-entered orders by doctors. Dr. Mark Allen, a primary care physician commented, “The hospital is trying to turn me into a 12-dollar-an-hour secretary, and they aren’t even paying me 12 dollars an hour.”
In securing board approval, Dryer and Roberts presented an aggressive implementation plan that called for the requirements analysis, Request for Proposal (RFP), vendor selection, and project implementation to be completed in less than 18 months in all eight hospitals. During the discussion with the board, several of the members questioned the timeline. One noted, “It took you 2 years to set up e-mail, and everyone wanted e-mail. This will affect every clinician in every hospital. Do you really think you can do this in 18 months?”
In an effort to demonstrate results, Dryer and Roberts demanded results from the clinical and IT team formed for the project. By this time, a rushed requirements analysis had been completed, an RFP issued, a vendor selected, and a contract signed. The acquisition process took a little more than 6 months, leaving a year for the implementation.
In protest, a number of prominent physicians took their referral business to the other health system in the area that seized upon the controversy by promising that they would not use a CPOE. How do you as nurse leaders use your
knowledge and insight to move forward and transfer
your vision for quality and safety into reality? What do
you need to do to get key stakeholders on the bus and,
in some cases, even drive the bus? The roadmap for
planned change allows for an infrastructure of thought
brought to increase the likelihood for successful
change. Successful change is important to our patients
and to us as providers of that care.
This article, the second of a two-part series,
focuses on the application of change theory and the
elements of project management most critical to
successfully implementing a bar-coded medication
administration (BCMA) program. Examples will be
from one hospital’s experience, Saint Francis Medical
Center in Grand Island, Nebraska, to a health
system’s (Catholic Health Initiatives, Denver, Colorado)
approach to planning for 30 hospitals.
The definition of the BCMA program includes a
consistent, integrated information technology strategy,
with a focus on point-of-care BCMA to ensure that the
right person receives the right medication, in the right
dosage, via the right route, at the right time (five
rights). The bar code on medication is scanned before
administration to patients.

APPLICATION OF CHANGE THEORY AND
PROJECT MANAGEMENT
The first article discusses concepts and tools of both change
leadership and project management that lend support in plan-
ning and managing large- or small-scale change. Change lead-
ership is a common methodology of theory and tools that,
when used routinely, are central to integrating a change man-
agement model with the people side of change.
Project management is an application of knowledge, skills,
tools, and techniques customized to the initiative.The project
management elements discussed in the first article that are
most critical to successfully implementing planned change are
project charter, project budget and budget management, proj-
ect plan and schedule management, project staff organization,
project communications management, and project risk and
issue management.
CURRENT STATE ANALYSIS
Changing a process as complex as BCMA can and will
impact a variety of stakeholders. It is important to review
the process of medication administration from the time the
medication enters the facility through the time that the med-
ication is billed to the patient. Employees working in depart-
ments that will experience change with BCMA need to
know that their role is important and that their viewpoint
is valued.

Regular communication with all stakeholder groups, whether expected to be primary users such as nursing staff and physicians or ancillary users such as laboratory technicians, pharmacists, other clinicians, administrators, billing staff, and housekeepers, helps reduce the uncertainty inherent in adopting health information technology (HIT) and engages all stakeholders at the appropriate level of participation in planning, implementing, and identifying opportunities for improvement.
The ADKAR model of communication (depicted below) recognizes a continuum of key messages for projects that introduce a great amount of change—such as adopting HIT.
It illustrates the relationship between communicating and project phases. As soon as the need for HIT arises, Awareness building needs to begin. As the HIT vision unfolds, creation of Desire helps turn uncertainty into interest. Throughout the planning process, stakeholders need to become more Knowledgeable about HIT in order to make decisions appropriate to their role in the project. During implementation, stakeholders need to understand their Abilities, build capability, and celebrate using HIT. Communication does not end with implementation, however, as continued success during ongoing operations requires Reinforcement. The ultimate goal is to celebrate successful implementation and optimal utilization.

technology (IT) in larger institutions, ranging from rural

The awards are named in honor of Dr. Nicholas E.
Davies, an Atlantabased practice physician committed to
the ideal of improving patient care through better health information management. As a member
of the Institute of Medicine’s Patient Record Study Committee, Dr. Davies helped coin the term
"computerbased patient record," now more widely known as electronic medical records and
electronic health records (EMREHR). A tireless advocate for IT solutions, Dr. Davies was
chairpersonelect of the American College of Physicians when he was tragically killed in a plane
crash with Senator John G. Tower of Texas in April, 1991. His spirit lives on in the Davies
Award of Excellence.
In 2003, HIMSS extended the awards to include ambulatory practices with EMREHRs and in
2004 added yet another category for public health. Sponsored by HIMSS, the Nicholas E. Davies
Award Program encourages and recognizes excellence in the implementation of EMREHR
systems through showcasing concrete examples, understanding and sharing the value of EMR
EHR systems, offering visibility and recognition for their projects and sharing successful
implementation strategies.
The papers are evaluated in terms of EMREHR implementation, strategy, planning, project
management, and governance. The clinicians who submit entries discuss the functionality of their
EMREHRs and how those systems met the needs of staff and patients. Additionally, they speak
to how the technology design works to bring about the desired functionality and the institutions’
return on investment.
The purpose of this white paper, and the three others in this collection, is to give healthcare
providers a survey of best practices in EMREHR implementation, solid examples of leadership,

Research indicates that an effective EMR system can centralize and acquire crucial patient information efficiently leading to enhanced service delivery.
EMR offers the ability to incur improvements on patient safety through the provision of timely and consistent care to patients in performance and through compliance with clinical regulations and standards, and the avoidance of duplicates. It can also lead to lower health care costs and improved efficiencies by promoting single patient records entailing integrated information. This would ensure improved coordination and continuity of care by reducing redundant tests and waste.

Currently, each community health services program of CHD has information systems that effectively track specific information for their assigned work. However, the systems were designed reactively without considering future expansions or an updated public health data plan. Each data base is almost a standalone system. While the state data bases are linked to CHD’s data bases, the environmental data bases are not linked with communicable disease data bases. Representatives from the Tuberculosis (TB) program, for example, had to enter identical client information in four different places. Moreover, the state mandates the public health department report the health conditions of its citizens and control infectious diseases. In order for this to happen, one needs informatics systems that are capable of tracking infectious diseases and reporting all investigations in a timely manner. While the benefits of an integrated data base stand out, most employees in CHD are reluctant to change, particularly with regard to adopting and using a new technology as evidenced by the implementations of the EMR systems and other specifically designed data bases. To ensure success of the implementation, the project manager must provide relevant information to the stakeholders on a regular basis. This case focuses on project communication management, especially within the communication plan.

founded the Davies Awards of Excellence and managed
the program until merging with the Healthcare
Information and Management Systems Society (HIMSS)
in 2002. The award has annually highlighted healthcare
providers who successfully led efforts to transform their
organizations through technology, with the award largely
focusing on the implementation of healthcare information
technology (IT) in larger institutions, ranging from rural
health systems to big city hospitals. [
The awards are named in honor of Dr. Nicholas E.
Davies, an Atlantabased practice physician committed to
the ideal of improving patient care through better health information management. As a member
of the Institute of Medicine’s Patient Record Study Committee, Dr. Davies helped coin the term
"computerbased patient record," now more widely known as electronic medical records and
electronic health records (EMREHR). A tireless advocate for IT solutions, Dr. Davies was
chairpersonelect of the American College of Physicians when he was tragically killed in a plane
crash with Senator John G. Tower of Texas in April, 1991. His spirit lives on in the Davies
Award of Excellence.
In 2003, HIMSS extended the awards to include ambulatory practices with EMREHRs and in
2004 added yet another category for public health. Sponsored by HIMSS, the Nicholas E. Davies
Award Program encourages and recognizes excellence in the implementation of EMREHR
systems through showcasing concrete examples, understanding and sharing the value of EMR
EHR systems, offering visibility and recognition for their projects and sharing successful
implementation strategies.
The papers are evaluated in terms of EMREHR implementation, strategy, planning, project
management, and governance. The clinicians who submit entries discuss the functionality of their
EMREHRs and how those systems met the needs of staff and patients. Additionally, they speak
to how the technology design works to bring about the desired functionality and the institutions’
return on investment.
The purpose of this white paper, and the three others in this collection, is to give healthcare
providers a survey of best practices in EMREHR implementation, solid examples of leadership,
and a glimpse at the return on their investment (ROI) the EMREHR offers. This paper, focused

The Electronic Medical Record, EMR, system comes in and its implementation and acquisitions would facilitate the following. The retrieval, management and capturing of patient data such as medical history, lab results, and demographic data (Jawhari, Keenan, Zakus, Ludwick, Isaac, Saleh, Hayward, 2016). Research indicates that an effective EMR system can centralize and acquire crucial patient information efficiently leading to enhanced service delivery.
EMR offers the ability to incur improvements on patient safety through the provision of timely and consistent care to patients in performance and through compliance with clinical regulations and standards, and the avoidance of duplicates. It can also lead to lower health care costs and improved efficiencies by promoting single patient records entailing integrated information. This would ensure improved coordination and continuity of care by reducing redundant tests and waste.

The County Health Department (CHD)’s strategic plan specific es increasing capacities for the program evaluation and response to major communicable disease outbreaks. Senior managers have identified ed the need to improve informatics capacity as an important step toward meeting these goals. Currently, each community health services program of CHD has information systems that effectively track specific information for their assigned work. However, the systems were designed reactively without considering future expansions or an updated public health data plan. Each data base is almost a standalone system. While the state data bases are linked to CHD’s data bases, the environmental data bases are not linked with communicable disease data bases. Representatives from the Tuberculosis (TB) program, for example, had to enter identical client information in four different places. Moreover, the state mandates the public health department report the health conditions of its citizens and control infectious diseases. In order for this to happen, one needs informatics systems that are capable of tracking infectious diseases and reporting all investigations in a timely manner. While the benefits of an integrated data base stand out, most employees in CHD are reluctant to change, particularly with regard to adopting and using a new technology as evidenced by the implementations of the EMR systems and other specifically designed data bases. To ensure success of the implementation, the project manager must provide relevant information to the stakeholders on a regular basis. This case focuses on project communication management, especially within the communication plan.

It all began when a new epidemiologist, hired by CHD, articulated the need to have an integrated public health informatics system. confused, breathing rapidly and having to urinate far more often than normal. Hes having one of
those acid attacks, she said. when everything gets out of balance. She further revealed that he
had not been careful about his dietthe night before he had gorged on pizza and ice cream and had
consumed 8 to 10 bottles of beer, becoming so inebriated that he went to sleep without taking his
insulin.
Vital signs included temperature of 39 degrees C (102.2 degrees F), heart rate 96 beats per
minute (normal 72), and blood pressure 90/60 (normal 120/80). Respirations were noted to be fast
and hard and his breath had an unusual apple juice odor. Luis was 5 ft 9 in tall and weighed 285
lb. He was drowsy and confused about time and place.
Lab tests revealed abnormally high blood glucose and low (acidic) blood pH. His urine was also
unusually acidic and contained large amounts of glucose and ketone bodies, neither of which
should normally be present.
In addition to the above abnormalities, Luis had high blood osmolarity (indicating increased blood
solutes), low blood bicarbonate, and low blood partial pressure of CO2. His urine also had a very
high specific gravity.
The emergency room physician made a diagnosis of diabetic ketoacidosis and admitted Luis to
the hospital. He was given insulin injections and large volumes of intravenous electrolyte solution
containing bicarbonate. Further studies failed to find infection or another underlying cause for his
fever.
Within 24 hours, Luis' blood pressure had returned to near normal. His breathing slowed to normal
and he became fully awake and coherent. Blood glucose had decreased but was not yet in the
normal range. Blood pH, bicarbonate and PCO2 also had returned to near normal. Urinalysis
revealed a stronger yellow color, less acidic pH, slightly positive glucose, and negative ketone
bodies. He was discharged on the third hospital day.
Although he is confused, Luis is still able to maintain bladder control despite the presence of a

Dear Dr. Kornbluth and Mr. Gorenberg,
The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. The Committee on Oversight and Accountability is the principal oversight committee of the US House of Representatives and has broad authority to investigate “any matter” at “any time” under House Rule X.

- 1. Integrated Health was a large private, nonprofit health care system located in Tempe,Arizona. A year ago, the decision was made to implement a computerized medication administration record (MAR) at Integrated Health. During the first year, the MAR system would be implemented at Central Hospital of Tempe. After the bugs were worked out, the computerized MAR would be implemented system-wide. The MIS department at Central Hospital was designated as the initiator and direction setter for the project. Art Smith, the Chief Information Officer at Central Hospital assigned Kate Cohen, a programmer/analyst, as project leader. Kate had the responsibility for developing and implementing the MAR project at Central. Kate did the programming work for the MAR and assembled a team from Pharmacy Services, Nursing Services, and Internal Auditing. Members of the project team provided feedback on the software, made suggestions related to user training, and worked on an implementation schedule. Both team members and outside observers recognized the difficulty in developing and implementing the MAR in one year. Nurses antagonism toward the project surfaced after their suggestions and recommendations were rejected without explanation. The MAR project was also affected when a management shakeup occurred during the project development phase. The MAR was implemented and immediately failed. By 10:00 a.m. on the first day of the implementation, users complained that the system did not work and was too complicated. After hearing nurses complaints, physicians expressed concern about patient safety. At 1:30 p.m., Central Hospitals computer system crashed, and the MAR project was suspended. Early the next morning, we find Kate contemplating what she is going to say when she meets with Central Hospitals management group later in the afternoon.
