I. Personal Data / Identifying Data

        The subject’s name was Federico B. Jabol, twenty-years of age, born on October 24,
1990 at East Avenue Medical Center. He was second of the eldest of Edna and Efren Jabol. He
was male, a purely Filipino Citizen, and a Born-Again Christian and lives at 185 Yellow Bell st.
Barangay Commonwealth, Quezon City. He was a Fourth Year College Student at the
Polytechnic University of the Philippines and tooked up Bachelor of Science in Clinical
Psychology. The person to be notified in case of emergency was his oldest sister, Janice B. Jbol
and currently resides at Room 4-C Padilla Bldg. Arellano St. Sta. Mesa, Manila. The subject was
accompanied by third of the oldest sister, Girlie B. Jabol last June 29, 2010 at 6:29 in the
evening.
        When first seen, the subject refused to be treaedt but when motivated by her sisters, he
become relaxed and likes to be treated. He is friendly, courteous, and humorous when making
conversation to people.

II. Chief Complaints

•   According to the Subject – “ Bakit niyo ba ko dinala dito? Ayokong maiwan dito baka
    mabaliw ako kaya isama ninyo ako”.

•   According to the Mother – “Naku, pasaway iyan. Ayaw kasing magpaiwan niyan dahil gusto
    niyan lagging may kasama”.

•   According to the Friends – “ Mabait sana siya, pero kapag may group activities sa
    eskuwela, hindi siya makagawa. Gusto lang niya magpresinta”.

III. History of Present Illness

          14 years PTA, the subject’s father was died due to a land dispute which was killed by his
own grandfather with a gun shot at Naga City, Isabela. At that time, the subject was trained by his
father on how to play a guitar. His expectation towards his father was high that when he grew up,
he will become a good but a brave person who can defend himself against suspicious-looking
person. Unfortunately, the subject loses hope, confidence and become frustrated because of his
high expectation from his father. After this unpleasant experience which the subject encountered,
he always wanting his mother to stay with him because he is preoccupied with fears of being left
to take care for him. He doesn’t want to encounter that circumstance because it may reminisce
the pain suffered by his father.
          13 years PTA, when the subject turns into Grade 1, his mother always accompanied him
because he has fear of separation. His mother observed that his son cannot cope up from his
father’s death and he deeply take it. To avoid from being anxiously feared of being detached, her
presence serve as a motivation for her son so that he will become confident to do activities in
their school. He likes to play basketball, but when he accidentally threw the ball into one of his
classmates, he was knock-off to his face. He can’t fight back, instead he tells this to his mother.
          12 years PTA, the subject’s mother needs to work as government employee. The subject
began to cried and hugs his mother claimed as “ Ma, huwag ka na lang umalis”. When his mother
left their house, he was cried until night time. He was forcefully preventing by his brother because
he will eventually run out to their house. After that, the subject assumed responsibility for his
brother and sisters for most major areas of his life, such as doing his assignments which they
acted temporarily as parents to the subject.
          11 years PTA, the subject was knocked off by his brother because of his demanding
attitude which serve as discipline for him. The subject was pressures to study, but difficulty
initiating projects because he lacks self-confidence of his own ability. His sisters were always
motivating him to study, but inadequate to substantiate the own nurturance and love from his
mother.
          10 years PTA, the school of the subject had their foundation day. The subject was
volunteered as a leader in their section to present their play which theme is about mother’s love.
When he heard that theme, he refused and being scolded by his classmates. He has excessive
volunteering to do things that are unpleasant. Until his sisters noticed some changes in the
subject’s behavior. According to them, “ Masyado niyang dinamdam yung pagkawala ni Papa. At
isa pa, yung pagkamatay ng tatay namin at yung pagpasok ng mama namin sa trababo nawalan
siya ng ganang mag-aral at mahilig dumepende”. This suggests that detachment from his mother
and father greatly influenced the subject’s behavior and productivity in school.
          8 years PTA, it was the subject’s graduation day. The subject was expected that his
mother will come but unfortunately she did not come because of heavy task from her work. His
brother and sisters accompanied him, but he was not happy. Instead of celebrating he began to
depressed and on that special day. He temporarily isolated himself in the room and began to cry.
          7 years PTA, the subject was accompanied by his brother for the enrolment in high
school. At first, he was shy and timid but when entertained by his classmates he becomes friendly
and humorous. But when it comes to group activities, he tends to be irresponsible. One of his
classmates noted that “Masaya siyang kasama kaso walang words of honor pagdating sa mga
activities”. He has difficulty expressing disagreement with others because of fear of loss of
support and approval. He cannot express making confrontation with people because he fears of
being loss of one of his friends. Because they are the ones who helped the subject in his stressed
in life.
          6 years PTA, the subject asked his brother on how to engage into relationship. In their
classroom, he was always rejected because of his passive aggressive behavior except the girl
named Celine. She always motivated the subject to avoid inducing inferiority feeling. As a result,
the subjects achieve good grades and obtain confidence for him. But oftentimes, he has difficulty
In making decision on what would be prioritized between love and academic affairs. Their
relationship ends because of the subject’s unusual behavioral condition.
          4 years PTA, the subject was engage into another relationship because he feels that he
is helpless or uncomfortable when alone. This time, he got more committed with his girlfriend
named Pie. Although instances of lying were initiated by his girlfriend, he cannot end their
relationship because of his mutual attachment to that girl.
          1 year PTA, his girlfriend ended their relationship which the reason was uncertainty and
emotional immaturity to each other. The subject noted that he risk all of his effort, time, and love
just to show proved his sincerity to his girlfriend. He got frustrated in which he feels that he would
like to gone on earth. After that consequence, he urgently seeks for another relationship to help
him in alleviating his depression. Their relationship was lasted for almost 2 years and the subject
was tried to cope up in the situation.
          1 week PTA, his sisters decided to bring the subject in the psychologist to find out what is
really happening to their elder brother.

The subject was expected that his mother will come but unfortunately she did not come because of heavy task from her work. His brother and sisters accompanied him, but he was not happy. Instead of celebrating he began to depressed and on that special day. He temporarily isolated himself in the room and began to cry. 7 years PTA, the subject was accompanied by his brother for the enrolment in high school. At first, he was shy and timid but when entertained by his classmates he becomes friendly and humorous. But when it comes to group activities, he tends to be irresponsible. One of his classmates noted that “Masaya siyang kasama kaso walang words of honor pagdating sa mga activities”. He has difficulty expressing disagreement with others because of fear of loss of support and approval. He cannot express making confrontation with people because he fears of being loss of one of his friends. Because they are the ones who helped the subject in his stressed in life. 6 years PTA, the subject asked his brother on how to engage into relationship. In their classroom, he was always rejected because of his passive aggressive behavior except the girl named Celine. She always motivated the subject to avoid inducing inferiority feeling. As a result, the subjects achieve good grades and obtain confidence for him. But oftentimes, he has difficulty In making decision on what would be prioritized between love and academic affairs. Their relationship ends because of the subject’s unusual behavioral condition. 4 years PTA, the subject was engage into another relationship because he feels that he is helpless or uncomfortable when alone. This time, he got more committed with his girlfriend named Pie. Although instances of lying were initiated by his girlfriend, he cannot end their relationship because of his mutual attachment to that girl. 1 year PTA, his girlfriend ended their relationship which the reason was uncertainty and emotional immaturity to each other. The subject noted that he risk all of his effort, time, and love just to show proved his sincerity to his girlfriend. He got frustrated in which he feels that he would like to gone on earth. After that consequence, he urgently seeks for another relationship to help him in alleviating his depression. Their relationship was lasted for almost 2 years and the subject was tried to cope up in the situation. 1 week PTA, his sisters decided to bring the subject in the psychologist to find out what is really happening to their elder brother.
