480V Switchgear & MCC deficiencies found
during inspections
• Loose PT connections
• Loose CT connection
• Dirty
• Panels and covers not affixed properly (bolts without nuts held in with tape
• Ground ball covers missing
• Stainless wire for tag securement and other foreign material left in sections (hard hats, gloves tools, metal bars, etc.)
• Deformed/damaged structure
• Modified structure
• Missing components
• Non Manufacturer hard ware
• Damaged cables
• Damaged breaker and breaker cubicle components
• Conduit entry into arc flash doors
• Wiring ferruls too small for the wire and strands that did not fit were “trimmed” off.
• Bus boots and covers not secured or covering bus bolts or splices
• Equipment numbers missing or on wrong devices
• Ferrous Gland Plate on MCC incoming sections.
• Incorrect cables on all Battery systems, flexible cable is required by spec
• LCS tags need to follow specific EMETL methods and include equipment tag number (not LCS P&ID number), Description,
and Source (MCC and cubicle), if more than one substation in facility then building number shall be included as well. S7C-
0702-06, 07, &08
• SEOLDs not created
• MCC cubicles not identified with cubicle numbers.
• Missing communication cable between SEL relays on MTM

wire strands were given a “haircut”
Foreign objects found in gear (here is wire that held on information tags left
on a ground ball that will be energized). Hard hats, gloves, metal rods, info
tags, tools etc. were found in the gear during our inspections.

Another example of CT layout also be sure that the
bus boots & covers are all in place and secured.
