10. The ICD-10 is divided into chapters, based on diseases of body systems, special
diseases or external factors.
The Chapters are further divided intoblocks which group together categories having
some common factor.
Blocks are divided into categories represented by 3-character codes, and
these in turn may be subdivided into specific 4-character subcategories. Certain
codes also have optional supplementary characters to add more detail.

27. Medicine
• Medicine : meaning the art of therapy; it is the science
that combines the experiences of humanity in caring for
the human being (or animal), and the suffering of
diseases, diseases and injuries that affect his body or

Blocks & Categories:
Each chapter is subdivided into BLOCKS
of 3-character CATEGORIES.
Categories:
The first character is always a letter of the
alphabet (A – Z), followed by two numeric
characters (0 – 9).
Examples: A28, J20, K34, etc.

30. ICD-10 and its Applications
The ICD (or International Classification of
Diseases) is a statistical classification of
diseases, injuries and causes of death.
Produced by the WHO (World Health
Organization). It is a unique code to be
assigned to any disease or condition diagnosed
by physicians or other paramedical personnel.
The codes are designed to be used for statistical
purposes.

31. Introduction
The 10th Revision of the International Classification
of Diseases and Related Health Problems
(ICD-10) has been introduced by the WHO in
1990.
It has been produced for comparing international
health care statistics, specially for:
a) Assisting record retrieval, and
b) Generating health care statistics

Volume 1 is divided into 22 chapters:
• Chapters I – XVII provide codes to all diseases
• Chapter XVIII covers Symptoms & Signs
• Chapter XIX codes for physical injuries & poisonings
• Chapter XX provides codes only for EXTERNAL
causes of morbidity & mortality
• Chapter XXI compiles codes for all factors influencing
health status & contact with health services of a person
NOT currently sick (eg. Fitness tests, immunizations,

subdivided by means of a fourth, numeric
character (0 – 9), after a decimal point,
allowing up to 10 SUB-CATEGORIES.

Five-character additional sub-categories:
ICD-10 allows certain conditions to be classified at the 5th Character level.
These additional SUB-CATEGORIES are found in:
i) Chapter XIII
Subdivisions by anatomical site (M46.25)

Subdivisions to indicate open & close fractures. Also, internal injuries,
with or without open wound (S42.21)
iii) Chapter XX

NEC (Not elsewhere classified):
Found usually in the Index it is a warning code telling you
to use it ONLY when you can’t find a more suitable or
appropriate code.
eg. “Pneumonia due to other infectious organisms,
NEC (J16)”: Elsewhere classified more suitably
as:
• J16.0 Chlamydial pneumonia
• J16.8 Pneumonia due to other specified infectious
organisms.
• P23.- Congenital pneumonia
• J18 Pneumonia, organism unspecified.

shows an incomplete code, i.e. “X43.- ”
You must find the 4th character before putting

55. 5. Dagger & Asterisk signs (†) (*):
Some categories show these 2 signs:
eg. Diabetic neuropathy (E25.0 †, GO1 *),
Tuberculosis meningitis (A17.0 †, G02*)
Dagger (†) denotes etiology or cause,
while asterisk (*) shows effect or appearance.
If only one code has to be shown, then the dagger code
(†) has preference.
The asterisk code, if used, must never be used alone.

64. Volume 3 (Index)
This Volume (Index) is divided into 3 sections, as follows:
i) Section I
Lists all terms belonging to chapters I – XIX and
Chapter XXI (“Contact with health services”, the “Z”
codes) and Chapter XXII.
ii) Section II
Lists all terms classifiable to chapter XX (“External
Causes of Morbidity & Mortality”, the “VWXY” codes).
Section III
The Table of Drugs & Chemicals. This table lists, for
each substance, the codes for poisonings and adverse
effects of drugs given in a correct dose.

65. Use of Index In Coding
i) LEAD TERM:
• Section I and II of the Index are organized in columns
• In each column, the term shown on the far left is called the Lead
Term.
• These Lead Terms are usually nouns, giving the name of a disease
or condition (eg. Sickness, infection, defect, absence, complication,
contraction, fracture etc.)
• Before coding a diagnosis you must identify the disease condition
and use this noun as a Lead Term
• If the diagnosis is expressed in an adjective form, convert it into a
noun to obtain the lead term (eg. Sick, infected, defective, absent,
complicated, contracted, fractured etc.)

67. ii) QUALIFIERS or MODIFIERS :
Qualifiers or modifiers further describe the Lead Term.
1. They refer to the site (“Fracture of Skull”), variety (“Hepatitis,
Type A”) or circumstance (“Drowned While Swimming”).
2. In the index, these modifiers are “Indented” (separated by one or
more spaces) to the right, under the Lead Term.
3. There may be more than one modifiers (“Open Fracture of
Skull”).
4. Try to find as many of the modifiers as you can in the Index
which match exactly to the diagnosis that the doctor has
recorded.
5. Select that code which does full justice to all qualifiers or
modifiers present in the diagnosis.
6. Now turn to Volume 1 (Tabular List) and confirm that the
selected code is the same at both place.

70. 1. Identify the Lead Term.
2. Find it in the appropriate section of the Index.
3. Look under the Lead Term for all modifiers and
qualifiers.
4. Try to match the chosen code with the recorded
diagnosis of the doctor.
5. Note the code which you found that does full justice to
the diagnosis.
6. Look in Volume I (Tabular List) that this is the correct
code before entering it on the form.

The Volume 3 is a vital part of the
classification. Without using the Index you
will find it extremely difficult to locate the
correct codes to use.

72. • The Index contains all the codes found in
Volume 1.
• It contains more diagnostic terms >
Volume1, reflecting the many & varied ways
that diagnosticians describe diseases.
• However, it is not possible to include all
possible variations of each diagnostic term
in Volume 3, so you must also use Volume 1.
• In the Volume 1, British spelling is used but
in Volume 3, American spelling conventions
are used.
