3. INTRODUCTION
Evaluation is an integral part of Nursing process often referred to
as the final phase.
It’s an ongoing process.
Without evaluation, All the other steps cannot provide
information required to improve Nursing practice and hence
quality patient care.
Therefore it is very vital and essential in Nursing professions.
4. DEFINITION OF EVALUATION
• Rigorous analysis of completed or ongoing activities that
determine or support management accountability, effectiveness
and efficiency (Business Dictionary)
• A critical assessment in as objective a manner as possible, of
the degree to which a service or its component parts fulfills
stated goals (St legal and words worth –bell Wikipedia)
5. DEFINITION OF NURSING EVALUATION
Evaluation is a planned, ongoing, purposeful appraisal of
client’s progress towards the achievement of desired,
expected outcome or objectives and the effectives of
intervention and helps in the reassessment of care plan.
Conclusions drawn from this part will determine whether
Nursing intervention should be terminated, continued or
changed.
7. TYPE OF NURSING EVALUATION CONT.
Helps to make immediate modification in nursing care
where necessary.
Reassessment of the whole care plan in relation to the
stated expected outcome.
It also measures to what extent the goals/ objectives have
being achieved at the end of the stated period.
8. PROCESS OF EVALUATION
Compare what the client is able to do with what has been set as
objective then ask the following questions.
Has the objective been achieved completely (positive outcome)
Is it partially met
Objectives/goals not met at all (Negative)
9. PURPOSE OF EVALUATION
Evaluation helps to make judgments about your action,
activities and interventions.
It determines their effectiveness.
Area to improve upon.
It increases productivity.
10. PURPOSE OF EVALUATION CONT.
It helps design objectives that are
more measurable and achievable.
It helps in academic research and
discussions.
11. GOOD NURSING EVALUATION
Good evaluation sought input from the client,
unbiased, impartial and honest.
A good evaluation also identifies area of
limitation.
It is evidence based

Risk for infection related to:-
I. surgical incision.
II. Tissue damage
iii. Obesity. e.t.c.
Objective
Patient wound will healed by first intention.
Patient will not develop pressure wound
Patient wound will not get infected.
Evaluation :
Stitches removed after 7days, 14days, 5 days and suture site healed well.
Wound is granulating well /skin graft 80%, 95%.
13. EXAMPLE 2
Activity intolerance
Patient will report ability to perform activities of daily
living
Patient will walk length of the ward corridor and back
Evaluation
Patient report that he /she took her bath, combed her
hair and or washed her hair.
Patient walked the length of the ward corridor and
back.
She/he feel dizzy on standing, with increased pulse rate
and visible perspiration (negative outcome)
