It requests the applicant's name, institution, current designation, email address, and preferences for one or two officer positions including President, Vice President of Finance & Operations, Events & Education, or External Relations divisions, or Assistant Vice President. Guidelines for selecting officer position preferences:
 If you are applying for a Vice President or Assistant Vice President position, indicate which
division (Finance & Operations, Events & Education, External Relations).
 If you are only interested in one specific division, you may apply for the Vice President
position of that division and its Assistant Vice President position as a backup.
 If your interests are not confined to a specific division, you may apply to multiple divisions.
 If you are most interested in the President position, select Vice President of one division
as a backup.
