Antiarrhythmic drugs
Antiarrhythmic medications prevent and treat
abnormal heartbeats (arrhythmias)
There are four main groups of antiarrhythmic
medications: class I, sodium-channel blockers;
class II, beta-blockers;
class III, potassium-channel blockers; -prolong
action potential
class IV, calcium-channel blockers; and
miscellaneous antiarrhythmics, or unclassified
antiarrhythmics.
25
26. CON...
Class I, sodium channel blockers: These drugs prevent
sodium from getting through cell membranes. This can slow
electrical impulses in the heart muscle. Examples include
disopyramide, flecainide, mexiletine, propafenone and
quinidine
Class Ia: Fast sodium (Na) channel blockers
Ib -Lidocaine, phenytoin, mexiletine (depress phase 0
selectively in abnormal/ischemic tissue, shorten
repolarization)
Ic -Flecainide, propafenone, moricizine (markedly depress
phase 0, minimal effect on repolarization) 26
27. CON...
Class II, beta blockers: These drugs slow down the heart
rate, often by blocking hormones such as adrenaline.
Examples include acebutolol, atenolol, bisoprolol,
metoprolol, nadolol and propranolol.
increasing action potential duration and the effective
refractory period
27
28. CON...
Class III, potassium channel blockers: These drugs
prevent potassium from getting through cell membranes.
This can slow down electrical impulses in all of the heart’s
cells.
prolongs repolarization without affecting conduction velocity
by blocking one or more time-dependent potassium
currents.
Examples include amiodarone
28
29. CON...
Class IV, non dihydropyridine calcium channel
blockers : These drugs block calcium channels in
heart muscle. This can decrease heart rate and
contractions.
decrease conduction through the AV node, and shorten
phase two (the plateau) of the cardiac action potential
Examples include diltiazem and verapamil.
Other antiarrhythmic drugs :
Adenosine: This medication can block or slow down
electrical impulses at the atrioventricular node, between
the upper and lower chambers of the heart.
29
33. CON...

Prevent synthesis of cholestrol in liver
Eg atrovastatin
34
35. DRUG CALCULATION
Drop rate
Drip Rate (dpm) =Volume of IV fluid (mL) x drops/ml(drop
factor)
Time to run (h) 60 min/h
Drip rate (mL/hour) = Total volume (mL) / Infusion time
Situation: The patient is ordered an IV of 1000ml of Normal Saline
over 12 hours with drop factor of 15 drops/ml
Calculate
rate(ml/hr)=1000/12=83.3ml/hr
drip rate =1000 x 15= 20.83drip/min
12x60 35
36. CON....
Heparin Infusion Rate: Total Units (in IV bag) =units/hr
Total volume (ml) x
eg .Heparin infusion is to start rate of 1350 units/hour at
per the practitioner’s order. The heparin infusion comes
in a 500ml bag with 25,000 units. Calculate the starting
rate of the infusion (ml/hour).
36
37. CON...
25000 Units= 1350 units /hr
500ml x
50x=1350ml/hr
x=27ml/hr
An IV with nitroglycerin is infusing at 8 mL/hr. The
concentration of the IV is 50 mg in 250 mL of D5W. How
many mg/hr is the patient receiving?
50mg divided by 250 ml = 0.2 mg x 8ml/hr = 1.6mg/hr
37
38. CON
RATIO AND PROPORTION METHOD
D= Desired dose or dose ordered by the primary care
provider.
H = dose on hand or dose on the label of bottle, vial,
ampule.
V = vehicle or the form in which the drug comes (i.e.,
tablet or liquid).
38
39. CON....
STANDARD FORMULA
Desired dose(D)XVehicle (V)
dose on hand(H )
Order: Amoxacilline 500 mg
On hand: Amoxacilline 250 mg in 5 mL
Desired (D) = 500 mg
On hand (H) = 250 mg
Vehicle (V) = 5 mL
39
40. CON...
Rate of Infusion
Ml/hr =desired dose x pt weight x60
drug concentration
For Noradernaline 4/50mglml =80mcq/ml
For dopamine concentration 200mg/5ml=4000mcq
For doubtamine concentration 250mg/5ml=5000mcq

Diseases of cardiovascular system;CHF,CAD,Hypertension,Cardiac arrhythemia
Arteriosclerosis -It refers to several diseases in which the arterial wall thickens and loses its elasticity
Atherosclerosis is the buildup of fats, cholesterol and other substances in and on the artery walls
Positive Inotropes helps the heart to get more blood, help in cardiogenic shock, increase Cardiac output, slow heart example
Digoxin HF,irregular heart beat called A Fib,stablize heart rhytm by slowing over active electrical signal
React with phenytoin
Beta-1 receptors are located in the heart.
The beta-2 receptors are located in the bronchioles of the lungs and the arteries of the skeletal muscles
Beta 2 beta blockers act primarily on the lungs.
Alpha 1 receptors found on vascular smooth muscle. They determine both arteriolar resistance and venous capacitance, and thus BP
Alpha 2 receptors are found both in the brain and in the periphery. In the brain stem, they modulate sympathetic outflow.
Dopamine naturally occur in our body
Negative Inotrope:verapamil ,atenolol-rx of high blood pressure and chest pain
Mechanism of action antianginal drugs inhibits calcium ion movement across cell members, depressing contraction of cardiac and vascular smooth muscle
Sildenafil -treat pulmonary hypertension (high blood pressure in the blood vessels that supply the lungs).
Beta –blockers-causes vasoconstriction of blood vessels, decrease stimulation of sympathetic nervous system –so decrease cardiac out put3
Class 1 :Sodium ioms channel blockers (Membrane stabilizing agents)
IA depress 0 depolarization-quinidine
IB Minimal effect of o depolarization-Lignocaine
IC Mainly depress o depolarization
IB
