Scenario and Summary
This lab introduces the next step in creating a data model, the Entity Relationship Diagram (ERD). You will be given a business scenario for a University Medical Center, which is a small community hospital. The business specifications will outline a number of things about the business, some of which will apply directly to the database you are being asked to model. There is a table that lists the entities (tables) that will be needed for the database and related attributes (columns) for each entity. There is also a column that lists specific information about the entity that will be helpful in determining its relationship to other entities within the model.
Be sure to include the minimum and maximum occurrences of each relationship (cardinality) and to supply a name to the relationship that will work in both directions. Make sure to use Crow’s Feet notation in your ERD.
Narrative/Case Study
The University Medical Center is a small, community hospital. A new hospital administrator has recently been hired by the Board of Directors, and directed to right-size patient care and pharmacy services and improve profitability. The hospital operates three clinical facilities: the main hospital, a mid-town clinic, and an Urgent Care location. The hospital also offers selected in-home care services. Many of the patients are repeat or regular patients who receive regular treatment for various conditions, and many utilize the hospital’s pharmacy services for prescription medications.
The pharmacy dispenses about 3,000 different prescription medications of various kinds. Every prescription is associated with one patient, and is logged by the dispensing clinic. The new administrator wants to know which drugs are most prescribed, and also which are the most profitable.
The following is some general information about the organization and its current processes.
· The hospital operates three clinical facilities.
· A healthcare worker logs in at a facility at the start of a shift and logs out at the end.
· The name, address, Social Security number and other information is recorded for every healthcare worker.
· All healthcare workers have one billing rate, which is determined by their job description.
· Each medication dispensed by prescription is linked to both the prescription number and the medication ID number, recording the item price and the quantity dispensed.
As a convenient and affordable means of providing hospice care, palliative care, and convalescent care to patients that need occasional/intermittent skilled nursing, but do not require hospitalization, the hospital offers limited in-home care. Recording of in-home care includes the healthcare worker ID, their departure time and return time, and also the prescription ID for any prescriptions administered by the healthcare worker in the home setting.

Situation
Care4US (Care) is a major health insurer operating in the western part of the United States. It is interested in entering the east coast marketplace and is trying to determine which segment of the health care market it should initially target.
Care’s current market has four segments, described below:
Retail: The retail segment covers private individuals. Care, which does not sell directly to individuals, services this market through Health Insurance Marketplaces, more commonly known as health exchanges. These exchanges are mandated under the Patient Protection and Affordable Care Act (ACA), and are usually state-run organizations offering government regulated and standardized health plans to those eligible for federal subsidies through the ACA. States that decline to offer their own health exchanges offer this insurance through the federal program. Care only offers insurance through the state exchanges, not the federal government, and contracts with each state in its market area offering an exchange. There are six state exchanges Care would be negotiating with in the new market. While many insurers initially signed up to offer insurance through these exchanges, some are finding that the regulations are making it unprofitable for them to offer insurance and are withdrawing from the exchanges. There is limited market growth data as the health exchanges have only been in operation since they were implemented in 2014, but enrollment did grow significantly between 2014 and 2015. Overall this market is expected to be counter-cyclical with the economy, i.e. when the economy is good and unemployment lower, more people are expected to have insurance through their employers and fewer from the health exchanges, and vice versa.
Employer: Most people have health insurance through their employers. Insurance companies offer a variety of group plans to companies and organizations, which choose the options best (and most cost-effectively) suited for their workforces. It should be noted that under the ACA, employers with more than 50 employees are required to offer health care to their employees; those who work for employers with less than 50 employees who do not offer health care can obtain it through the health exchanges. Care offers a variety of products to companies and organizations using its own sales force, who sell directly to employers in Care’s market. Insurance sales to employers took a jump under the provisions of the ACA as smaller employers were forced to add health insurance for their employees; sales have leveled off since then and are expected to follow economic and employment trends.
Military: The military provides health benefits to active personnel, retirees, and dependents through Tricare, a Department of Defense (DoD) administered program. The DoD contracts with private companies on a regional basis to provide this insurance. Insurers negotiate directly with the DoD to be the sole Trica.

In the health care industry quality of care means everyone participating in ways to improve health care such as health care professionals, patients and their families, researchers, payers, planners and educators. These changes lead to better outcomes in health, a better system performance in care, as well as better professional development.
When you describe quality, it’s the process for making strategic choices in health systems for quality assurance in health care and decision making. Although there are many outcomes to improve quality of care, the main concern is accomplishing a goal that will be beneficial for the future.
Good quality means providing patients with appropriate services in a technically competent manner, with good communication, shared decision making, and cultural sensitivity. In practical terms, poor quality can mean too much care (e.g., providing unnecessary tests, medications, and procedures, with associated risks and side effects), too little care (e.g., not providing an indicated diagnostic test or a lifesaving surgical procedure), or the wrong care (e.g., prescribing medicines that should not be given together, using poor surgical technique).
Quality can be evaluated based on structure, process, and outcomes (Donabedian 1980). Structural quality evaluates health system characteristics, process quality assesses interactions between clinicians and patients, and outcomes offer evidence about changes in patients' health status. All three dimensions can provide valuable information for measuring quality, but the published quality-of-care literature reveals that there is more experience with measuring processes of care.

Care4US (Care) is a major health insurer operating in the western part of the United States. It is interested in entering the east coast marketplace and is trying to determine which segment of the health care market it should initially target.
Care’s current market has four segments, described below:
Retail: The retail segment covers private individuals. Care, which does not sell directly to individuals, services this market through Health Insurance Marketplaces, more commonly known as health exchanges. These exchanges are mandated under the Patient Protection and Affordable Care Act (ACA), and are usually state-run organizations offering government regulated and standardized health plans to those eligible for federal subsidies through the ACA. States that decline to offer their own health exchanges offer this insurance through the federal program. Care only offers insurance through the state exchanges, not the federal government, and contracts with each state in its market area offering an exchange. There are six state exchanges Care would be negotiating with in the new market. While many insurers initially signed up to offer insurance through these exchanges, some are finding that the regulations are making it unprofitable for them to offer insurance and are withdrawing from the exchanges. There is limited market growth data as the health exchanges have only been in operation since they were implemented in 2014, but enrollment did grow significantly between 2014 and 2015. Overall this market is expected to be counter-cyclical with the economy, i.e. when the economy is good and unemployment lower, more people are expected to have insurance through their employers and fewer from the health exchanges, and vice versa.
Employer: Most people have health insurance through their employers. Insurance companies offer a variety of group plans to companies and organizations, which choose the options best (and most cost-effectively) suited for their workforces. It should be noted that under the ACA, employers with more than 50 employees are required to offer health care to their employees; those who work for employers with less than 50 employees who do not offer health care can obtain it through the health exchanges. Care offers a variety of products to companies and organizations using its own sales force, who sell directly to employers in Care’s market. Insurance sales to employers took a jump under the provisions of the ACA as smaller employers were forced to add health insurance for their employees; sales have leveled off since then and are expected to follow economic and employment trends.
Military: The military provides health benefits to active personnel, retirees, and dependents through Tricare, a Department of Defense (DoD) administered program. The DoD contracts with private companies on a regional basis to provide this insurance. Insurers negotiate directly with the DoD to be the sole Trica.

In the health care industry quality of care means everyone participating in ways to improve health care such as health care professionals, patients and their families, researchers, payers, planners and educators. These changes lead to better outcomes in health, a better system performance in care, as well as better professional development.
When you describe quality, it’s the process for making strategic choices in health systems for quality assurance in health care and decision making. Although there are many outcomes to improve quality of care, the main concern is accomplishing a goal that will be beneficial for the future.
Good quality means providing patients with appropriate services in a technically competent manner, with good communication, shared decision making, and cultural sensitivity. In practical terms, poor quality can mean too much care (e.g., providing unnecessary tests, medications, and procedures, with associated risks and side effects), too little care (e.g., not providing an indicated diagnostic test or a lifesaving surgical procedure), or the wrong care (e.g., prescribing medicines that should not be given together, using poor surgical technique).
Quality can be evaluated based on structure, process, and outcomes (Donabedian 1980). Structural quality evaluates health system characteristics, process quality assesses interactions between clinicians and patients, and outcomes offer evidence about changes in patients' health status. All three dimensions can provide valuable information for measuring quality, but the published quality-of-care literature reveals that there is more experience with measuring processes of care.

Here you will look for a particular element that occurs in the object (an object, action, gesture, pose) and explain either:
when that same element occurs in other objects through history and how this object’s representation of it is unique, or
what that element means generally in art or to art historians—in other words, the traditional association an art historian might make between that depiction and some other thing.

John and Natalie meet and discuss her business. John suggests that he do the tasks listed below for Natalie.
Hold cash until there is enough to be deposited. (He would keep the cash locked up in his vehicle). He would also take all of the deposits to the bank at least twice a month.

Record all of the deposits in the accounting records.

Prepare the monthly bank reconciliation.
Transfer all of Natalie’s manual accounting records to his computer accounting program. (John would maintain all of the accounting information that he keeps for his clients on his laptop computer.)
Prepare monthly financial statements for Natalie to review.
