home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the

Mrs. M was a 54 year-old woman who was transferred to a tertiary care hospital's critical care unit from a
community hospital. She had been diagnosed with an acute anterior wall myocardial infarction (heart
attack). Secondary diagnoses were acute pancreatitis, disseminated intravascular coagulation, acute
respiratory failure, and lactic acidosis. She was placed on a ventilator. Due to medication and her serious
medical problems, she was only periodically alert, but responsive when directly addressed. There were
no written advance directives.
She was hospitalized in 1990 for acute pancreatitis. She also had a history of anxiety and depression,
which were treated by Haldol and Prozac for several years. She had attempted suicide about 10 years
ago.
Her husband and daughter (Martha), were supportive. A son was kept out of the decision-making process
because of a history of depression and the fear that he may harm himself. Mr. M and Martha voiced
agreement that Mrs. M should make her own decision regarding treatment or withdrawal from the
ventilator. Mr. M said his wife had spoken about potential end-of-life situations. She was clear that she did
not want to be kept alive if the quality of her life would be more compromised than it already was.
For the first three days of hospitalization she was aware and responsive. She was presented with the
possibility of pancreatic surgery to relieve her from the terrible pain she was experiencing, and told that
the surgery was high risk with a 50 percent chance she could die in surgery. Recovery would require
extensive respiratory care, which meant time in an extended care facility. She was also informed that she
now required dialysis. She declined the surgery and dialysis expressing a desire to be withdrawn from the
ventilator.
The physician agreed to her request, which had the support of Mr. M and Martha. On the fourth day of
hospitalization, the doctor had a conversation with Martha and hesitated on withdrawing the ventilator. He
called for an ethics committee consult. He became concerned about Mrs. M's age and potential to rally
medically. He was also concerned about her past history of depression, and wondered whether she
desired a kind of physician-assisted suicide. He also began to question her competence and/or decision-
making capacity.
Mr. M and Martha were angered at this decision-making reversal and the consult with the ethics
committee. They had told Mrs. M that her decision would be honored and they were in the process of
already struggling with the grief that would be inevitable. Such ambivalence caused even more conflict
and anxiety.

Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrangement
with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed

The Case of Mrs. M
Mrs. M was a 54 year-old woman who was transferred to a tertiary care hospital's critical care unit from a
community hospital. She had been diagnosed with an acute anterior wall myocardial infarction (heart
attack). Secondary diagnoses were acute pancreatitis, disseminated intravascular coagulation, acute
respiratory failure, and lactic acidosis. She was placed on a ventilator. Due to medication and her serious
medical problems, she was only periodically alert, but responsive when directly addressed. There were
no written advance directives.
She was hospitalized in 1990 for acute pancreatitis. She also had a history of anxiety and depression,
which were treated by Haldol and Prozac for several years. She had attempted suicide about 10 years
ago.
Her husband and daughter (Martha), were supportive. A son was kept out of the decision-making process
because of a history of depression and the fear that he may harm himself. Mr. M and Martha voiced
agreement that Mrs. M should make her own decision regarding treatment or withdrawal from the
ventilator. Mr. M said his wife had spoken about potential end-of-life situations. She was clear that she did
not want to be kept alive if the quality of her life would be more compromised than it already was.
For the first three days of hospitalization she was aware and responsive. She was presented with the
possibility of pancreatic surgery to relieve her from the terrible pain she was experiencing, and told that
the surgery was high risk with a 50 percent chance she could die in surgery. Recovery would require
extensive respiratory care, which meant time in an extended care facility. She was also informed that she
now required dialysis. She declined the surgery and dialysis expressing a desire to be withdrawn from the
ventilator.
The physician agreed to her request, which had the support of Mr. M and Martha. On the fourth day of
hospitalization, the doctor had a conversation with Martha and hesitated on withdrawing the ventilator. He
called for an ethics committee consult. He became concerned about Mrs. M's age and potential to rally
medically. He was also concerned about her past history of depression, and wondered whether she
desired a kind of physician-assisted suicide. He also began to question her competence and/or decision-
making capacity.
Mr. M and Martha were angered at this decision-making reversal and the consult with the ethics
committee. They had told Mrs. M that her decision would be honored and they were in the process of
already struggling with the grief that would be inevitable. Such ambivalence caused even more conflict
and anxiety.

The defendant was informed that the results of the evaluation will be released to the court and that the results may be used against her in the court. Ms. Tyler gave her written consent to be evaluated.
Psychosocial History:
Only limited information is available regarding Ms. Tyler's background. She is a lifetime resident of the state. Her mother died of cancer at the age of seventy-nine years. Her father, L. Defendant, is eighty-four years old and is a retired farmer. Ms. Tyler has five living sisters, four living brothers, and two deceased siblings. Three of her sisters have received inpatient psychiatric treatment. One sister lives in Close Town, one in Europe with her husband who is in the military, and one sister died in a drowning accident. One brother committed suicide by hanging himself. There is no history of psychiatric problems among the other brothers.
Ms. Tyler has completed the tenth grade. She is literate. Ms. Tyler has worked on an assembly line for five years until 1988 when the factory closed. She then did an office-cleaning job. Her reasons for leaving this position are vague. She was unemployed for several years and was supported by her family. She lives in rent-free government housing. Her church pastor, A. Reverend, arranged for her to get employment at Helpful Industries through a vocational office at the State Psychiatric Hospital. She was employed at Helpful Industries for approximately two months prior to the incident. Ms. Tyler has never married. She has a thirteen-year-old son who lives with his father.
Psychiatric History:
Ms. Tyler denies any previous psychiatric treatment. Family members and individuals who know Ms. Tyler state that she has a long history of psychiatric symptoms such as loose associations and persecutory delusions. Ms. Tyler's sister, Ms. Sister, reports that Ms. Tyler has been violent in the past, threatening her niece with a knife. Her emotional difficulties were also apparently common knowledge among her congregation. As stated above, Ms. Tyler has a positive family history for psychiatric illness. Ms. Tyler is currently on haloperidol, an antipsychotic medication.

She decided to have hospice care at home and, with the help of her family, set up her first floor as a living and sleeping area. She was cared for by family members around the clock for approximately 3 days. Sandra observed that she was tiring everyone out so much that they could not really enjoy each other’s company. At this time, she contacted the Visiting Nurse Association (VNA) to seek assistance. Her plan was to try to enjoy her family and friend’s visits. Working With Families:
The Case of Carol and Joseph
Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrange-
ment with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for addi-
tional case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father sepa-
rated when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrange-
ments for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially

her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels exten-
sively in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature

Margins
The left-hand margin must be 1inches (4 cm.). Margins at the right, top, and bottom of the page

Line Spacing
Double-spacing is required for most of the text in documents submitted during the Research

If "Never work!" was the apex of critical strategy in
the twentieth century, perhaps "Never play!" could
be the same point of extreme negation for the twenty-
first. It is of course almost impossible to never work,
but it at least defines an ambition: to abolish wage
labor and the commodity form.
It was and remains a surprisingly popular ambi-
tion. The cycle of struggles in the overdeveloped
world in the late twentieth century took it as their
lodestar, whether unwittingly or not. The whole
counterstrategy of making work seem like some-
thing else, like play even, stems from the boredom
that both wage labor and the commodity form
generated.
This boredom is spreading even to what was once
called the underdeveloped world. By their tens of
millions, peasants left the land, in China and else-
where, and in some cases ended up in the factories
that make the world's commodities. But it's getting
harder to keep them there. The riots and suicides at
Foxconn are just the tip of the iceberg. Nobody much
likes to do this sort of work if there's any option.
In the overdeveloped world, work is only one of
the ways of creating a value chain and extracting a
profit. The more advanced form of spectacular
economy extracts value from play. This is why the
critical slogan of the times might need to be "Never
play!" Where play was some kind of alternative for
so many late-twentieth-century avant-gardes, from
the Situationists to the Fluxus movement to the New
Games movement, the overdeveloped world in the
twenty-first century is all about recuperating those
energies, those desires, those appetites, for the com-
modity form.

This study explores the role of urban public spaces for democratic and social engagement.
It examines the impact of wireless Internet use on urban public spaces, Internet users, and
others who inhabit these spaces. Through observations of 7 parks, plazas, and markets in 4
North American cities, and surveys of wireless Internet users in those sites, we explore how
this new technology is related to processes of social interaction, privatism, and democratic
engagement. Findings reveal that Internet use within public spaces affords interactions with
existing acquaintances that are more diverse than those associated with mobile phone use.
However, the level of colocated social diversity to which Internet users are exposed is less
than that of most users of these spaces. Yet, online activities in public spaces do contribute
to broader participation in the public sphere. Internet connectivity within public spaces
may contribute to higher overall levels of democratic and social engagement than what is
afforded by exposure within similar spaces free of Internet connectivity.

Internet access in public parks, plazas, markets, and streets has been made possible
through the proliferation of broadband wireless Internet in the form of municipal
and community wi-fi (e.g., NYC Wireless) and advanced mobile phone networks
(e.g., 3G). The experience of wireless Internet use in the public realm contrasts with
traditional wired Internet use, which is confined primarily to the private realm of the
home and the parochial realm of the workplace. An extensive literature has addressed
the influence of Internet use on the composition of people’s social networks
(Hampton, Sessions, & Her, in press), their engagement in political, voluntary, and
other organizational activities (Boulianne, 2009), and their interactions within home
and workplaces (Bakardjieva, 2005; Quan-Haase & Wellman, 2006). But, Internet
use in the public realm has remained relatively unexplored. This type of use carries
with it significant implications for urban planning, the structure of community, and
the nature of democracy.

Appendix C - Mrs. Thomas Give the questions some thought and answer honestly Give examples if you have them Cite resources Number your answers to correspond with the question Worth 12 points / 5% Scenario: Mrs. Thomas is a 54, year old African American widow, mother and grandmother, who lives with her daughter and four grandchildren (ages 12, 10, 7 and 5) in a 4 story walk up apartment. She is an active member of her church community and friends, comments that she had so much energy that she exhausted all of them just being around her. At age 51, she was diagnosed with non-Hodgkin’s lymphoma. Busy with raising her grandchildren, a little more than 3 years went by before she sought attention for her symptoms and was diagnosed. Despite aggressive treatments with chemotherapy and radiation, her diseased progressed and she was considering undergoing a bone marrow transplant. Climbing the stairs to the apartment one afternoon she became very short of breath and collapsed. Her twelve, year old granddaughter called 911. At the hospital she was minimally responsive and in severe respiratory distress. She was intubated and transferred to the ICU. A family meeting with the oncology and ICU team was called to discuss Mrs. Thomas’s advanced condition, the fact that she would probably not
- 2. survive further treatment of the lymphoma and to decide on goals of care. Fifteen family members arrived, including her daughter, pre-teen granddaughters and grandson, three nieces, four nephews, several friends from her church and the minister. On being asked that only the immediate family participate in the meeting, the family and friends became angry and insisted that all of them be involved in this discussion. Discussion questions: What is your impression regarding this scenario? What are some concerns you have with this case and what do you anticipate would happen? Support your answer. Have you thought about what kind of care you would want if you could no longer speak for yourself at the End of Life? Give examples. Explain and describe the pain you were experiencing. What were the effective methods used to deal with the pain? Describe pharmacological and non-pharmacological therapies used to relieve pain. Did those around you understand the pain you were experiencing? Why or why not? How did their understanding or lack of understanding make you feel? How could this experience with pain impact your response to a patient with pain?
