6. Uitgangspunten DRN voor Computer Tomografie
• Om goed aan te sluiten bij huidige praktijken voor het
vaststellen van blootstellingen bij Computer Tomografie wordt
bij deze verrichtingen gebruik gemaakt van dosimetrie met
fantomen.
• Voor CT zijn de diagnostische referentieniveaus afgeleid van
dosismetingen met een CT dosis fantoom en een 100 mm
lange CT-ionisatiekamer (voor CT onderzoek van de romp
wordt een cilindervormig fantoom met een lengte van 150 mm
en een diameter van 320 mm toegepast).
• De dosiswaarden CTDI en DLP worden bij moderne scanners
als indicatie voor elk onderzoek en als dosiswaarden na het
onderzoek op de bedieningsconsole weergegeven.
8. Verrichtingen
• Als gangbare representatieve CT-onderzoeken zijn
respectievelijk gekozen:
• CTA-thorax (klinische vraagstelling longembolie) en
• CT-buik/bekken (klinische vraagstelling acute buik)
• Het referentieniveau voor zowel CT-angiografie van
de thorax, als het referentieniveau bij CT-
buik/bekken heeft betrekking op één enkele serie.
9. Bron voor de diagnostische referentie niveaus
• De referentieniveaus zijn onder andere ontleend aan het
ZonMW demonstratieproject patiëntendosimetrie, een
Europese veldstudie en een CT-IAEA-coordinated Research
Project.
• In het kader van deze studies werd in 46 ziekenhuizen
dosimetrische data verzameld voor een groot aantal
verrichtingen.
• De haalbare niveaus zijn ontleend aan een publicatie van Van
der Molen et al. uit 2006 en een CT-IAEA-coordinated
Research Project.
12. Verificatie dosisindicatie van de scanner
• Bij de ingebruikname van nieuwe CT-scanners is
het gebruikelijk de CTDI daadwerkelijk te meten.
• Deze gemeten niveaus dienen om de output van de
CT-scanner te toetsen aan de specificaties maar
ook om de nauwkeurigheid van de indicatie van
CTDI en DLP op de bedieningsconsole te
controleren.
• Voor het meten van CTDI-niveaus en het berekenen
van DLP wordt verwezen naar IEC 60601-2-44

22. Automatic Exposure Control (AEC)
• Voor de CT-scanner geldt dat deze in de nabije toekomst de
dosis gedurende de scan steeds vaker zal gaan variëren
afhankelijk van de verzwakking die de röntgenstraling
ondervindt in de patiënt (aangeduid als Automatic Exposure
Control (AEC)).
• Daar in Europa nog geen niveaus en methodieken m.b.t.
DRN’s gedefinieerd zijn die toepasbaar zijn op de deze
relatief nieuwe techniek is AEC bij CT in deze rapportage
buiten beschouwing gelaten.
• Overigens wordt verwacht dat toepassing van AEC bij CT in
de praktijk, voor een patiënt met een gemiddeld postuur, zal
leiden tot dosisreductie.
