As you know, I have held several meetings with Middle County Medical PC (MCMPC) over the last few weeks to discuss a possible engagement. On January 20, 2017, we concluded our discussions and Lehman Health Care Consulting is under contract with MCMPC. Our task is to assist them with a significant strategic management challenge. The narrative that follows presents the situation that our client faces. Our assignment appears at the conclusion of the narrative.
Middle County Medical PC
Introduction
MCMPC is facing a unique situation in a market that has evolved significantly. With its location in the northwest suburbs of NYC, it faces challenges. How will it compete effectively in a market where health care is dominated by one enterprise, Innovative Medical Group (IMG)? IMG has broad strategic approaches that give it a significant market share and an alignment with the major county hospital, County Regional Medical Center (CRMC). It launched an Accountable Care Organization, taking advantage of the opportunities created by the Affordable Care Act. Its bargaining power, negotiating leverage and financial strength give it significant competitive advantages.
Middle County Medical PC (MCMPC)
MCMPC is a fifty (50) physician group practice with fifteen offices with 350 employees. They have physicians covering twenty (20) different specialties and subspecialties, but not every specialty/subspecialty. The Middletown location is the largest and busiest for this group. The group was formed in 1984 by Dr. Scott Balko. Dr. Balko is a board certified ER physician who had been Chief of Emergency Medicine at Bolton Hospital (Bolton Hospital has merged with Darien Hill Hospital creating CRMC) for ten years. He decided to leave the hospital and create an Urgent Care Center serving the Middletown community. Over the least thirty years, MCMPC grew, providing urgent, primary and many specialty services. For period 1997 to 2013, its annualized visits grew by eight percent (8%). Revenues reached $22 million in 2014 and the group was consistently profitable. In 2014, annualized visits declined and profits fell. Dr. Balko told us that the practice had an operating loss in 2016. The repeal of the ACA exacerbates this situation. MCMPC implemented the Medical Home and ran it profitably. The elimination fo this program and the lost revenue make fiscal situation even worse. This presentation explores whether there will be enough workers to fill these positions. According to our research, the UK economy could see a shortfall of 2.6 million workers by 2030 - almost twice the workforce of the NHS.

Senior living is a burgeoning industry driven by an exploding senior population and demand from an aging demographic with increasing needs for specialized residential requirements. This demand will drive strong growth in the senior living industry over the next several decades. The baby boomers are the largest generation in U.S. history, constituting a sizable demographic wave. With 82.8 million people born between 1946 and 1964, the baby boomers represent over 30% of the U.S. population. There is an estimate of 75 million baby boomers who are on the verge of retirement. For the next twenty years, an average of 10,000 people each day will reach age 65, which has historically been the retirement phase of life (Pew Research: http://pewrsr.ch/T4o2Hs ).

These shortfalls will affect the whole economy, with manufacturing, retail, construction, transport, health and social care among the sectors projected to be hardest hit.
To plug these gaps, Government must introduce a comprehensive Workforce Strategy looking at:
How to support people to stay in the workforce for longer, e.g. by supporting healthy workplaces, supporting carers and creating flexible conditions that suit people’s needs.
How to ameliorate childcare costs and reintegrate people into the workforce following timeout for caring or a health need
The role of migration and automation in addressing major workforce gaps
Retirement communities
Currently, one of the key challenges on a global scale is the issue of an aging society. The UN predicts that the number of people aged 60 and over will double by 2050 and treble by 2100. In the coming years, the more supply for real estate that addresses the specific needs of older people will increase both qualitatively and quantitatively by the private company. That differs from the past, the main support is from the government and many foundations.

providing a full range of services. The primary service area is a coastal city and three coun-
ties, with a total population greater than 995,000, located in the Sunbelt. This tricounty
area has had one of the fastest population growth rates in the country for the past five
years. According to the local health planning council, the tricounty population is projected
to increase by 15 percent from 2015 to 2020. Appendix A, at the end of this case study,
provides detailed population statistics for the city and tricounty area.
The population growth rate for households (families) has been 1 to 2 percentage
points higher than the overall population growth. The growth rate of the population under
age 44 shows a young and growing community. Per capita (i.e., per person) income in the
tricounty area is high and increasing. As the population of the tricounty area increases, the
need for healthcare services is anticipated to increase. The area’s economy is largely supported
by manufacturing, with service companies and agriculture accounting for another 35 percent.
Unemployment is typically 6 percent. The overall poverty rate is 12.4 percent. A recent study
revealed that 40,000 city residents are below 125 percent of the established federal poverty level.

The Center maintains the Western Blue Chip Economic Forecast and Greater Phoenix Economic Forecast websites. McPheters also oversees the Job Growth USA website that tracks employment for all industries in the U.S. states and metropolitan areas. The website is frequently used by economists, financial analysts, economic development specialists, and, during election season, fact-checking organizations to evaluate claims by candidates regarding job creation statistics and policies. His writings on the Western region have been quoted in the Wall Street Journal, USA Today, The Economist, Business Week, The New York Times, and Newsweek as well as major metropolitan area newspapers throughout the nation. He has appeared nationally on Good Morning America, Fox News, Marketplace on NPR, and CNN commenting on the economic outlook. As director of the Economic Outlook Center, since 1987 McPheters has delivered a cumulative total of more than 500 speeches and presentations to various public and private audiences at business and academic conferences in Arizona and across the nation.
McPheters has published numerous articles in books and professional journals on various topics, including immigration, executive compensation, monetary policy, international business cycles, and issues in law and economics. At the federal level, his work has been supported by the United States departments of Justice, Transportation, Agriculture, and the Treasury. In Arizona, he has completed research projects for the Arizona Department of Transportation, Sky Harbor International Airport, Boeing, and many other public and corporate sponsors.
McPheters completed his undergraduate studies at San Francisco State University and received his Ph.D. in economics from Virginia Polytechnic Institute. He has been at ASU since 1976, teaching courses at the undergraduate and graduate level in monetary and regional economics. In addition, has held various administrative positions at ASU including Senior Associate Dean for Graduate Programs in the W. P. Carey School of Business from 1991 – 2008.

This comprehensive case study serves as a basis for the exercises included throughout the
book.
Coastal Medical Center (CMC) is a licensed, 450-bed regional referral hospital
providing a full range of services. The primary service area is a coastal city and three coun-
ties, with a total population greater than 995,000, located in the Sunbelt. This tricounty
area has had one of the fastest population growth rates in the country for the past five
years. According to the local health planning council, the tricounty population is projected
to increase by 15 percent from 2015 to 2020. Appendix A, at the end of this case study,
provides detailed population statistics for the city and tricounty area.
The population growth rate for households (families) has been 1 to 2 percentage
points higher than the overall population growth. The growth rate of the population under
age 44 shows a young and growing community. Per capita (i.e., per person) income in the
tricounty area is high and increasing. As the population of the tricounty area increases, the
need for healthcare services is anticipated to increase. The area’s economy is largely supported
by manufacturing, with service companies and agriculture accounting for another 35 percent.
Unemployment is typically 6 percent. The overall poverty rate is 12.4 percent. A recent study
revealed that 40,000 city residents are below 125 percent of the established federal poverty level.

Lee McPheters, director of the JPMorgan Chase Economic Outlook Center, examined the local and regional economies for 2020.
He's a research Professor of Economics in the W. P. Carey School of Business at Arizona State University and Director of the school’s JPMorgan Chase Economic Outlook Center. The Center maintains the Western Blue Chip Economic Forecast and Greater Phoenix Economic Forecast websites. McPheters also oversees the Job Growth USA website that tracks employment for all industries in the U.S. states and metropolitan areas. The website is frequently used by economists, financial analysts, economic development specialists, and, during election season, fact-checking organizations to evaluate claims by candidates regarding job creation statistics and policies. His writings on the Western region have been quoted in the Wall Street Journal, USA Today, The Economist, Business Week, The New York Times, and Newsweek as well as major metropolitan area newspapers throughout the nation. He has appeared nationally on Good Morning America, Fox News, Marketplace on NPR, and CNN commenting on the economic outlook. As director of the Economic Outlook Center, since 1987 McPheters has delivered a cumulative total of more than 500 speeches and presentations to various public and private audiences at business and academic conferences in Arizona and across the nation.
McPheters has published numerous articles in books and professional journals on various topics, including immigration, executive compensation, monetary policy, international business cycles, and issues in law and economics. At the federal level, his work has been supported by the United States departments of Justice, Transportation, Agriculture, and the Treasury. In Arizona, he has completed research projects for the Arizona Department of Transportation, Sky Harbor International Airport, Boeing, and many other public and corporate sponsors.
McPheters completed his undergraduate studies at San Francisco State University and received his Ph.D. in economics from Virginia Polytechnic Institute. He has been at ASU since 1976, teaching courses at the undergraduate and graduate level in monetary and regional economics. In addition, has held various administrative positions at ASU including Senior Associate Dean for Graduate Programs in the W. P. Carey School of Business from 1991 – 2008.
