Our study included 1635 records from 159 hospitals in 57 countries, collected between Nov 1, 2018, and Jan 31, 2020. 328 (20%) records were from countries in the very high HDI tier, 539 (33%) from countries in the high HDI tier, 614 (38%) from countries in the medium HDI tier, and 154 (9%) from countries in the low HDI tier. The median age was 35 years (IQR 24–51), with the oldest patients in the very high HDI tier (median 54 years, IQR 34–69) and the youngest in the low HDI tier (median 28 years, IQR 20–38). The most common procedures were elevation of a depressed skull fracture in the low HDI tier (69 [45%]), evacuation of a supratentorial extradural haematoma in the medium HDI tier (189 [31%]) and high HDI tier (173 [32%]), and evacuation of a supratentorial acute subdural haematoma in the very high HDI tier (155 [47%]). Median time from injury to surgery was 13 h (IQR 6–32). Overall mortality was 18% (299 of 1635). After adjustment for casemix, the odds of mortality were greater in the medium HDI tier (odds ratio [OR] 2·84, 95% CI 1·55–5·2) and high HDI tier (2·26, 1·23–4·15), but not the low HDI tier (1·66, 0·61–4·46), relative to the very high HDI tier. There was significant between-hospital variation in mortality (median OR 2·04, 95% CI 1·17–2·49).

Conference agenda for the 1999 STAMP Conference on CNS Shunts and anti siphon devices in Bethesda, MD, brought about by patient advocate Stephen Dolle and his efforts with a 1996 petition to FDA on anti siphon shunts. Dolle oddly was not invited to speak or be a panelist, or to have his new mHealth DiaCeph Test included in the conference. In the years since, CNS shunts have suffered significant medical device and design failures, most of which have not been reported to FDA, which Dolle attributes to the cover ups dating back to this conference. Medical Students in Global Neurosurgery: Rationale and RoleAhmad Ozair
Approximately 5 million essential neurosurgical cases are unmet each year, all in low- and middle-income countries (1). After the Lancet Commission on Global Surgery described the absence of global surgery from global health discourse in January 2014 (2), the field of neurosurgery quickly recognized the importance of increasing equity in care globally (3-5). Although existing initiatives in global neurosurgery have focused on neurosurgeons and trainees, medical students represent a promising group for sustainable long-term engagement. We characterize why medical students are fundamental to success, outline the importance of incorporating medical students, and delineate how to increase medical student interest and participation in global neurosurgery.
Background: Cancer is a disease caused when cells divide uncontrollably and spread into the surrounding tissue. Changes to DNA cause cancer. It is one of the most common and largest killer diseases in the world. It usually affects physically, and the disease can alter one’s perspective on life and personality. Many treatment options are there to treat cancer. Among them, chemotherapy treatment may have more side effects like lethargy, esophagitis, nausea, vomiting, Fatigue, and insomnia, the most common problems among chemotherapy patients in India. Methods: A quasi-experimental study with a sample size of 60, out of which 30 subjects were in the experimental group and 30 were in the control group. A convenient sampling method was used to select the subjects. A structured questionnaire tool was used to collect the data. Result: The result of the study showed that, during pre-test in the study group, among 30 subjects 3(10%) had moderate Fatigue, 22(73.33%) had severe Fatigue, 5(16.67%) had worst Fatigue and 12(40%) had moderate insomnia, 18(60%) had severe insomnia and in control group among 30 subjects, 6(20%) had moderate Fatigue, 13(43.33%) had severe Fatigue, 11(36.67%) had a worst fatigue, and 14(46.67%) had moderate insomnia, 16(53.33%) had severe insomnia. With post-test, in experimental group, 14(46.67%) had no fatigue, 16(53.33%) had mild fatigue, 14(46.67%) had no insomnia, 16(53.33%) had mild insomnia, and in control group, 6(20%) had moderate fatigue, 13(43.33%) had extreme fatigue, 11(36.67%) had worst fatigue, and 14(46.67%) had moderate insomnia, 16(53.33%) had severe insomnia. Conclusion: The study concluded that clients who were receiving chemotherapy had fatigue and insomnia problems. The Warm water foot bath therapy is very effective in clients undergoing chemotherapy in reducing Fatigue and insomnia. A positive correlation between pre-test and post-test was found by using the Mann-Whitney test.

Diagnostic Efficacy of Ultra-High-Field MRS in Glioma PatientsUzay Emir
Synopsis: We have recently initiated a 7T MRS glioma consortium, intending to bring together experts in the field to discuss pitfalls, promises, and potential research avenues of MRS in gliomas. During the "GlioMaRS-NET Workshop" in 2020, we tested the efficacy of UHF MRS for predicting the molecular characteristics by visual inspection by conducting a survey with a set of previously acquired UHF spectra from glioma patients. In the post-genomics era, the World Health Organization (WHO) classification of gliomas has become even more tightly integrated with molecular parameters in addition to histology. Integrated diagnoses offer prospects for precision medicine strategies tailoring therapies for each individual. The International Society of Neuropathology-Haarlem Consensus has proposed the personalized layered diagnosis for gliomas that merge all of the distinct but related information. For the final diagnosis, the concept of integrated diagnosis (Layer 1) relies on all available data, which include the histological classification (Layer 2, e.g., astrocytoma vs. oligodendroglioma), the grading (Layer 3, e.g., WHO grade II vs. III), and molecular characteristics (Layer 4, e.g., isocitrate dehydrogenase (IDH)-mutant, 1p/19q-codeletion) (1). Thus, the personalized layered diagnosis approach has emphasized to tailor the diagnostic imaging to contribute to the integrated diagnosis by discovering reliable imaging biomarkers and tools capable of differential diagnosis according to genetic subtypes. It has recently been shown that definitional features of glioma, such as IDH mutation and 1p/19q codeletion (Layer 4), can be identified with non-invasive MRS at 3T, opening up exciting new opportunities for diagnostics, clinical trials, and assessment of treatments (2,3). Ultra-high-field (UHF, >=7T) MRI scanners offer enhanced detection relative to routine 3T MRI of 2-HG peaks in the MRS spectra of IDH mutated patients (4,5). This is due to increased SNR at the higher field strength and increased spectral dispersion, which can improve the delineation of 2HG from neighboring metabolites such as glutamate and glutamine. Furthermore, at 7T, the 2-HG peak is not only to contribute to layer 4 of the integrated diagnosis by identifying molecular features but also to detect subtle changes due to the reprogramming of cellular metabolism during the disease progression or treatment. With the recent successes of 2-HG and increased use of UHF MRI in clinical settings as a basis, we have recently initiated a 7T MRS glioma consortium, intending to bring together experts in the field to discuss pitfalls, promises, and potential research avenues of MRS in gliomas.
