provide. Each of my prior education, experiences, licensures

consumers.

Treatment Program: This gave me a background in dealing
withcriminal thinking, institutionalization, substance abuse
and mental health disorders.
3. TurningLeafAdult Residential Treatment Program:
This experience further developed my diagnostic and
evaluation skills, family counseling, individual and group
therapy abilities.
Specific Training:
Iam currently involved inthe yearlong Trauma Focused –
Cognitive Behavioral Therapy trainingthroughMOACTS to
deal withthe trauma experienced by many of our consumers.
Funding Sources:
Iam currently a Medicaidprovider allowing me to provide
assessment, individual counseling and family counseling for
those on minors on Medicaid.
In additiontothis this Ialso am approved to see adults under
the Children’s Division’s CTSprogram andin process of
being approved tosee Missouri Alliance consumers though
Great Circle and Multi Plan insurance. Further we have a
sliding scale and will make arrangements with consumers
who are covered by other insurance providers.
4. How doIrefer a consumer andwhat canIexpect?
Unless you as a referral source request another method of
referral and follow upthe following has been our practice:
Most often a referral can be made with a phone call, email or
fax. After a referral is made to us we will normally attempt to
contact the consumer the same day toset upan initial
assessment appointment intheir home and ifnocontact was
possible we notify you for follow up.
As soon as the initialvisit occurs, anda release of information
is signed, we will notify you as a referral source, most often by
emailthat the consumer has engaged. After wards, at your
convenience, either weekly or bi-weekly a briefcontact will
be made withyou to exchange information and ensure the
ongoing engagement and positive progress of the consumers.
