Integrated care occurs when healthcare organizations work together to meet the needs of the local population by addressing fragmentation in patient services. The goal is to enable better coordinated and continuous care for an aging population with increasing chronic diseases. For care to be integrated, organizations and care professionals must combine all of the different elements of care needed by a person.
