You are a survey taker, and your task is to create an
Excel spreadsheet survey by surveying the class to
determine which was their favorite lunch food. The
challenge involves gathering data and then displaying
that data in an Excel spreadsheet. By letting your
classmate and the school cafeteria manager know which
food turned out to be the class favorite, You will create
an Excel spreadsheet using the data you obtained and
share it in a letter to the cafeteria manager. Your product
must meet the following standards: the letter is written
correctly and contains the correct data displayed in
Excel and the chart.

STUDENT INTERVIEW: A Student Interview is based on the belief
that, through conversation and a student’s demonstration of his

We can use checklist to assess procedures as well as
the product or student’s output. Example: Suppose we
want to assess if pupils can use geometrical
instruments effectively.
OBSERVATION CHECKLIST
•Is a strategy to monitor specific skills, behavior, or
dispositions of individual students or all of the students
in the class?
•It is also a record keeping device for teachers to use to
keep track of who has mastered the targeted skills and
who still needs help.
WHY USE OBSERVATION CHECKLIST
•Provides a quick and easy way to observe and record
many of the skills, criteria, and behaviors.
•Shows the area of concern early
•Provide the opportunity to "change gears" in a
classroom if a large percentage of students are not
doing well
HOW SHOULD WE USE OBSERVATION CHECKLISTS?
(1) Individual checklist
• Checklists that are used with students on an individual basis
• May be done one-on-one or may be done during class activities
(2) Whole Group checklist
•Basically, the same as the individual, but is more generic or global
in scope of material.
•Criteria usually consist of degree of performance. Such headings
would include, "Not yet mastered", "Beginning of mastery", and
"Mastery level performance"

•Is a checklist that can be used by a student
and/or class that outlines the steps necessary to
complete simple or complex math problem

algorithm for solving math problems
•Help students to master simple and complex
algorithms.
INTERVIEW
•A two-way method which permits an exchange of ideas
and information
•As a technique, it is a conversation carried out with the
definite purpose of obtaining certain information by
means of the spoken word.
•Is an opportunity for the teacher to probe the student's
understanding of a topic in ways that cannot be used
effectively with other forms of assessment

• A tool which is also called the conference recording
form. Interview sheets consist of a list of questions the
teacher intends to ask and space for recoding the
