2. OSCE 1- CASE SCENARIO
A 4-year-old boy has chronic nasal congestion,
poor weight gain, and frequent otitis media.
There has been no improvement with oral
antihistamines, topical decongestants, or topical
corticosteroid sprays. Physical examination of
the nares reveals bilateral polyps.

2-Enumerate 5 diseases associated with nasal polyposis?
3-What is your therapy?
3. Areas of focus
Presentation title 3
1- The boy described has nasal polyps associated with poor
weight gain, chronic otitis media, and nasal congestion. Any
child who presents with nasal polyps should be evaluated for
cystic fibrosis (CF) with a sweat chloride test.
2-Allergic fungal sinusitis, allergic rhinitis, aspirin sensitivity
(Samter triad ), chronic sinusitis, Kartagener syndrome, non-
allergic rhinitis, and Churg-Strauss syndrome.
3- Therapy for nasal polyps includes an initial investigation for
the underlying cause. Medical and surgical interventions have
included topical and systemic glucocorticoids, functional
endoscopic sinus surgery, and aspirin desensitization.
4. OSCE 2- COUNSELLING
You are a pediatrician providing genetic counseling to the
parents of a newborn baby who has been diagnosed with
cystic fibrosis through newborn screening. The parents
are understandably anxious and have many questions
about the implications of the diagnosis for their child's
health and their family's future.

5. Presentation title 5
• Introduction
• Begin by explaining what cystic fibrosis is and how it affects the body's systems, particularly the respiratory and
digestive systems.
• Emphasize that cystic fibrosis is a genetic condition caused by mutations in the CFTR gene, resulting in the production
of thick, sticky mucus that can clog the airways and lead to respiratory problems, as well as affect digestion and
nutrient absorption.
• Discuss the autosomal recessive inheritance pattern of cystic fibrosis, where both parents must be carriers of the
defective CFTR gene for the child to inherit the condition.
• Explain that each future pregnancy carries a 25% chance of having a child with cystic fibrosis if both parents are
carriers.
• Offer genetic testing for the parents to determine if they are carriers of the CFTR gene mutation.
• Explain the implications of carrier status for future pregnancies and the possibility of prenatal testing for cystic fibrosis
if desired.
• Outline the comprehensive treatment approach for cystic fibrosis, including medications to improve lung function,
nutritional support, airway clearance techniques, and regular medical monitoring.
• Discuss the importance of early intervention and adherence to treatment regimens to optimize health outcomes and
quality of life for the child.
• Provide information about local cystic fibrosis care centers, support groups, and resources available to help the family
navigate their journey with cystic fibrosis.
• Offer emotional support and reassurance to the parents, acknowledging the challenges they may face and
emphasizing that they are not alone in this journey.
• Schedule regular follow-up appointments to monitor the child's growth, development, and response to treatment.

12. Areas of focus
Presentation title 12
• Introduction
• Explain about the chronic nature of the disease
• Make it clear that its not their fault( as it’s a genetic problem)
• Common signs and Sx ( epistaxis, bleeding from gums, headache ,
vomiting, joint, muscle swelling)
• Offer genetic counselling and prenatal testing for the next pregnancy
• Discuss about factor prophylaxis
• Precautions during vaccination/ injection
• Avoid contact sports
