4. Eligibility is based on a strict assessment
of you or your loved ones health needs.
5. Do you suffer from…
Complex medical conditions that need additional care and support
Long-term medical conditions
Physical or mental disabilities
Terminal illnesses
Rapidly deteriorating health
Mobility problems
Behavioral or cognitive disorders.
6. If you answered ‘YES’ to any of the
previous list then you or your loved one
could be eligible for NHS Continuing Care
Funding.
7. If you answered ‘NO’ to all of the above, it
is still worth arranging an assessment.
The list is not finite.
8. Ask your GP or social
worker to arrange an
assessment.
9. ASSESSMENT
If the initial assessment points at you being
eligible for Continuing Care Funding, a
secondary assessment will be carried out.
10. Two or more health care professionals will assess:
Behaviour
Cognition (understanding)
Communication
Psychological and Emotional needs
Mobility
11. Nutrition (food and drink)
Continence
Skin (including wounds and ulcers)
Breathing
Symptom control through drug therapies and medication
Altered states of consciousness
Other significant care needs
13. If you have at least one priority need or two severe
needs, you should qualify for funding.
If you have one severe need and a number of other
high or moderate needs, you may also be eligible.

17. Are you already in a care home but have never
been assessed?
Get Assessed!
18. By getting assessed you may find that you are
entitled to Continuing Care Funding and possibly a
refund for previous care which you have already
paid for which should have been covered.
19. If you or a loved one were never assessed or if you
believe your original assessment to have now
changed, it is possible to make a care home claim.
How?
20. If you feel your situation has changed or if you
were unaware that you maybe entitled to
Continuing Care Funding, then you need to
contact both your GP and a Solicitor.
Why?
21. You or your loved one will have to be reassessed in
order to pursue a care home claim. A GP can
arrange this.
A solicitor will be needed to raise the claim on your
behalf.

Seek a solicitor with experience in this field. The

you successfully through it.
