This study compares sentinel lymph node biopsy to elective neck dissection in patients with early stage (T1/T2) oral squamous cell carcinoma without clinical or radiological evidence of neck lymph node metastasis (N0). The study uses a matched pair analysis to compare outcomes between the two neck staging procedures. Sentinel lymph node biopsy aims to stage the neck while avoiding unnecessary dissection in patients who do not have occult metastasis, whereas elective neck dissection removes lymph nodes prophylactically without knowing their status. The study found that sentinel lymph node biopsy identified occult metastasis in approximately 20-30% of N0 patients, suggesting it may help avoid elective neck dissection in the majority who are metastasis-free.
