A 37-year-old male presented with testicular swelling and underwent a right orchidectomy. Microscopic examination of the testicular parenchyma revealed granulomatous orchitis. Granulomatous orchitis can be caused by infections such as tuberculosis, atypical mycobacteriosis, leprosy, and syphilis. Differential diagnoses include pyogenic epididymo orchitis, seminoma with granuloma, lymphoid orchitis, and testicular lymphoma.
