Advocacy
WHO (1995) defines health advocacy as ‘a combination of
individual and social actions designed to gain political
commitment, policy support, social acceptance and systems
support for a particular health goal or programme’.
Advocacy can be described as efforts taken within the existing
system and biomedical model that generally use knowledge of
experts and aim to effect change through primarily education
(Zoller, 2005).

Activism
Unlike advocacy, activism often challenges the existing system,
power structures and models, and emphasises the fundamental
need for public participation in policy-making (Zoller, 2005).
Health activism aims to change the status quo (e.g. social
norms, embedded policies, dominance of certain actors), as
such, people often need to work outside their organisation and
in individual roles. Examples of activism include demonstrations,
public rallies, filing litigation, internet-based actions, and
lobbying as well as direct action.

Direct Action
Direct action is ‘essentially nonviolent
methods of noncooperation, obstruction or
defiance.’
This strategy is often employed where there
is a significant imbalance of power between
people and corporations / institutions /
governments which lead people to adopt
tactics such as non-cooperation (e.g. tax
refusal, strikes, boycotts), civil disobedience
or physical obstruction (Carter, 2005).

In an early module we also explored a number of opportunities (parliamentary levers) to engage with the parliamentary process and engage with allies in parliament to access information, highlight issues and influence legislative development.
We have also discussed conventional strategies such as legislative action and touched on electoral activities as mechanisms for influencing policy. Legislative action focuses on proposing laws and regulations that aim to achieve public health goals (e.g. taxes on tobacco and alcohol, standards for advertising, restrictions on access and use of products such as firearms, tobacco) using tools such as lobbying, education, media, demonstrations and rallies, and coalition building. Electoral activities relate to actions taken to voters such as endorsing candidates, campaigning for politicians, and ballot initiatives.
Public health advocacy can be described and categorised according to who or what is being advocated for or in relation to the function an advocate is performing. Carlisle (2000) defined four types of advocacy: Representation, Community development, Community activism and Social policy reform.
