A parent is providing consent to allow their child to transfer classrooms during a literature class taught by Mrs. ADONAI SHAMMAH M. LUMBA. The parent understands that Mrs. Lumba is a cancer survivor and agrees to the transfer for humanitarian reasons given her health condition. The parent also acknowledges being aware of the COVID-19 pandemic and the precautions needed for their child to safely transfer classrooms.

of Mrs. Lumba who is a cancer survivor and due to humanitarian reason
we definitely agree to the idea.
My child is in good health. I am fully aware of Covid-19 Pandemic and also
fully aware of the precautionary measures to be taken while my child is
transferring from classroom to classroom. I will make sure that I will
always be on guard in reminding my child to follow all the instructions
given for Covid-19 Pandemic.
