perform a limited asbestos survey at the subject property. This limited asbestos survey was conducted for
the purpose of identifying the presence of asbestos containing materials (ACM) as required by state and/or
federal regulatory agencies in the areas of the building that was part of the scope of work communicated to
Prokop Environmental, LLC. The specific limitations of the area(s) and suspect ACM tested as part of this
survey can be found in the Scope of Work Limitations section on page 3 of the attached report. This survey
was performed by a Colorado Department of Public Health and Environment (CDPHE) certified asbestos
inspector and in compliance with applicable federal, state, and local regulatory rules.

See the Scope of Work Limitations section on page 3 of this report for details on the specific area(s) and
material(s) that the asbestos testing for this project and report is limited to. This limited asbestos survey was
conducted following federal and CDPHE protocols for identifying ACM. Areas of suspect ACM may be
present in areas not visible or accessible during this limited asbestos survey. Prior repairs or remodeling of
the subject property and building components may have altered building materials so that suspect ACM
could not be identified during this inspection. If any additional suspect ACM is discovered these materials
should be properly sampled and tested for asbestos content. Reasonable attempts have been made to ensure
that this report is complete and accurate with respect to Prokop Environmental, LLC’s authorized scope of
investigation. Our site visit was of a limited nature with emphasis on bulk asbestos testing based on the
scope of work communicated to Prokop Environmental, LLC at the subject property identified herein. Test
results represent the samples collected and the conditions present at the time of the inspection and are limited
to the scope described herein. This report shall not be reproduced except in full and only with the written
approval of Prokop Environmental or the client.
Our services consist of professional opinions, conclusions and recommendations that are made in accordance
with generally accepted consulting standards, principles and practices. Prokop Environmental assumes no
liability for any loss, injury, claim, or damages arising directly or indirectly from any use or reliance on this
report or the opinions expressed herein. Prokop Environmental makes no risk assessment, warranty,
guarantee or insurance policies of any kind, express or implied.
As part of this limited asbestos survey a CDPHE certified asbestos building inspector performed an onsite
inspection of the property to visually and physically identify suspect ACM and it’s condition within the area
of the building that was damaged and/or part of the scope of work that was communicated to Prokop
Environmental, LLC (see Scope of Work Limitations on Page 3). The identified suspect ACMs were each
assigned a homogeneous area (HA) number and randomly sampled in accordance with regulatory guidelines.
Homogenous areas are defined as an area of material that is uniform in color and texture. The HA list for
this project including the description, location, categorization, and condition of each material on the HA list
is attached to this report. Also attached is a digital photo of each HA. Specific sample location, material
type, condition, classification, and material quantification can be found on the attached charts.
Materials that were not observable, accessible, or not part of the scope of work communicated to Prokop
Environmental, LLC are not included in this limited survey. If additional materials that are not included in
this survey are found, samples of these materials should be tested for asbestos content.
The samples collected were delivered under chain of custody to
Voluntary Laboratory Accreditation Program (NVLAP) and CDPH&E certified laboratory. Laboratory
accreditations and certifications are attached to this report. Laboratory analytical method used: EPA Method
600/M4-82-020; 600/R-93/11.

The following material(s) were found to contain greater than 0% but less than 1% asbestos:
● The Texture located on the Stairwell and Common Hallways Walls. This material is categorized as
Surfacing, Friable, and in Significantly Damaged Condition. These samples have not been point
counted. See NOTE 2 below regarding trace material regulations and point counting. At the time of
the inspection there was 225 square feet of this material was damaged.
NOTE 1: In accordance with CDPHE regulations joint compound is allowed to be composited. By
composite calculation the Wall/Ceiling System is considered to be less than 1% asbestos containing. If this
joint compound is going to be impacted directly (e.g. sanding) then the project must be handled as an
asbestos abatement project, subject to all applicable requirements of Regulation No. 8. If not impacted
directly, the material is not considered ACM and does not fall under the abatement requirements of Colorado
Regulation 8. Contractors and building owners be advised that other regulation and guidelines such as
OSHA may still apply for worker and general public safety.
Findings/Conclusions:
The following material(s) were found to contain greater than 1% asbestos:
● The Popcorn Texture located on the Stairwell and Common Hallways Ceilings. This material is
categorized as Surfacing, Friable, and in Significantly Damaged Condition. At the time of the
inspection there was 40 square feet of this material was damaged.
● The Joint Compound located on the Stairwell and Common Hallways Walls. This material is
categorized as Surfacing, Friable, and in Significantly Damaged Condition.

but 1% or less (trace) asbestos must be considered asbestos containing materials unless a point count analysis
has been performed on each trace sample layer and the point count analysis results confirm that the material
is 1% or less asbestos containing. If a point count analysis has not been performed then any trace materials
must be considered to be asbestos containing material and handled according to Colorado Regulation 8
regulations. Trace wall textures for this report have not been point counted and are considered asbestos

PLM samples were analyzed utilizing the Environmental Protection Agency's Test Method: Method for the Determination
of Asbestos in Building Materials (EPA 600/R-93/116. July, 1993) 40 CFR, Part 763, Appendix E to Subpart E. Reporting
Limit <1% Asbestos. Additional treatment and tests may be required to accurately define composition (i.e. ashing, extractions,
acetone treatment, and TEM). Unused portions of samples are archived for one year unless client requests special handling.
Samples not taken by laboratory personnel.
Laboratory analysis was accomplished utilizing an Olympus BH-2 polarized light microscope. The microscope is
equipped with dispersion staining lenses.
Atlas Technical Consultants LLC is accredited by NVLAP Bulk Asbestos Sample Quality Assurance Program (Lab
Code 102031). Atlas participates in the NVLAP Bulk Asbestos Sample Quality Assurance Program and maintains
an in-house QC/QA program for bulk samples whereby 10% of all submitted samples are reanalyzed and
documented in a Quality Control Manual. Atlas also participates in a quarterly round robin QC/QA program for bulk
samples with several accredited laboratories throughout the United States.
Approved Signatory :
The non-detection of asbestos fibers in floor tile by PLM is of itself inconclusive. Confirmation by Transmission
Electron Microscope (TEM) is recommended for negative floor tile samples.
This report must not be used by the client to claim product endorsements by NVLAP or an agency of the U.S. government.
This test reports only to the items stated. This report is confidential to customer. Results are not published on a public domain.
Samples were analyzed by Jeff Lomme, Laboratory Director. Mr. Lomme is a professional geologist who has
successfully completed the McCrone Institutes's "Advanced Asbestos Identification" Course.
