Respected Sir / Mam,
I Mangala V having the above subjected saving bank a/c with your SIET Branch,
which is marked as dormant. Due to my sickness I have not operated my bank account
for more than two years & I have lost my bank passbook & Debit card in transit.
I humbly request you to activate my above said SB a/c since I have to encash my
cheque to meet my medical expenses, further I like to inform that few more cheques are
likely to be received by me in couple of years by the way of maturity of fixed deposits
placed with some other institutions where I have mentioned the above bank account
number.
Further I authorize my daughter B. Nagamani to operate my above said bank
account since am not able to come to the bank to transact.
