Case Study #1 Jennifer is a 29-year-old administrative assistant married to Antonio, an Italian engineer, whom Jennifer met four years earlier while on a business trip for her marketing company. The couple now lives in Nebraska, where Antonio works for the county's transportation department and Jennifer commutes an hour each way to her marketing office. The chest pains began when Michael signed his final divorce papers, ending his 15-year marriage. He fought for joint custody of his two children, ages 12 and 10, but although he wants to be with them more frequently, he only sees them every two weeks. This schedule is, in great part, a result of his employer's announcement that budget constraints would result in layoffs. For this assignment I will use my colleague at work Irene who has gone through separation with her husband due to abuse of drugs. Separation refers to the situation when two people man and female who had been legally married are no longer living together, however they have not legally dissolved their marriage. Separation of married people is common problems affecting many families across United States. The case of Irene is not exceptional there other people undergoing same family problems over the world. Irene has separated with her husband whom they have been married for the last ten years. Irene family has been experiencing separation problems for the last five months now when issue started. Despite her efforts to have good family she has accepted that separation is necessary for the last five months now. The problems of separation between Irene and her husband mostly have affected their two children negatively in several ways. Since their children are aged below ten years they do not understand separation of the parents. Drug abuse has been singled out as the main reasons for separation between Irene and her husband. Particularly excessive consumption of alcohol by husband been blamed for problems facing Irene family. As result of drug by Irene family has gone through hard times both financially and emotionally. The husband has not been in a position to hold any kind for job because of absenteeism and poor performance. He has not been able to pay bills for the family as well has provision of necessities of life for their young family. He felt dizzy and fell down, he could not use his legs, and he began to scream. By 10:00 p.m., he was hallucinating and was transported to the DC General Hospital by ambulance. He went into convulsions. His stomach was pumped, and they found traces of marijuana and possibly PCP. He soon stopped breathing, and by the next morning, brain scans showed no activity. Four months later, Yusef’s condition had not changed. The physicians believed his brain was not functioning and wanted to pronounce him dead based on brain criteria. Several difficulties were encountered, however. First, there was some disagreement among the medical personnel over whether his brain function had ceased completely. Second, at that time the District of Columbia had no law authorizing death pronouncement based on brain criteria. It was not clear that physicians could use death as grounds for stopping treatment. Most important, Ronald Camp, the boy’s father, protested vigorously any sug- gestion that treatment be stopped. A devout Muslim, he said, “I could walk up and say unplug him; but for the rest of my life I would be thinking, was I too hasty? Could he have recovered if I had given it another 6 months or a year? I’m leaving it in Almighty God’s hand to let it take whatever flow it will.” The nurses involved in Yusef’s care faced several problems. Maggots were found growing in Yusef’s lungs and nasal passages. His right foot and ankle became gangrenous. He showed no response to noises or painful stimuli. The nurses had the responsibility not only for maintaining the respiratory tract and the gangrenous limb, but also for providing the intensive nursing care needed to maintain Yusef in debilitated condition on life support systems. Had the aggressive care been serving any purpose, they would have been willing to provide it no matter how repulsive the boy’s condition was and in spite of there being many other patients desperately needing their attention. However, some of the nurses caring for Yusef were convinced that they were doing no good what- soever for the boy. They believed they were only consuming enormous amounts of time and hospital resources in what appeared to be a futile effort. In the process, other patients were not getting as much care as would certainly be of benefit to them. Could the nurses or the physicians argue that care should be stopped because he was dead? Could they overrule the parents’ judgment about the usefulness of the treatment even if he were not dead? Answer below discussion. Two paragraphs and two references no later than 5 years. This week’s discussion questions caused a lot of reflection on how I practice nursing. The questions led me to reflect on what type of nurse I am. Am I doing the best I can, or have I done the best I could? While considering the six ways of knowing I kept coming back to unknowing. The act of being unaware or not being able to fully understand what the patient is experiencing or going through (Heath, 1998). That is how I approach every patient. I have no preconceived notion that I have even the faintest idea of what they must be feeling. It is the ability to be fully present of yourself and open to the patient and their life. The other is empirical knowing. Empirical knowing is the first introduction we have in nursing school. Understanding the science, the epidemiology, the physical nature of the illness or medical situation. I believe there is a lot of value in empirical knowing, as the patients want us to know why and how to do what we do. They place a lot of trust in us to do the right thing. It is part of our daily responsibility to assess and monitor therapeutic response to treatment (Zander, 2007). Though the entire 6 ways of knowing were brand new to me and it took me some time to reflect on my understanding of these concepts, I fear the two I have put the least focus on in the past but am keenly aware of their implications are emancipatory and aesthetic knowing. This is not to say that I did not practice the art of nursing, using my years of experience to draw on and I am astutely aware of the social, cultural, and political implications of those in my care. I never was able to put a name or definition to them. Emancipatory knowing, the ability to be astutely aware of the social injustices and be able to act in a way to reduce the negative impact of these inequalities (Peart & MacKinnon 2018). Aesthetic knowing, the ability to interpret the patient’s behavior or expression of self and then asking what this means (Zander, 2007). Aesthetic knowing in a simpler phrase might be considered that gut feeling a nurse often refers to. I am reminded of a recent experience I had when I was given the privilege to assist another nurse on a home visit. This nurse is an incredible pediatric nurse. I admire her aesthetic way of knowing and how she is so patient and kind with the children she cares for. This young boy, age 11, was recently diagnosed with Pediatric Autoimmune Neurological Disorders Associated with Streptococcal Infections (PANDAS). It is a horrid and unexpected disease that takes an otherwise perfectly healthy child and they become severely paranoid, have expressions of sudden onset OCD type behaviors, and a myriad of other symptoms (International OCD Foundation, n.d.).
