Dermatomyositis and Undifferentiated Nasopharyngeal Carcinoma. A Rare Present...asclepiuspdfs
The incidence of nasopharyngeal carcinoma (NPC) is 0.5–25 cases over 100.000 of all malignancies. Undifferentiated NPC type is the most frequent of all in endemic areas. Dermatomyositis (DM) is a relatively rare disorder, associated with malignancy in 10–50% of cases. Few cases have been reported by the literature referring to DM and undifferentiated NPC in Caucasian population.[1] We present a 65-year-old man with a history of relapsing bilateral serous mucoid otitis, who developed skin lesions and muscle fatigability, compatible with DM as a paraneoplastic syndrome (PNS). He received standard therapy for NPC and 4 months after the end of treatment; we observed a complete remission of both the tumor and PNS. The existence of DM must aware physicians of underlying cancer, due to the fact that it is frequently associated with a large number of tumors. However, PNS and NPC are an unusual presentation.

Angiosarcomas are rare and represent less than 2 percent of all sarcomas. The most frequently involved primary sites are heart, liver and breast with over 100 cases of each reported. Angiosarcomas of the lungs are usually metastasis. Differential diagnosis of vascular neoplasm metastatic to the lungs includes Kaposi sarcoma, epithelioid hemangioendothelioma, and angiosarcoma.
Primary angiosarcoma of the lung is extremely rare and is usually diagnosed quite late due to low index of suspicion.
Reported chest radiograph findings have ranged from normal to multiple nodular densities with or without pleural effusions to diffuse alveolar infiltrates compatible with pulmonary haemorrhage. Metastatic angiosarcoma lung lesions are commonly multiple. There have been reported as solid nodular or thin-walled cysts, with haemorrhagic change.
The HPE was consistent with an epithelioid angiosarcoma and this was also supported by positive reaction for factor VIII-related antigen, CD34 and CD31, which are specific markers for tumours derived from the endothelium.
Predisposing factors for angiosarcomas include polyvinyl chloride and thorium dioxide exposure, postmastectomy and postirradiation states (cutaneous angiosarcoma) and chronic empyema for pleural space angiosarcomas.

Malignant melanoma is one of the most aggressive neoplasms of the skin. It originates from the melanocytes, which are cells derived embryologically from the neural crest and migrate to the epidermal basal layer. It is characterized by producing pigmentation as well as being susceptible to metastasis. We report the case of a 36-year-old female patient with advanced clinical stage and distant commitment. The biopsy confirmed the presence of Grade III invasive nodular cutaneous melanoma in the left subscapular region with lymph node metastasis with reactive hyperplasia. An exploratory research is carried out with the bibliographic review in scientific journals with evidence level II–IV. In portals PubMed, Redalyc, BVS, and UpToDate. 81241 met criteria 2248 of which 629 were chosen for having access to the full text and of these 496 are more current (as of 2008), and in the end, 27 articles were selected that met all the inclusion criteria to this article. Due to the increase in the incidence of this disease in recent years and its poor prognosis in short to medium term, it is important to know and follow-up on patients with known risk factors for this disease such as the presence of previous nevi, with emphasis on measures of prevention.
Primary malignant schwannoma is a rare neoplasm of nerve sheath origin. It is a cancer of the connective tissue surrounding nerves. Given its origin and behavior it is classifi ed as sarcoma. The estimated incidence of Malignant Peripheral Nerve Sheath Tumor (MPNST) in general population is 0.001% and in patients with Neurofi bromatosis 1 (NF-1) is 2-5%. It is an uncommon spindle cell sarcoma accounting for approximately 5% of all soft-tissue sarcoma. They are highly aggressive and occur in the second or third decade. This neoplasm usually affects the extremities. There is strong association between MPNSTs and neurofi bromatosis (NF-1) and previous irradiation. We present the case of a 61-year-old woman manifesting with recurrent sciatalgy near for the fourth and fi fth lumbar vertebral bodies. She underwent resection of a mass at the L4-5 level that was subsequently recognized as a malignant peripheral nerve sheath tumor.

Our case describes an unusual presentation of dermatomyositis
in a patient with ovarian carcinoma. Th e eruption appeared as a venous stasis like dermatitis. Th e temporal sequence of onset after chemotherapy administration suggested a possible drug-induced process. However, in the context of underlying ovarian carcinoma, a paraneoplastic process offered an alternative explanation for
dermatomyositis.

Natural killer (NK)/T-cell lymphoma is a rare malignancy accounting for less than 1 percent of all lymphomas in North America and Europe. Ocular manifestations frequently presented as orbital cellulites that does not improve with adequate antibiotics. We report a case of NK/T cell lymphoma with rare ocular manifestation that was initially confused with orbital cellulites and chronic sinusitis due to multiple negative biopsies. 92-year-old female presented with right eye vision loss, and ipsi lateral orbital swelling for four weeks. MRI demonstrated right intra-orbital extension to the orbital apex. Workups for infection and vasculitis were negative. Multiple biopsies with histopathology and flow cytology were unrevealing for malignancy. Patient failed to improve on antibiotics and steroids. Five months after the initial presentation, patient presented with the same complaint; however, the orbital mass had grown in size and involved the maxillary sinus and contra lateral side. Re-biopsy revealed positive CD 56 for NK lymphoma.

Background: Perforated tympanic membrane and middle ear infection are among common complications treated by tympanoplasty. Objectives: To determine the potential of“Black Bone” MRI as an alternative to CT in the maturation parameters of the palatine suture and identify the optimal time to perform orthodontic palatal expansion. Methods: two patients from 10.3 and 25.9 years underwent “Black Bone” MRI “, T1 and T2 weighted spin echo imaging were obtained in the axial plane in addition to routine cranial RMI. “Black Bone” datasets weresubsequently compared to T2 wheighted RMI for the visualisation medial palatine sutures by using a similarbased method to evaluate suture fusion stages. Results: Patient palatine sutures were consistently identified on “Black Bone” MRI as áreas with different signal intensity according to the maturation stage, from complete separation of the halves to total fusion, as also found in the T2 weighted protocol.“Black Bone” MRI can be considered as an alternative o CTBC to assess the maturation of medial palatine suture for orthodontic purposes. Conclusions “Black Bone” MRI has considerable clinical potential as a nonionising alternative to CT in the assessement of the medial palatine suture.

Premalignant Lesions of Oral Cavity - A Clinicopathological Study by Mohit Srivastava in Experiments in Rhinology & Otolaryngology
We conducted a study on a total of 360 patients to find out the incidence of premalignant disorders in the oral cavity who attended the ENT opd of Saraswathi Institute of Medical Sciences, Hapur. The study design was cross sectional in nature. Out of all the 360 patients who were suspected to have premalignant disorders of oral cavity were subjected to biopsy and 87 of them were found to be premalignant. Incidence was found to be 24.53% in males and 23.6% in females. Buccal mucosa was most common site of involvement.

A Rare Case of Nasal Inferior Meatus Polyps by George MV in Experiments in Rhinology & Otolaryngology
This is a case report of a patient with a sided nasal inferior meatus polyposis. A 34-year-old female patient rested with history of discharge coming from right side of nose of 6 months duration. On examination, there was minimal discharge in the floor of the nose. On endoscopy, there was a swelling under the inferior turbinate. CT PNS showed an enlarged inferior turbinate of the same side and bilateral concha bullosa, and mild maxillary sinusitis. The patient had undergone an endoscopic excision biopsy of the swelling and the histopathological report was Inflammatory Nasal polyps.

Management Outcomes of Post-Thyroidectomy Bilateral Recurrent Laryngeal Nerve Paralysis at National Hospital Abuja by Olusesi Abiodun Daud in Experiments in Rhinology & Otolaryngology
Bilateral recurrent laryngeal paralysis is an uncommon complication of total or subtotal thyroidectomy, observed in approximately 0.4 per cent of cases. This paralysis could be temporary or permanent. An audit of 5 cases referred to the ENT Department of National Hospital Abuja, between January 2010 and July 2017 is presented. All cases were referred already on tracheostomy tubes and were females aged 11 to 59 years. 4 of the cases had external arytenoidectomy, bilateral in 2 cases, and unilateral in 2 cases. 4 out of the 5 cases were successfully decannulated. The preferred approach to cases of post-thyroidectomy bilateral recurrent laryngeal nerve paralysis referred to ENT Specialists in resource-poor economy like ours is not very clear from existing literature and we discuss our adopted protocol for management of such cases in this case series.

Post-thyroidectomy laryngeal diplegia is the most common and most feared complication [1]. It occurs following a recurrent nerve lesion in 26 to 59% of cases [1,2]. Tracheotomy was considered until 1922 as the only reference treatment [3,4]. Therapeutic approaches have evolved over time, ranging from convention altranslaryngeal or extralaryngeal therapy to endoscopic laser approaches [5]. These endoscopic methods emphasized endoscopicary tenoidectomy and posterior transverse cordotomy [4,6]. Metastatic Atypical Fibroxanthoma: Case Report
of an Uncommon Pathology in the Head and Neck
Introduction
Atypical fibroxanthoma (AFX) is a rare skin malignancy
generally developed in elderly male patients with actinic damage.
Due to its low rate of metastasis, it is considered a benign neoplasm
[1,2]. Metastatic AFXs occur in fewer than 6% of patients. Although
many risks factors have been described, older age and deep tissue
invasion are the most important. We present a case of an elderly
patient with metastatic disease; due to its uncommon presentation
we made a small review of the most outstanding features of this
challenging pathology.

Abstract
Atypical fibroxanthoma (AFX) is an uncommon skin neoplasm developed mainly in the head and neck region in older senior patients. Prolonged
sun exposure, actinic damaged, history of previous neoplasms and immunosuppressant are considered the most important risks factors. Subcutaneous
extension of these tumors is related to a more aggressive biology. Metastatic extension is extremely rare but might lead to a fatal outcome. Differential
diagnosis might be a challenge for pathologist due to its uncharacteristic features. We report the case of an elder patient who presented a recurrent AFX
and who developed lung and lymph node metastasis. Even though treatment with chemotherapy and radiation was proposed, the patient final passed
away due to a worsening of his condition.

We present the case of a 77-year-old male patient who was
referred to the head and neck surgery service for a skin lesion
on the scalp. He complained of a painless, ulcerated, elevated
nodule, with 1-year progression. A full thickness resection with
a 1cm margin was performed, and AFX was diagnosed after
final immunohistochemistry (IHC) analysis. The AFX recurred
after 2years, with an ulcerated, firm, erythematous scalp nodule
associated with an enlarged neck and supraclavicular lymph
nodes. A CT scan showed bone and lung secundarism as well as
cervical, supraclavicular, and mediastinal adenopathies Figure 1.
A fine-needle aspiration (FNA) biopsy confirmed both primary
and secondary lesions Figure 2. The oncological board committee
recommended surgical and adjuvant treatment; however, the
patient’s condition worsened, and the family and attending team
ultimately decided to submit to palliative care.

Discussion
AFX is an uncommon mesenchymal tumor that represents 0.2%
of all skin tumors. Regarded as a benign skin tumor, is usually seen
in sun-damaged skin, mainly in senior patients, with an average
age of 71 years, with a male-to-female ratio of 4:1 [1-5]. It most
often arises in sun-exposed areas, predominantly in the head and
neck region. Less frequently appears in trunk and extremities,
usually in young people. Other risk factors described are traumas,
ionizing radiation, and history of previous skin neoplasms.
Immunosuppressant may also be considered a relevant factor [1,3].
AFX usually presents as an elevated, reddish, painless solid
nodule limited to the dermis; it rarely exceeds 2cm in diameter, and
the center might be ulcerated. Its histological description includes
atypical spindled and pleomorphic cells with numerous bizarre
mitotic figures and hyper chromatic multilobulated nuclei; no
established pattern of distribution is recognized. They are generally
restricted to the reticular dermis, but some lesions may extend into
the upper one-third of the sub cutis [1,2,6,7].
Some researchers consider AFX a superficial variant of
malignant fibrous histiocytoma (MFH) or undifferentiated
pleomorphic sarcoma (UPS), which is both histological and
immunohistochemically indistinguishable from AFX [3,6] However,
AFX is generally limited to the dermis, with minimal subcutaneous
tissue involvement .
According to the latest classification which replaces MFH and
UPS [8,9], if the tumor invades the subcutaneous tissue should
be reclassified as a pleomorphic dermal sarcoma (PDS). Other
differential diagnoses include melanoma, derma to fibroses coma,
and sarcomas. The IHC analysis might determine the final diagnosis;
it is usually negative for S100, CD23, epithelial membrane antigen
(EMA), MELAN A, cytokeratin, and desmin. However, there are
no specific stains to identify AFX, which generally return positive
results for vimentin, CD 10, CD 99, and CD 68. Surgery has been
described as the therapy of choice [1, 3-8]. A 1 cm has been the
averagemarginused,butsomeauthorshaveproposeda2cmmargin
[1,4,7]. Both wide local excision (WLE) and Mohs micrographic
surgery (MMS) have shown excellent results, although there many
reports with some contradicting results in terms of recurrence with
MMS vs. WLE. However, in a recent review, MMS has proved to have
a lower rate of recurrence [9]. The rate of recurrence for WLE and
MMS is 8 and 0-4,6% respectively. However, surgery (in general)
with oncological safety margins remains to be the best option
as there is still no clear evidence of the best option of surgical
treatment. It is important to mention that the whole management
should be directed towards the prevention of local recurrences.
AFX has poor metastatic potential (1%) 7. However, even cases
with minimal subcutaneous involvement can lead to death. The
reported rate of recurrence and metastasis is around 2-20% and
