AFFIDAVIT OF SOLE CUSTODY
I, MA. LORELIE B. AGLABTIN, oflegal age, married, Filipino
citizen and a resident of No. 22, Beningno Aquino Avenue, Barangay
Quezon, Solano, Nueva Vizcaya, Philippines, after having been duly
sworn to in accordance with law hereby depose and state:
1. That I am the biological mother of the minor MICHLORE
GJERZHON B. AGLABTIN with my husband MICHAEL E.
AGLABTIN;
2. That since he left us, I never received any financial support from
my husband for our child.
3. That my husband MICHAEL E. AGLABTIN had abandoned me
and MICHLORE GJERZHON B. AGLABTIN the child
sometime in 2014 and I have not seen him anymore;
4. That since my former husband had left me, the minor
MICHLORE GJERZHON B. AGLABTIN have always been
in my sole custody;
5. That Iattest to the veracity ofthe foregoing statements and hereby
execute this affidavit for whatever legal intent or purpose it may
serve under the premises.
