Job Summary:
The Direct Support position provides support for adolescents and adults with
developmental disabilities, assisting them in all aspects of daily life in a group home
setting, including recreational activities.
Essential Duties:
Assist consumer in development of community living skills
Assist consumer in preparation of meals
Assist consumer in maintaining household
Assist consumer with toileting, dressing, grooming and personal maintenance
Assist in assuring opportunities for consumer(s) within the home to participate in
their own lives and to become part of the community to the fullest extent possible.
This may include, but is not limited to:
o Plan and participate in recreational activities of the residents own choosing
o Identify and pursue outside interests, hobbies, involvements and events
o Develop, foster and maintain relationships with family and friends
o Foster independence and maximize involvement with regard to all daily
living skills through goal-directed, creative, imaginative and consumer
efforts
o Promote and protect self-worth, respect and dignity
Provide supportive assistance in all aspects of residential direct care including but
not limited to:
o Administer medication, consistently and timely, following the medication
administration procedures
o Grooming
o Hygiene and toileting
o Meals (following menus and all nutritional and/or dietary guidelines)
o Cleaning and laundry
o Mobility
o Exercise
o Budgeting/spending
o Individual therapeutic needs, as necessary
o All individual health and safety needs
Provide support and maintain continued involvement with consumer especially
with regard to:
o Health, welfare and happiness
o Relationships
o Interests, capacities and talents
o Social skills and activities
o Goals, dreams and aspirations
o Preferences and choices
o Respect and dignity
Transport consumer(s), as required

Attends all Medical/Dental appointments with consumer
Maintain ongoing communication with consumer, other treatment providers and
other program staff
Perform record keeping as required by CPS/DES/DDD/DHS and ESBF to include
mandatory reporting of incident report (IR) and Serious incident report (SIR)
Maintain familiarity with consumer’s profile and medical history, Individual
Support Plan (ISP) and Behavior Treatment Plan (BTP) when applicable,
including: running objectives, following skill plans and tracking data, knowing
long term goals, knowing what works and what doesn’t for each individual
consumer, results of all professional evaluations, team assessment summary and
all related support information for consumer.
Maintain confidentiality of consumer(s) and protected information at all times
Maintain professional and positive working relationship with consumer, other
support staff and management team
Any other duties as assigned
Essential Functions:
Adhere to and stay up-to-date on all Easter Seals Blake Foundation policies and
procedures
Accurately report and maintain all work time in online timesheet system
Maintain company assigned email account
Report to work timely and follow specified schedule as directed
Arrange alternate coverage for shift when you cannot work & communicate the
same with manager in advance
Complete and maintain any training or retraining as required
Education & Requirements:
High school diploma or equivalent
Valid Arizona Driver’s License with clean driving record
Must be 21 years or older
Must clear DES Central Registry
Must be able to obtain and maintain a fingerprint clearance card
Must be able to lift: Male: 75 pounds, Female: 50 pounds
Must be proficient in English (read, write & comprehend)
Successfully complete all training for position within established timelines

Static Position – Sitting for prolonged periods of time.Standing Position –Standing for
extended periods of time.
Stooping – Bending body downward and forward by bending spine at the waist.
Walking – Moving about on foot to accomplish tasks.
Pushing – Using upper extremities to press against something with stead force in order to
thrust forward, downward, outward, side-to-side such as a broom or mop. Pushing with
up to 25 lbs. force.
Pulling – Using upper extremities to exert force in order to draw, drag, tug, or move side-
to-side an object in a sustained motion such as a broom or mop. Pulling with up to 25 lbs.
force.
Reaching – Extending hand(s) and arm(s) in any direction.
Lifting –Ability to lift 75 pounds (50 lbs. for women) from a lower to a higher position or
moving horizontally from position to position.
Fingering - Picking, pinching, typing, writing, or otherwise working, primarily with
fingers rather than the whole hand or arm as in handling.
Grasping – Applying pressure to an object with the fingers and palm.
Talking – Expressing or exchanging ideas by means of spoken word. Those activities in
which they must convey detailed or important spoken instructions to other workers and/or
clients accurately, loudly or quickly.
Hearing – Perceiving the nature of sounds. Ability to receive detailed information
through oral communication and to make fine discriminations in sound.
Seeing – The ability to perceive the nature of objects by the eye. Seeing is important for
hazardous positions in which defective seeing would result in injury and positions in
which special and minute accuracy, inspecting and sorting exist. A high degree of visual
efficiency, placing intense and continuous demands on the eyes by machinery that is
moving and other objects are also considered important. Other important factors of
seeing are acuity (near and far), depth perception (three-dimensional vision),
accommodation (adjustment of lens of eye to bring an object into sharp focus), field of
vision (area that can be seen up and down or to the right or left while eyes are fixed on a
given point) and color vision (ability to identify and distinguish colors).
