Local teams delivered nearly 700 programs to help children and adults live healthier lives, welcoming their 60,000th participant to the MEND program. They partnered with various organizations to launch health initiatives and conduct research on child obesity prevention. MEND also expanded their international reach by establishing partnerships and programs in the US and Canada.

We teamed up with various organizations to deliver health and wellness programs around the UK and establish partnerships to expand MEND's international reach. This included launching childhood obesity initiatives, bringing Olympic athletes to mentor communities, conducting research studies, and adapting programs for families in Canada and the US while delivering nearly 700 local programs and welcoming their 60,000th participant.

The AQS is important because it presents information for the public and patients about services in NHS Wales in an accessible, easy-to-read format. It’s easy to get bogged down in data and statistics, and if figures are presented without any context then it’s hard to know if they represent good performance or whether urgent improvement is needed.
The All-Wales AQS is hopefully easy to read. It contains great examples of excellent care in Wales and notes when things could be better. It also includes links to the Annual Quality Statements produced by all the health boards and trusts in NHS Wales. These publications are important because we all want to know how well our local health services are doing.
Scarlet Design coproduced the design of the All Wales AQS with the Welsh Government and 1000 Lives Improvement teams.

HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

MEND in 2011
- 1. Local teams helped children and adults to live fitter, healthier and happier lives, delivering nearly 700 programmes. We also welcomed our 60,000th MEND participant.
- 2. We teamed up with Diabetes UK to run Healthy Lifestyle Roadshows around the country. Graduates shared their experiences of how their lives had changed after MEND.
- 3. We hopped, skipped and jumped to a hopscotch world record with Tower Hamlets kids, as part of Bupa’s worldwide corporate volunteering day.
- 4. We worked with Bupa and NHS Tower Hamlets to launch Building Powerful Communities, a community wide approach to obesity in the borough.
- 5. With Sinitta, we launched the second National Childhood Obesity Week alongside Change4Life’s Really Big Summer Adventure.
- 6. We partnered with the Rugby Premiership to get six of their clubs delivering MEND programmes locally, encouraging their communities to get active and healthy.
- 7. We teamed up with the Dame Kelly Holmes Legacy Trust to launch a new mentoring project in Tower Hamlets. Former Olympic swimmer Adam Whitehead mentored children and families, helping to build the Olympic legacy.
- 8. We established the MEND International Research Group (MIRG), an independent group of leading academics, to guide MEND’s research strategy.
- 9. Our experts got busy developing new programmes and services and tailoring them to meet local needs: Momenta for adults, an under 5s service for NHS Ealing, MEND SEN and MEND Teens.
- 10. We wrapped up the £8 million Big Lottery Fund Wellbeing portfolio, after hitting our targets and achieving a status of ‘outstanding’.
- 11. We were chosen to deliver large-scale contracts for child weight management services in The Wirral and Buckinghamshire.
- 12. Our research team continued to build our evidence base, and we published our 29th MEND publication.
- 13. We were awarded a major CDC funded child obesity prevention grant in the USA to evaluate MEND’s family programmes using an RCT.
- 14. We presented data on over 15,000 children at the Obesity Society Conference in Orlando and the World Diabetes Congress in Dubai.
- 15. We formed new corporate partnerships. We were chosen as one of three national partners for the Johnson & Johnson Corporate Citizenship Trust to support families in the most deprived areas of the UK. We are also working with Kellogg’s Swim Active to fund swimming facilities and train staff for local MEND programmes.
- 16. We showcased our fantastic partnerships and spread positive health messages at the Autumn party conferences.
- 17. We grew our international reach by setting up MEND Inc in the USA, which has partnered with AmeriGroup.
- 18. We adapted MEND 2-4 and MEND 7-13 in partnership with Alberta Health Services for families in Canada.
- 19. The MEND Foundation, a US charity, secured partnerships with the Colorado Health Foundation, Blue Cross Blue Shield and Anthem Wellpoint among others.
