6. Click on the Assessments tab
to Create a new ASSESSMENT

7. Click Create
New Assessment

8. Select Assessment Type
as SURVEY
Select Next to move
to next screen
No. of days the
assessment
should show

9. Select Question Type: based on type of
responses to be captured -
Rating: option to rate by selecting from 5 stars
Likert Scale: option to select from a scale of 1 to
5/10/20/30/100
Progress Bar: Shows progress on a scale of 1-100
Radio: Yes / No or True /False answers
Multi Select: option to select more than one
answer from multiple choices
Single Select: Single answer can be selected
from multiple choices
Comments: Text box can be entered in a text box
Date Picker: drops down a calendar to select
date from
Select Save Question at the bottom of the page
to move to next screen

to add more question
to the assessment

12. ROLES: Assign by roles Ex.
Admin, AGM etc.
SPECIALITY: to add Medical
Specialization of Doctors
Select PUBLISH to see
the entered assessment
in the iPad .
