This document contains premium charts for my:health Medisure Super Top Up Insurance. It lists premium rates for various coverage amounts and deductibles based on the number of adults and children covered. Premiums increase with higher coverage amounts and lower deductibles. Rates also vary by the age of the policyholder, with older policyholders having higher premiums. The premium rates include a 14% service tax.
