5-hydroxytryptamine or 5-HT or Serotonin is a neurotransmitter that serves a range of roles in the human body. It is sometimes referred to as the happy chemical since it promotes overall well-being and happiness.
It is mostly found in the brain, intestines, and blood platelets.
5-HT is utilised to transport messages between nerve cells, is known to be involved in smooth muscle contraction, and adds to overall well-being and pleasure, among other benefits. 5-HT regulates the body's sleep-wake cycles and internal clock by acting as a precursor to melatonin.
It is hypothesised to regulate hunger, emotions, motor, cognitive, and autonomic processes.

Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

Product Design Trends in 2024 | Teenage EngineeringsPixeldarts
The realm of product design is a constantly changing environment where technology and style intersect. Every year introduces fresh challenges and exciting trends that mold the future of this captivating art form. In this piece, we delve into the significant trends set to influence the look and functionality of product design in the year 2024.
How Race, Age and Gender Shape Attitudes Towards Mental HealthThinkNow
Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).
AI Trends in Creative Operations 2024 by Artwork Flow.pdfmarketingartwork
Creative operations teams expect increased AI use in 2024. Currently, over half of tasks are not AI-enabled, but this is expected to decrease in the coming year. ChatGPT is the most popular AI tool currently. Business leaders are more actively exploring AI benefits than individual contributors. Most respondents do not believe AI will impact workforce size in 2024. However, some inhibitions still exist around AI accuracy and lack of understanding. Creatives primarily want to use AI to save time on mundane tasks and boost productivity.
Organizational culture includes values, norms, systems, symbols, language, assumptions, beliefs, and habits that influence employee behaviors and how people interpret those behaviors. It is important because culture can help or hinder a company's success. Some key aspects of Netflix's culture that help it achieve results include hiring smartly so every position has stars, focusing on attitude over just aptitude, and having a strict policy against peacocks, whiners, and jerks.

Google's Just Not That Into You: Understanding Core Updates & Search IntentLily Ray
1. Core updates from Google periodically change how its algorithms assess and rank websites and pages. This can impact rankings through shifts in user intent, site quality issues being caught up to, world events influencing queries, and overhauls to search like the E-A-T framework.
2. There are many possible user intents beyond just transactional, navigational and informational. Identifying intent shifts is important during core updates. Sites may need to optimize for new intents through different content types and sections.
3. Responding effectively to core updates requires analyzing "before and after" data to understand changes, identifying new intents or page types, and ensuring content matches appropriate intents across video, images, knowledge graphs and more.

One of these lands in your inbox
Can be a little bit stress inducing
Might not be what you need
The Magento community has a lot about patches
We can talk to each other about them
until we are blue in the face
Just like we can anything Magento related
What we aren’t so great at taking about though
is mental health
Well, we should.
It is easy to think there’s no right place
to talk about mental health.
80% of people suffering from mental health issues
do not speak up and do not seek help.
So many of us who struggle
are made to feel isolated, worthless and ashamed.
But the more we talk about it,
the better life is for all of us.
It is estimated that 1 in 6 people in the past week
Have experienced a common mental health problem
And so often we don’t know what to do about it
Both for ourselves and others.
But we can’t help others unless we are in a good place ourselves
Which is why I want to talk to you about applying your patches.
If you search google for the definition of a patch
This is the answer you will get
A piece of cloth or other material
used to mend or strengthen
a torn or weak point.
Just like Magento and those jeans
We all sometimes need to apply a patch
So that we don’t wear our selves out
So that we aren’t worn so thin we can’t function
Your patches are personal to you
You need to figure out what makes you tick and tock
So I am just going to talk about some general patches
Unlike Magento patches that are only every so often on Fridays
These patches should be applied everyday
That’s right. It’s patch day, every day!
Now, I’m no expert and I often struggle with a lot of these patches
But I find talking about it
and being more aware of them really helps
This is your food patch.
Don’t eat. Don’t live.
Simple? Nope.
Diet is a crucial to both physical and mental well-being.
Many of us, including myself, go through life
Not really paying much attention to what we eat
For me its normally whats within arms length
But that does mean our diets consist of lots of
highly processed, nutrient poor, energy dense foods
We are undernourished but overfed.
And because the effects
on our mental health aren’t instant.
Y’know The crap we eat today,
Actually makes us feel crap tomorrow or later this week even
We don’t always put a high importance on it
Yes we think about it when we want to lose physical weight,
But we don’t think about it when it comes to emotional weight
So just being a little more conscious of it will mean
You can give that complex organ in your head
The vitamins, minerals, fats and amino acids it needs
To summarise: eat crappy = feel crappy.
Ah the sleep patch.
We all know this seriously effects our mood
“Someone got up on the wrong side of the bed this morning”
Adults typically need between
7 and 9 hours of shut-eye a night
to function at their best
For those that struggle to get this amount
and those with insomnia
Are at an extremely high risk of increased
anxiety, stress, and depression
And sleep debt is a thing.
It’s very similar to technical dept.
Doing it properly once,
does not make up for all the shortcuts you took.
A single early night will not make up for months of rubbish sleep
If you have significant sleep debt,
you can expect recovery to take several weeks.
You do also need to be weary of too much sleep
For me, this is my downfall
When I am feeling especially low
Or if I have a lot on
Or I don’t feel in control of my life
All I want to do is sleep and I am almost constantly fatigued
And it’s just as bad for you as too little sleep
It is also known as hypersomnia
According to the National Sleep Foundation,
up to 40% of people experience
symptoms of hypersomnia
Which include anxiety, low energy,
and memory problems
Exercise!
Gosh I am so anti-exercise
And moving in general
muscular physique
But it’s so important
I wish I didn’t HATE exercise
and in particular, running as much as I do
I actually get goosebumps at the thought
of having to run. Actual chills.
BUT Occasionally my partner will drag me out,
kicking and screaming, and often crying
And, don’t tell her, but I can’t half feel the benefits of it.
Its like my body is so grateful for me using it for what it was made for
that everything feels more in sync and my mind feels clearer.
Yes, I know it can be hard if you are like me
and would rather… not.
but it does make a huge difference to your mental wellbeing
So start small and try to walk more
or just generally move more
Going outside is so important for our wellbeing
When you are battling a mental heal issue
Finding the motivation and energy
to do something as simple as going outside
can be a real struggle
But it can be exactly what you need
The increase in vitamin D, and natural light
do wonders for lifting your mood
And it can actually help you sleep better at night
as humans, we were made to be awake when it’s light
and asleep when it’s dark
mindfulness is the buzzword of mental health
And I spent a long time not actually
understanding what it meant
I think I thought it was yoga poses on a beach
Or a pile of rocks and rippling water
Alas, no.
To be mindful is to be conscious
or aware of something.
And you don’t have to meditate or do yoga
To become more self aware
In fact if you take the pressure off what it means to be self aware
It can be quite easy
What is the loudest voice in your head?
For me, it’s my self critic.
The one that tells me over and over how spectacularly I failed
Yeah it’s a noisy one
When she pipes up I can physically feel my body cowering
But this critical voice is by no means unique to me
As mammals we are threat focused
trampled by a mammoth
To being fired or failing an exam or missing a deadline
But how we cope is still aggressively the same
So when this critic pipes up, it can be really helpful to listen
And then ask, why?
Why is the self critic doing this?
Usually it is coming from this threat defence instinct that we all have
It’s trying to protect us in some way
Let’s say you are unfit and your self critic beats you up about it
Well maybe that’s because at some level
its genuinely worried about your health
Or if you don’t reply to a friends text, or you miss their birthday
And your critic gives you hell for it and makes you feel awful
Its probably because its worried about you losing those relationships
So instead of withdrawing and pushing these feeling away
It may be more productive to listen and then even thank that critic
Hey, critic - I appreciate what you are trying to do for me
And then your critic can be like, oh ok. And actually pipe down a bit.
Every part of your brain is actually working to keep you alive and well
Even though it can feel like it’s not sometimes,
Your instincts are for survival
Saying no
It’s hard when you want to help others
And get involved in everything
And do everything
All the time
But if your plate is full,
be honest with yourself and those around you.
Stop taking things on!
Personally, I am terrible at this.
I think everything is a great idea and I want to do it now.
I have to really fight an internal battle to turn away an opportunity,
be it a suggestion or invite from someone else,
or a random ass idea that popped into my head.
Our brains don't like to be overloaded,
no matter how good you think you are at multitasking
Focus is key!
One bit of advice that I found extremely useful was that
If saying yes feels heavy
Say no.
Thank you, but no.
This is THE most important method for
preventing and reversing mental health issues.
But sometimes I’d rather go for a run than ask for help.
I’d rather run and hide, than actually get the support I need.
Stress makes us withdraw and often I don’t even know why that is.
Is it pride, feeling shy, scared of embarrassment or being seen as a failure,
does saying it out loud make the problem more real,
Being able to get support for both our work and personal affairs is vital.
We can’t do it all, all of the time.
Sometimes we need to share the load a bit.
Loved ones, team members, friendly folk - will be and are there for you.
They are your most powerful allies in the war for wellbeing .
Heck, ask me! Play on my inability to say no!
Whatever you do, however you feel, talk about it
Share it out I promise it will help.
And you will help others by doing so.
When a friend, or a colleague battling a mental illness
It can be hard to know what to do and
Unfortunately there is no one single answer to this question.
But the best thing you can do is ask.
It is such a shame that “How are you?”
Has become such a throw away question
I’m alright, you? Yeah, I’m alright…. nothing.
It can feel awkward to ask again, but it makes a huge difference
Show that you want to know if they are really all right.
and if there is anything they want to talk about
Or anything you can help with
Bare in mind, they may not be ready to talk about it
If they turn down your support, don’t take it personally,
And don’t stop showing that you are there for when they are ready
Yes you can apply patches to help others too.
If they are ready to talk, be ready to listen.
Let them take the lead.
Avoid advice or trying to fix their problems
Also, give yourself a break too.
You don’t need to be an expert, or a doctor to offer some support.
So relax, don’t force yourself
or the person you are trying to help
Into a therapy session
Make sure you tell them it’s ok to feel the way they do.
And that you would never judge them.
Or let it change the way you feel about them.
Our minds can be a dark place, we all know that
People have different experiences
Ask open-ended questions.
Help them to talk, not just say “yes” or “no”.
And don’t rush them. It’s so important that they open up at their own pace.
If there are things they don’t want to expand on, don’t force them to
Have they seen a doctor or spoken to a professional?
Do they want to?
How can you help them find the help they need.
It’s ok to say you don’t know how to help,
but you can help them get it.
If you want to know more on helping others
And helping yourself there are resources out there to help you
These are a few short links that you can go to find out more.
Magento Mental Health or MageMH is something I have recently set up
To get the conversation going and keep going
I have also set up a slack group which is free and open to sign up for a join
Because I believe this is important
and will help our community grow and strengthen
Myself and David Manners, Community Engineer at Magento
Are also going a 3 hour live steam this Thursday with OSMI charity guests
And lots of advice, stories and chat
