Current perspectives in robotic assisted pediatric surgeryApollo Hospitals
Minimal access surgery (MAS) in pediatrics, has exponentially expanded in India in the last 2 decades. Since its introduction in the beginning of this century, Robotic assisted surgery (RAS) is being performed for various indications in pediatric population. We reviewed various articles to seek the evidence regarding the indications, benefits, complications and costs of robotic assisted surgery (RAS) in children. The article also compares RAS with conventional surgical techniques both open and laparoscopy with the aim to provide its current and future implications in developing country like India. The peer-reviewed literature was searched for studies that provided evidence of the feasibility and safety of robotic surgery in children and for studies that compared RAS with laparoscopy or open surgery. Review of various studies showed that RAS in children was safe and feasible as compared with traditional laparoscopic and open surgery. However, the advantages of the robotic system were best seen in complex urological procedures and in surgeries involving more of suturing especially in areas which are deep to access. Cost considerations and lack of training centers in India is a major drawback in the current scenario.
Arthritis affects one out of every five Americans – an estimated 52.5 million people – and is the leading cause of disability in the nation. The condition itself can make it difficult and painful to conduct even the most mundane daily tasks such as walking, climbing stairs, using a computer, or brushing your teeth. The term “arthritis” encompasses over 100 rheumatic conditions that impact the body’s joints, causing pain and inflammation to those who suffer with it.
This document provides an overview of osteoarthritis. It begins by defining osteoarthritis as a type of arthritis caused by the breakdown of cartilage between bones in a joint. It then discusses the common joints affected, occurrences based on age and population, and primary versus secondary causes related to aging, injury, genetics, and other conditions. The document outlines symptoms, the etiopathogenesis of cartilage breakdown, diagnosis methods, and treatment options including non-pharmacological therapies, topical analgesics, oral medications, injections, and surgery.
Osteoarthritis is a progressive disorder caused by loss of cartilage in joints. It has two classifications: primary which occurs with aging or for unknown reasons, and secondary which results from other diseases or injuries. Risk factors include age, sex, obesity, repetitive joint stress, and other medical conditions. Symptoms include pain that worsens with use and improves with rest, stiffness, swelling, and loss of function. Diagnosis is based on history, examination findings like limited range of motion, and x-rays showing bone spurs and joint space narrowing. Treatment focuses on exercise, weight loss, medications like acetaminophen, NSAIDs, and corticosteroid injections, and possibly surgery like joint replacement for severe cases.
Osteoarthritis is a common form of arthritis where cartilage in joints breaks down over time. It causes pain, stiffness, and loss of mobility in joints like the hands, hips, and knees. While there is no cure, treatments can help manage symptoms and maintain activity. For mild cases, initial treatments include exercise, weight loss, heat/cold therapy, and over-the-counter pain relievers. For moderate to severe cases, stronger medications, cortisone shots, and surgery may be needed to relieve pain and disability. Joint replacement surgery can help severely affected patients resume an active lifestyle.

Osteoarthritis is a degenerative joint disease characterized by the breakdown of articular cartilage. This leads to pain and stiffness in the joints. As the disease progresses, cartilage continues to deteriorate and bone may start rubbing against bone, causing more damage. While aging is a risk factor, genetics and mechanical stresses on the joints also contribute to osteoarthritis. For many patients, joint replacement surgery like knee or hip arthroplasty may become the only viable treatment option once bone-on-bone contact occurs to relieve pain and improve mobility.
This document provides a comprehensive overview of osteoarthritis, including its definition, causes, risk factors, signs and symptoms, diagnosis, treatment options, self-care strategies, and the role of yoga and complementary therapies. It describes how osteoarthritis results from the breakdown of cartilage in joints, most often affecting the hands, spine, knees and hips. Risk factors include age, genetics, injury and obesity. Treatments include medications, exercise, weight control, and in some cases surgery.
Osteoarthritis (OA) is the most common form of arthritis, typically affecting older adults over age 45. It occurs when the cartilage between bones breaks down, causing pain, stiffness, and reduced mobility. Risk factors include age, female sex, joint injuries, obesity, genetics, and overuse. Symptoms include joint pain, stiffness, swelling, and crepitus. Diagnosis is made through physical exam, x-rays showing joint space narrowing and bone spurs, and ruling out other causes. Treatment focuses on reducing symptoms through medications, exercises, weight loss, bracing, and joint replacements for severe cases.
The document discusses the key aspects of effective detailing in the pharmaceutical industry. It defines detailing as advertising a product to targeted customers to induce them to buy or prescribe it. The summary is:
Detailing is an important pharmaceutical marketing tool used to promote products to doctors. It involves thoroughly rehearsed personal selling to create interest in a product and influence prescribing habits. Effective detailing requires perfect communication through text, voice, visual aids, eye contact, and other factors to produce an audio-visual impact within the doctor's limited time.
Osteoarthritis is a degenerative joint disease characterized by breakdown of cartilage and bone changes. It most commonly affects weight-bearing joints like the hips and knees. Risk factors include obesity, joint injury, genetics, and age. Symptoms include joint pain, stiffness, swelling, and decreased range of motion. Diagnosis is made based on clinical features and confirmed with x-rays showing cartilage loss, bone spurs, and bone changes. Treatment focuses on education, exercises, braces, medications, and sometimes surgery to relieve symptoms and improve function.
Homeopathic medicines are very highly effective in management of piles and the symptoms associated with it such as pain, bleeding, itching, etc. The point to be emphasized is that the relief of symptoms is obtained with absolute gentleness and without invasion or surgery of any kind. Besides, the condition has high backslide rate taking after surgical treatment, since surgery does not focus on the main drivers such as hereditary inclinations, periodic clogging, and so forth. Homeopathic drugs work at the root level and can change these hereditary inclinations accordingly decreasing odds of backslide and repeat of the condition essentially. Homeopathy treatment reduces the intra abdominal pressure. There are good medicines that relieve constipation which plays major role in developing piles.

This chapter discusses the history of ultrasonic-assisted liposuction (UAL) and its evolution over time. Early UAL devices from the 1990s were inefficient at fragmenting and aspirating fat and often resulted in complications from excessive ultrasound energy application. Later generation UAL devices introduced in the early 2000s addressed these issues through more efficient solid titanium probes and separating fat fragmentation from aspiration, allowing for safer tissue temperatures. The VASER system in particular was shown to fragment fat using minimal ultrasound energy and avoid complications seen with prior UAL devices. Overall, the technology of UAL has improved but relies on surgeons having proper training and a defined process to achieve quality outcomes.

CLASSIFICATION OF H1 ANTIHISTAMINICS-
FIRST GENERATION ANTIHISTAMINICS-
1)HIGHLY SEDATIVE-DIPHENHYDRAMINE,DIMENHYDRINATE,PROMETHAZINE,HYDROXYZINE 2)MODERATELY SEDATIVE- PHENARIMINE,CYPROHEPTADINE, MECLIZINE,CINNARIZINE
3)MILD SEDATIVE-CHLORPHENIRAMINE,DEXCHLORPHENIRAMINE
TRIPROLIDINE,CLEMASTINE
SECOND GENERATION ANTIHISTAMINICS-FEXOFENADINE,
LORATADINE,DESLORATADINE,CETIRIZINE,LEVOCETIRIZINE,
AZELASTINE,MIZOLASTINE,EBASTINE,RUPATADINE. Mechanism of action of 2nd generation antihistaminics-
These drugs competitively antagonize actions of
histamine at the H1 receptors.
Pharmacological actions-
Antagonism of histamine-The H1 antagonists effectively block histamine induced bronchoconstriction, contraction of intestinal and other smooth muscle and triple response especially wheal, flare and itch. Constriction of larger blood vessel by histamine is also antagonized.
2) Antiallergic actions-Many manifestations of immediate hypersensitivity (type I reactions)are suppressed. Urticaria, itching and angioedema are well controlled.3) CNS action-The older antihistamines produce variable degree of CNS depression. But in case of 2nd gen antihistaminics there is less CNS depressant property as these cross BBB to significantly lesser extent.
4) Anticholinergic action- many H1 blockers
in addition antagonize muscarinic actions of ACh. BUT IN 2ND gen histaminics there is Higher H1 selectivitiy : no anticholinergic side effects

GASTROINTESTINAL INFECTIONS AND GASTRITIS
Osvaldo Bernardo Muchanga
Gastrointestinal Infections
GASTROINTESTINAL INFECTIONS result from the ingestion of pathogens that cause infections at the level of this tract, generally being transmitted by food, water and hands contaminated by microorganisms such as E. coli, Salmonella, Shigella, Vibrio cholerae, Campylobacter, Staphylococcus, Rotavirus among others that are generally contained in feces, thus configuring a FECAL-ORAL type of transmission.
Among the factors that lead to the occurrence of gastrointestinal infections are the hygienic and sanitary deficiencies that characterize our markets and other places where raw or cooked food is sold, poor environmental sanitation in communities, deficiencies in water treatment (or in the process of its plumbing), risky hygienic-sanitary habits (not washing hands after major and/or minor needs), among others.
These are generally consequences (signs and symptoms) resulting from gastrointestinal infections: diarrhea, vomiting, fever and malaise, among others.
The treatment consists of replacing lost liquids and electrolytes (drinking drinking water and other recommended liquids, including consumption of juicy fruits such as papayas, apples, pears, among others that contain water in their composition).
To prevent this, it is necessary to promote health education, improve the hygienic-sanitary conditions of markets and communities in general as a way of promoting, preserving and prolonging PUBLIC HEALTH.
Gastritis and Gastric Health
Gastric Health is one of the most relevant concerns in human health, with gastrointestinal infections being among the main illnesses that affect humans.
Among gastric problems, we have GASTRITIS AND GASTRIC ULCERS as the main public health problems. Gastritis and gastric ulcers normally result from inflammation and corrosion of the walls of the stomach (gastric mucosa) and are generally associated (caused) by the bacterium Helicobacter pylor, which, according to the literature, this bacterium settles on these walls (of the stomach) and starts to release urease that ends up altering the normal pH of the stomach (acid), which leads to inflammation and corrosion of the mucous membranes and consequent gastritis or ulcers, respectively.
In addition to bacterial infections, gastritis and gastric ulcers are associated with several factors, with emphasis on prolonged fasting, chemical substances including drugs, alcohol, foods with strong seasonings including chilli, which ends up causing inflammation of the stomach walls and/or corrosion. of the same, resulting in the appearance of wounds and consequent gastritis or ulcers, respectively.
Among patients with gastritis and/or ulcers, one of the dilemmas is associated with the foods to consume in order to minimize the sensation of pain and discomfort.

Receptor Discordance in Breast Carcinoma During the Course of Life
Definition:
Receptor discordance refers to changes in the status of hormone receptors (estrogen receptor ERα, progesterone receptor PgR, and HER2) in breast cancer tumors over time or between primary and metastatic sites.
Causes:
Tumor Evolution:
Genetic and epigenetic changes during tumor progression can lead to alterations in receptor status.
Treatment Effects:
Therapies, especially endocrine and targeted therapies, can selectively pressure tumor cells, causing shifts in receptor expression.
Heterogeneity:
Inherent heterogeneity within the tumor can result in subpopulations of cells with different receptor statuses.
Impact on Treatment:
Therapeutic Resistance:
Loss of ERα or PgR can lead to resistance to endocrine therapies.
HER2 discordance affects the efficacy of HER2-targeted treatments.
Treatment Adjustment:
Regular reassessment of receptor status may be necessary to adjust treatment strategies appropriately.
Clinical Implications:
Prognosis:
Receptor discordance is often associated with a poorer prognosis.
Biopsies:
Obtaining biopsies from metastatic sites is crucial for accurate receptor status assessment and effective treatment planning.
Monitoring:
Continuous monitoring of receptor status throughout the disease course can guide personalized therapy adjustments.
Understanding and managing receptor discordance is essential for optimizing treatment outcomes and improving the prognosis for breast cancer patients.
