The doctor is in. Do you
have an idea why some
school doctors and
nurses visit your school
and do some procedures
concerning you?
I. WHAT HAPPENED?

4. Today, you will learn
those examinations that
you should go regularly
in order to keep on
check of your health.
II. WHAT YOU NEED TO KNOW?

5. Health Appraisal
Provides important information on the
pupil’s current health status. It
provides teachers ideas if the pupil
is fit to participate in school
programs. It gives teachers ideas if
they need to modify school activities
so pupils could benefit from it.

6. Here are the following
components that are
part of your school
health appraisal.

7. Health history
Your school will keep a record of
your history like immunizations,
childhood illnesses, allergies,
previous hospitalization,
medications used, significant
accident or injuries, surgery, and
chronic illness.

9. A record of your family history
This includes the age and general
health of parents and sibling. In
addition, the following will be
recorded too.
a. Education level of parents
b. History of family disease
c. Physical environment

10. Current status
a. Dental
b. Family medical problems
c. Child’s health problems/
allergies
d. Behavior

11. Screening Procedures
are supplemental evaluations of:
a. Vision
b. Hearing
c. Blood Pressure
d. Height and weight
e. Urine specimen for glucose and
protein as determined by
school/district/policy guidelines
