JOB DESCRIPTION
Complete a head to toe assessment and a social assessment of the patient upon
admission.
Develop a written nursing care plan upon admission, indicating the independent and
collaborative nursing functions. This is reviewed and updated daily or when a new
nursing problem arises.
Initiate cardiac monitoring, oxygen therapy, non-invasive blood pressure and
oximetry monitoring. Start and evaluate intravenous access. Execute insertion of
nasogastric tube and urinary catheter. Does assessment and continuous monitoring of
drainage systems in place such as: chest tube, Jackson pratt, and nasogastric drainage.
Perform blood withdrawal from an arterial line, obtain readings from a pulmonary
catheter.
Recognize electrocardiogram readings, correlates with clinical symptoms presented.
Intervene with physician orders based on ACLS Algorithm.
Continuous re assessment of patient to determine progress and trends in the health
status that require referral to the physician.
Provide routine patient care. Close monitoring of vital signs, precise regulation of
fluids administered, careful repositioning of patients with contraptions, ensure bedside
cleanliness at all times. Performs suctioning, accurate recording of intake and output,
renders daily bed bath, hair shampoo as ordered, change of linens and regular oral
care. Anticipate the needs by efficient communication.
Carry out Physician’s order in a systematic manner, collaborate with other
departments such as the Operating Room, diagnostics department and other ancillary
relevant to the patient’s stay in the hospital. Assist the physician with bedside
procedures such as; lumbar tap and CVP insertion.
Facilitate diagnostic requests of the Physician such as laboratories, radiology and
imaging, cardiac and pulmonary function.
Operate equipment used in critical care: Intravenous Fluid infusion pumps, Syringe
pumps,Cardiac monitors, Nasogastric tube feeding pumps, Accuvein, defibrillator,
temperature probes, arterial line, pulmonary artery catheter, swan ganz, Doppler,
ventilator, suction machine (open or closed suction), capillary glucose monitoring
machine.
Accurately administer medications with knowledge of its action, indication, side
effects, desired administration time, and nursing responsibilities. Correctly calculate
doses and maintain an updated knowledge of the basic pharmacokinetics of the drug.
Follow the standard for infection control; hand washing and use of personal protective
equipments such as mask, gown, gloves, eyewear, head cover as appropriate. Proper
disposal of contaminated items and use of isolation is observed. Consistently abide
with waste management and proper segregation.
Assess the patient’s and the family’s learning needs, Educate correct and required
information accordingly. Treat patients and their families with respect and dignity.
Adhere to the visiting and non-smoking policies.
Maintain concise, accurate and continued nursing documentation.
Participate in maintenance of the unit’s cleanliness, orderliness and organization.
Check stocks and supplies every shift, ensure emergency cart medications and
supplies are viable and not to expire within the next three months.
Complete exact patient charges daily.
Enter orders in the computer. Request of medications, supplies and diagnostics.
Participate in measures for quality improvement.
Updated with hospital policies, heads, organizational chart, new forms and protocols,
religiously attends lectures on updates with the hospital’s system and protocols.
Enhance professional growth by attending educational opportunities pertaining to ICU
nursing.
Direct and coordinate infection control programs, advising and consulting with
specified personnel about necessary precautions.
Perform administrative and managerial functions, such as taking responsibility for a
unit's staff, budget, planning, and long-range goals.
Initiate, coordinate and participate in Code Blue. Attend to patients requiring
cardiopulmonary resuscitation in a swift and efficient manner.
When the code is in the wards, the ICU nurse brings the tackle box and assists with
administering emergency medications.
When the Code is in the ICU, the ICU nurse assists the physician with intubation,
administers emergency medications and does cardiopulmonary resuscitation
Behave in a professional manner and maintain good interpersonal relationships with
other staff, departments, heads, and physicians.
Participate in the orientation of new ICU staff. Provide nursing students with
opportunities to observe and learn.

Job description:
Performs assessment and classification of wound and stoma of patients admitted to
the intensive care unit.
Coordinates with institutions wound care nurse for evaluation, referral and proper
wound care and treatments.
Collaborates with the attending physician regarding wound care, infection control,
nutrition and use of special mattresses.
Performs wound care and apply appropriate dressing materials such as hydrocolloids,
alginates/cavity fillers, foams, impregnated gauze, gels autolytic debridement,
transparent dressing, powders, ointments and the like.
Provides proper education to nursing personnel, patient, and family for prevention and
treatment of pressure ulcers such as bladder and bowel care, proper positioning/
turning and nutrition.
Accomplishes proper documentation of wound assessment, treatment and nursing
care plans and proper implementation pressure ulcer protocol.
Cases handled:
A. Cardiovascular
1. Acute myocardial infarctions with complications
2. Cardiogenic shock
3. Complex arrhythmia requiring close monitoring and interventions.
4. Acute congestive heart failure with respiratory failure and/or
hemodynamic support.
5. Hypertensive emergencies.
6. Unstable anginas, particularly with dysrhythmias, hemodynamic instability
or persistent chest pain.
7. Status post cardiac arrest
8. Cardiac Tamponade or constriction with hemodynamic instability.
9. Dissecting aortic aneurysms.
10.Complete Heart Block.
B. Pulmonary System
1. Acute respiratory failure requiring ventilatory support
2. Pulmonary emboli with hemodynamic instability
3. Patients in an intermediate care unit who are demonstrating respiratory
deterioration
4. Need for nursing/respiratory care not available in lesser care areas such as
floor or intermediate care unit.
5. Massive hemoptysis
6. Respiratory failure with imminent intubation
C. Neurologic Disorders
1. Acute stroke with altered mental status
2. Coma: metabolic, toxic, or anoxic
3. Intracranial hemorrhage with potential for herniation
4. Acute subarachnoid hemorrhage
5. Meningitis with altered mental status or respiratory compromise
6. Central nervous system or neuromuscular disorders with deteriorating
neurologic or pulmonary function
7. Status epilepticus
8. Brain dead or potentially brain dead patients who are being aggressively
managed while determining organ donation status
9. Vasospasm.
10. Severe head injured patients
D. Drug Ingestion and Drug Overdose
1. Hemodynamically unstable drug ingestion
2. Drug ingestion with significantly altered mental status with inadequate
airway protection
3. Seizures following drug ingestion
E. Gastrointestinal Disorders
1. Life threatening gastrointestinal bleeding including hypotension, angina,
continued bleeding, or with comorbid conditions.
2. Fulminant hepatic failure.
3. Severe pancreatitis.
4. Esophageal perforation with or without mediastinitis.
F. Endocrine
1. Diabetic ketoacidosis complicated by hemodynamic instability, altered
mental status, respiratory insufficiency, or severe acidosis
2. Thyroid storm or myxedema coma with hemodynamic instability
3. Hyperosmolar state with coma and/or hemodynamic instability
4. Other endocrine problems such as adrenal crises with hemodynamic
instability.
5. Severe hypercalcemia with altered mental status, requiring hemodynamic
monitoring.
6. Hypo or hypernatremia with seizures, altered mental status
7. Hypo or hypermagnesemia with hemodynamic compromise or
dysrhythmias.
8. Hypo or hyperkalemia with dysrhythmias or muscular weakness
9. Hypophosphatemia with muscular weakness
G. Surgical
1. Post-operative patients requiring hemodynamic monitoring/ventilatory
support or extensive nursing care.
2. Status Post Coronary arterial Bypass Graft patients.
3. Status Post angioplasty patients.
4. Post-Operative atrial and mitral valve repair or replacement patients.
5. Patients who undergone implantable cardioverter defibrillator and
temporary or permanent pacemaker insertion.
6. Patients who undergone endovascular coiling.

1. Septic shock with hemodynamic instability

Employment History Staff Nurse Intensive Care Unit-MICU,SICU,CCU The Medical City Iloilo July1 2010 - September 5 2015 JOB DESCRIPTION Complete a head to toe assessment and a social assessment of the patient upon admission. Develop a written nursing care plan upon admission, indicating the independent and collaborative nursing functions. This is reviewed and updated daily or when a new nursing problem arises. Initiate cardiac monitoring, oxygen therapy, non-invasive blood pressure and oximetry monitoring. Start and evaluate intravenous access. Execute insertion of nasogastric tube and urinary catheter. Does assessment and continuous monitoring of drainage systems in place such as: chest tube, Jackson pratt, and nasogastric drainage. Perform blood withdrawal from an arterial line, obtain readings from a pulmonary catheter. Recognize electrocardiogram readings, correlates with clinical symptoms presented. Intervene with physician orders based on ACLS Algorithm. Continuous re assessment of patient to determine progress and trends in the health status that require referral to the physician. Provide routine patient care. Close monitoring of vital signs, precise regulation of fluids administered, careful repositioning of patients with contraptions, ensure bedside cleanliness at all times. Performs suctioning, accurate recording of intake and output, renders daily bed bath, hair shampoo as ordered, change of linens and regular oral care. Anticipate the needs by efficient communication. Carry out Physician’s order in a systematic manner, collaborate with other departments such as the Operating Room, diagnostics department and other ancillary relevant to the patient’s stay in the hospital. Assist the physician with bedside procedures such as; lumbar tap and CVP insertion. Facilitate diagnostic requests of the Physician such as laboratories, radiology and imaging, cardiac and pulmonary function. Operate equipment used in critical care: Intravenous Fluid infusion pumps, Syringe pumps,Cardiac monitors, Nasogastric tube feeding pumps, Accuvein, defibrillator, temperature probes, arterial line, pulmonary artery catheter, swan ganz, Doppler, ventilator, suction machine (open or closed suction), capillary glucose monitoring machine.

Correctly calculate doses and maintain an updated knowledge of the basic pharmacokinetics of the drug. Proper disposal of contaminated items and use of isolation is observed. Consistently abide with waste management and proper segregation. Updated with hospital policies, heads, organizational chart, new forms and protocols, religiously attends lectures on updates with the hospital’s system and protocols. Enhance professional growth by attending educational opportunities pertaining to ICU nursing. Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions. Perform administrative and managerial functions, such as taking responsibility for a unit's staff, budget, planning, and long-range goals. Initiate, coordinate and participate in Code Blue. Attend to patients requiring cardiopulmonary resuscitation in a swift and efficient manner. When the code is in the wards, the ICU nurse brings the tackle box and assists with administering emergency medications.
