This document presents the case of a 26-year-old woman and her 30-year-old husband who have been unable to conceive after 3 years of marriage. Evaluation found the woman has polycystic ovarian syndrome and irregular menstrual cycles. The man was found to have obstructive azoospermia, with no sperm found on multiple semen analyses. Both partners underwent further testing and examination, and the diagnosis was primary infertility due to female and male factors - her PCOS and his obstructive azoospermia.
