INTRODUCTION
Extended Family, Inc., a community-based, not-for-profit (NFP) organization, was founded in 1995 by a group of parents who were concerned about the stigma surrounding children with disabilities. Specifically, these parents found that such children did not have a place to go to receive specialized counseling aimed at helping them cope with their disabilities. The founding parents also wanted their adult children with disabilities to have an independent, yet supervised, living situation. Originally a grass roots organization with limited support, Extended Family was incorporated in 1999 in order to expand the program from advocating for services to providing services. Extended Family's mission is to educate the community on disabilities and to provide a safe living environment that will enable people with disabilities to live with dignity.

Extended Family operates three separate programs: counseling, education, and residential. Clients may be referred to one of these programs by their schools, social service agencies, physicians, or their parents.

2. The New Jersey Division of Developmental Disabilities
(DDD), the state agency responsible for providing and
funding services for adults with developmental disabilities, is
undergoing changes to its service delivery system. These
changes are based on a fiscal shift away from a contracted
services reimbursement model to a Fee for Service (FFS)
model; and an ideological shift away from segregated,
institutionally based care to a system that promotes
integration through community-based service and support
options. Implementation of these changes is expected to
occur over the next 12-18 months.
For families who are new to DDD and adult services for
people with Intellectual and Developmental Disabilities,
we’ve compiled a list of 5 important things for you to know
about the upcoming changes as per the current information
shared by DDD:
The Supports Program will become the primary
channel for accessing services
Support Coordination will be the key community
resource for accessing services
The Individualized Service Plan (ISP) will be the
primary Support Coordination document
DDD will approve and authorize funding and
provide oversight
Participants will receive an“up to”budget for
services based on assessed level of need
Each Supports Program participant will have a budget based
on assessed level of need that is made up of three components:
(1) Employment/Day Services, (2) Individual/Family Supports,
and (3) Administrative (consisting of Support Coordination,
and the use of a Fiscal Intermediary if needed).
Access to a Person Centered Team Approach, Support
Coordination honors the hopes and dreams of individuals
and their families. Support Coordinators facilitate a
customized and individualized planning process that
enables people and their families to make informed choices.
Support Coordination services ensure participants have
access to needed programs and State plan services. Support
Coordination is managed by one Support Coordinator for
each participant. The Support coordinator is responsible for
developing and maintaining the Individualized Service
Plan (ISP) with the participant, their family and other team
members designated by the participant. The Support
Coordinator is responsible for the ongoing monitoring of
the provision of services included the Individualized Service Plan.
Our agency is located in the financial District of Newark,
New Jersey. Currently, our service area includes the
following counties within New Jersey: Essex, Bergen, Morris,
Hudson, Union, Passaic, and Middlesex.
Our agency has been approved by the Department of
Human Services, Division of Developmental Disabilities
(DDD) as a Support Coordination Agency.
