3. How to use the 5 Why’s
1. Write down the specific problem.
2. Ask Why the problem happens and write the answer
down below the problem.
3. If the answer you just provided doesn’t identify the
root cause of the problem that you wrote down in
Step 1, ask Why again and write that answer down.
4. Loop back to step 3 until the team is in agreement
that the problem’s root cause is identified.
4. PDCA Model
The PDCA is a four-step model, which is to be used as a tool for
continuous improvement.
The PDCA is should be used when doing the following:
When starting a new improvement project.
When developing a new or improved designed of a process,
product or service.
When defining a repetitive work process.
When planning data collection and analysis in order to verify and
prioritize problems or causes.
When implementing any change.
5. PDCA Model
Plan: When you use plan, you are actually analyzing your
company, organization, and work center’s current process.
Do: We need to generate possible solutions and implement
the solution to a small group of personnel.
Check: The check is used to measure success of the
implemented improvement plan. The check is done more
than once.
Act: The act will be completed when the improvement has
become the standard.
