DCIS is a pre-invasive breast cancer that is often asymptomatic and non-palpable. While most cases of DCIS are unicentric, studies have shown it can also be multifocal with gaps of uninvolved tissue between foci. The rate of recurrence for DCIS ranges from 4% for low-grade to 15% for high-grade disease. Positive margins significantly increase the risk of local recurrence compared to negative margins. Large clinical trials found that clear margins and radiation therapy significantly decrease DCIS recurrence rates. However, the optimal excision width remains unclear. Current consensus guidelines recommend margins of at least 2mm for DCIS treated with radiation based on evidence from over 7,800 patients, though smaller margins may be acceptable
