Background
Hisham Soliman, MPH, MD
Dr Soliman, has graduated from Ain-Shams University,
Medical School in Cairo, Egypt. He moved to California,
where he finished his residency training with University of
Caliornia,SF, the San Juaqin Valley affiliate. Dr Soliman,
has a lot of hands on experience in wide variety of settings.
He has worked or trained through community hospitals,
crisis residential, consultation-liasion, forensic psychiatry
settings, county and community outpatient clinics,
convalescent hospitals, dementia units, Nursing homes,
and private practice.
He has had training in Psychopharmacology,
Psychotherapy, Child Psychiatry, Geriatric Psychiatry and
many other areas.
Dr Soliman is particularly highly skilled in the art of
Psychopharmacology and medications use concomitantly
and synergistically to relieve the symptoms of patients.
Dr Soliman, believes that both patient and doctor are part
of management team to make the desicions regarding
treatments. He believes that its quite important to develop a
therapeutic relationship and rapport with the patient for the
medications to make an effect. Dr Soliman is also available
for more extensive modes of therapy for select patients. As

This packet can also be made available to you in our

● A list of current medications.
It is also helpful for the physician to know what
medications you have tried in the past.
● Any history or mental health records
you may have in your possession.
If you do not have copies of your mental health records,
we can request records from previous providers, if you
sign a release.

sign one ahead of time by filling out the release in the
new patient paperwork on our website.
Please fax your signed release to the office in which you
are scheduled.
A list of office phone and fax numbers is located under

● Any lab test results from lab work
performed in the last year.
If you do not have copies of your lab results, you may
request that the results be
sent to our office prior to your first appointment by
contacting your primary care
physician or the lab where your tests were done.
● Any psychological test results you
may have in your possession.
Your physician will want to review the results of any
psychological testing prior to prescribing medication.
These tests are not a requirement, but will aid your
physician in deciding the best course of treatment.
