Imagine an event of catastrophic proportion involving mass casualties, disrupted or non-existent services (power, transportation, and communications), scarce food and water, limited emergency personnel and medical supplies, overwhelmed hospitals, perhaps contamination from biohazard materials or nuclear fallout, etc.
Now imagine that a new set of rules has been established to guide first responders in the field whenever a “catastrophe” occurs. A system of “response triage” is required, whereby precious and limited resources will be directed to those who could most probably contribute to continued survival and eventual recovery of the community. Those who would require a disproportionate share of resources to live, and those who will most likely not survive the event, are given lower priority for distribution of assistance, including food supplies and medical treatment.
Without any formal discussion of what ethics are and how ethical decisions might be made in the field, we can see that the ethical problems are endless, but are basically summed up by asking:

Technological advances in medicine and preventative care means that Americans are living longer lives than ever before. Hayutin, Deitz, and Mitchell (2010) assert that by the year 2030 Americans over the age of 65 will account for 20% of the population. There will soon be more elderly Americans than children, and the number of working adults is expected to decrease concurrently. This shift in the population will yield significant economic, political and social challenges. Healthcare needs are also changing. Death and disability rates are declining, yet the incidence of chronic illness within the elderly population continues to rise (Hayutin, Deitz, & Mitchell, 2010). Crippen and Barnato (2011) contend that 20% of the population assume 80% of all healthcare-related costs. As much as 75% of these costs are attributable to chronic diseases (Crippen & Barnato, 2011). Revenues for healthcare are projected to decrease while expenditures are expected to increase. Healthcare providers, policymakers, and industry experts need to work towards solutions that will optimize healthcare dollars and create sustainability for future generations.
Ethical Considerations
The dynamics of healthcare are complicated; financial resources seem insignificant when making life and death decisions. Nonetheless, resources are finite, and therefore, distribution and allocation of funds must be ethical. According to Craig (2010), the theory of distributive justice requires that people with the same health needs have equitable access to all available resources. However, distributive justice also requires that the associated costs also be shared equitably. Fairness is another ethical principle that should be applied in the allocation of healthcare resources. Policies that are fair must be transparent, understandable, and there must be regulatory process to address complaints and resolve conflicts. The idea that healthcare is a human right is outlined in the declaration of independence which guarantees citizens the right to life, liberty, and the pursuit of happiness. The need of the patient should also be considered. A burn patient needs plastic surgery more than a patient that wants rhinoplasty (Craig, 2010).
Nurses provide the best possible care to every single patient regardless of gender, ethnicity, sexual orientation, ability to pay, or age. The American Nurses Association (2012) provides ethical guidelines for nurses to employ in their practice. Provisions one, two, and three promote the principle of beneficence, and the obligation nurses have to advocate for the best interests of their patients. aid in estimating the prevalence, magnitude and frequency of the problem in different regions.
On the other hand, qualitative methods like case studies and observation will help describe the
extent and complexity of the issue. The two approaches need to work in complementation to
obtain a clear understanding of this menace.
Surveys, as a quantitative research method, is one of the most effective in the social
research and present a more viable method of examining the cost of health in the country. They
involve asking of questions in the form of questionnaires and interviews. Questionnaires are
written questions to which the response can be open ended or multiple-choice format. This
would be used to gain information about cost within determinants that are of
disagree/neutral/agree nature. An example is if patients are contented with the cost of services
they get or they deem the cost of cover worthy. Interviews, the researcher discussing issues with
the respondents, are to be used to gain more details on already known aspects of the system. This
may include gathering information to inform policies, administration and use of technology to

On the other hand, qualitative methods like case studies and observation will help describe the
extent and complexity of the issue. The two approaches need to work in complementation to
obtain a clear understanding of this menace.
Surveys, as a quantitative research method, is one of the most effective in the social
research and present a more viable method of examining the cost of health in the country. They
involve asking of questions in the form of questionnaires and interviews. Questionnaires are
written questions to which the response can be open ended or multiple-choice format. This
would be used to gain information about cost within determinants that are of
disagree/neutral/agree nature. An example is if patients are contented with the cost of services
they get or they deem the cost of cover worthy. Interviews, the researcher discussing issues with

Unlike other developed countries the medical care system in the United States of America demands more and more funds while its quality remains the same. 1/3 of the US citizens are still uninsured and there is no future hope for improving the situation. People suffer from rapidly growing prices of medical services and slow growth of salaries inclement. They showed, for example, that groups of strangers could persuade people to believe statements that were obviously false. Psychologists had also found that research participants were often willing to obey authority figures even when doing so violated their personal beliefs. The Yale studies by Stanley Milgram in 1963 demonstrated that a majority of ordinary citizens would continually shock an innocent man, even up to near-lethal levels, if commanded to do so by someone acting as an authority. The "authority" figure in this case was merely a high-school biology teacher who wore a lab coat and acted in an official manner. The majority of people shocked their victims over and over again despite increasingly desperate pleas to stop.
In my own work, I wanted to explore the fictional notion from William Golding's Lord of the Flies about the power of anonymity to unleash violent behavior. In one experiment from 1969, female students who were made to feel anonymous and given permission for aggression became significantly more hostile than students with their identities intact. Those and a host of other social-psychological studies were showing that human nature was more pliable than previously imagined and more responsive to situational pressures than we cared to acknowledge. In sum, these studies challenged the sacrosanct view that inner determinants of behavior--personality traits, morality, and religious upbringing--directed good people down righteous paths.
Missing from the body of social-science research at the time was the direct confrontation of good versus evil, of good people pitted against the forces inherent in bad situations. It was evident from everyday life that smart people made dumb decisions when they were engaged in mindless groupthink, as in the disastrous Bay of Pigs invasion by the smart guys in President John F. Kennedy's cabinet. It was also clear that smart people surrounding President Richard M. Nixon, like Henry A. Kissinger and Robert S. Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. A CASE STUDY: THE LONG TERM CARE OF OLDER
PEOPLE
1. INTRODUCTION
In December 2017 a Royal Commission was appointed by the
present Labor government to exam the options for a supportable
system of financing the long-term care of older people. The
forms of care considered included care in the community, in a
person's own home, and care in residential or nursing homes.
Older people receiving long-term care in these settings may also
require acute medical care so it was relevant to also look briefly
at the general health provision for older people. Since the
creation of the NHS older patients have been seen as a drain on
resources diverting services away from other patient groups
(Wilkinson and Hughes, 2006). Elderly care has always been
one of the Cinderella services within the NHS, receiving less
than adequate resources despite repeated attempts to ensure it
was given priority in resource allocation (Ibid.). Numerous
studies have focused on the social deprivation and abuse
suffered by people in elderly care homes (Barton, 2009; Biggs
et al., 2005; Robb, 2007; Townsend, 2002). The current debates
about health care and older people focus on rationing, and on
standards of care for the old both in hospitals and in residential
and nursing homes. Evidence exists that older people are denied
access to treatments provided for younger patient. (Aaron and
Schwartz, 2004; Age Concern, 2007). The continuing poor
quality of care for older people within the NHS has been the
focus of considerable media attention (Abrams, 2008; Brindle,
2007; Evans 2007). In October 2007 The Observer newspaper
launched a campaign to ensure that older people in hospital are
treated with dignity, following reports of neglect. In the same
month The Sunday Times launched a campaign for better care
for the elderly in long-term care and highlighted abuse and
10. supportive, older people are then removed from sight and from
society. The result is social exclusion of a whole section of
society.
1.2 The Commission's Recommendations
The Commission Report sets out in the first chapter the values
that have guided its deliberations. The recommendations begin
from the position that old age should not be seen as a problem,
but as a time of life with fulfilment of its own. According to the
Commissioners older people do continue to play a positive role
in society by acting as careers, supporting families, by
providing wisdom and advice and by playing an active part in
society. However, these positive images are often over
shadowed by negative ones. Underlying much of the political
language about old age is assumption that old age is a problem.
With the expected rise in the numbers of elderly and the rise in
their proportion of the population this is thought to be a
problem which will become increasingly insoluble in future
years. However, such an approach treats the old as a
homogenous group, which clearly they are not, and fails to
recognize people as individuals with their own needs aspiration
and perceptions. Moreover the Commission found that for the
UK there is no 'demographic timebomb' as far as long-term care
is concerned, and as a result costs of care would be affordable.
Against these negative images, the Commission suggests a new
understanding: older people should not simply be seen as the
passive recipients of welfare, rather society should recognize
the value inherent in older people, and the value in helping
them to continue to realize their potential. The department of
Education and Employment's commitment to lifelong learning
should mean learning for life beyond work and into the
retirement years. Old age should be seen as a special time when
we have the gift of time to develop interests.
