HOW TO TACKLE ANTIBIOTIC
RESISTENCE
➢ Antimicrobial Resistance(AMR) is the ability of a
microorganism (like bacteria, viruses, and some
parasites) to stop an antimicrobial (such as
antibiotics, antivirals and antimalarials) from working
against it. As a result, standard treatments become
ineffective, infections persist and may spread to
others.
➢ Antibiotic resistance develops when bacteria adapt and grow in the presence
of antibiotics
➢ The development of resistance is linked to how often antibiotics are used.
➢ When microbes become resistant to medicines, the options for treating the
diseases are reduced.
➢ This resistance to antimicrobial medicines is happening in all parts of the
world for a broad range of microorganisms with an increasing prevalence
that threatens human and animal health.
➢ The direct consequences of infection with resistant microorganisms can
be severe, including longer illnesses, increased mortality, prolonged
stays in hospital, loss of protection for patients undergoing operations
and other medical procedures, and increased costs.
➢ Antimicrobial resistance affects all areas of health, involves many
sectors and has an impact on the whole of society
➢ The indirect impact of antimicrobial resistance, however, extends
beyond increased health risks and has many public health
consequences with wide implications, for instance on
development.
➢ Antimicrobial resistance is a drain on the global economy with
economic losses due to reduced productivity caused by sickness
(of both human beings and animals) and higher costs of treatment.
➢ Antimicrobial resistance, therefore, threatens the very core of modern
medicine and the sustainability of an effective, global public health
response to the enduring threat from infectious diseases.
➢ Effective antimicrobial drugs are prerequisites for both preventive and
curative measures, protecting patients from potentially fatal diseases
and ensuring that complex procedures, such as surgery and
chemotherapy, can be provided at low risk. Yet systematic misuse and
overuse of these drugs in human medicine and food production have
put every nation at risk.
➢ Few replacement products are in the pipeline. Without harmonized and
immediate action on a global scale, the world is heading towards a
post-antibiotic era in which common infections could once again kill.
The Way Forward..
➢ Despite proposals and initiatives over many years to combat antimicrobial resistance,
progress has been slow, in part because of inadequate monitoring and reporting at
national, regional and global levels, and inadequate recognition by all stakeholders of the
need for action in their respective areas.
➢ Therefore an action plan was required which had the following principles:
(1) Whole-of-society engagement including a one health approach.
(2) Prevention first.
(3) Access.
(4) Sustainability.
(5) Incremental targets for implementation.
World Health Assembly adopted a global action plan on antimicrobial
resistance, which outlines five objectives:
➢ to improve awareness and understanding of antimicrobial resistance through
effective communication, education and training
➢ to strengthen the knowledge and evidence base through surveillance and
research;
➢ to reduce the incidence of infection through effective sanitation, hygiene and
infection prevention measures
➢ to optimize the use of antimicrobial medicines in
human and animal health;
➢ to develop the economic case for sustainable
investment that takes account of the needs of all
countries and to increase investment in new
medicines, diagnostic tools, vaccines and other
interventions
“ Improve awareness and understanding of antimicrobial
resistance through effective communication, education and
training “
➢ Steps need to be taken immediately in order to raise awareness of
antimicrobial resistance and promote behavioural change, through
public communication programmes that target different audiences
in human health, animal health and agricultural practice as well as
consumers
➢ Making antimicrobial resistance a core component of
professional education, training, certification, continuing
education and development in the health and veterinary
sectors and agricultural practice will help to ensure proper
understanding and awareness among professionals
➢ Potential measures of effectiveness: extent of reduction in
global human consumption of antibiotics and reduction in
the volume of antibiotic use in food production
OBJECTIVE 2:
“Strengthen the knowledge and evidence base through
surveillance and research”
➢ Actions and investments to tackle antimicrobial resistance should be supported.
➢ National governments, intergovernmental organizations, agencies, professional
organizations, nongovernmental organizations, industry and academia have important roles
in generating such knowledge and translating it into practice.
➢ Potential measure of effectiveness: extent of reduction in the prevalence of antimicrobial
resistance, based on data collected through integrated programmes for surveillance of
antimicrobial resistance in all countries
“Reduce the incidence of infection through effective sanitation, hygiene and
infection prevention measures”
➢ Many of the most serious and difficult-to-treat antibiotic resistant infections occur in
health care facilities, not only because that is where patients with serious infections are
admitted but also because of the intensive use therein of antibiotics
➢ Better hygiene and infection prevention measures are essential to limit the
development and spread of antimicrobial-resistant infections and multidrug-resistant
bacteria
➢ Vaccination, where appropriate as an infection prevention measure, should be
encouraged
➢ Existing vaccines can prevent infectious diseases whose treatment
would require antimicrobial medicines
➢ Existing vaccines can reduce the prevalence of primary viral infections,
which are often inappropriately treated with antibiotics
➢ Potential measure of effectiveness: extent of reduction in the
prevalence of preventable infections, and in particular the incidence of
drug-resistant infections in health care settings
OBJECTIVE 4:
“ Optimize the use of antimicrobial medicines inhuman and
animal health “
➢ High antibiotic use may reflect over-prescription, easy access through
over-the-counter sales, and more recently sales via the Internet which
are widespread in many countries
➢ Despite measures taken by some Member States, antibiotic use in
humans, animals and agriculture is still increasing globally
➢ More widespread recognition of antimicrobial medicines as a public good is needed in
order to strengthen regulation of their distribution, quality and use, and encourage
investment in research and development.
➢ Decisions to prescribe antibiotics are rarely based on definitive diagnoses. Effective,
rapid, low-cost diagnostic tools are needed for guiding optimal use of antibiotics in
human and animal medicine, and such tools should be easily integrated into clinical,
pharmacy and veterinary practices.
➢ Potential measure of effectiveness: extent of reduction in global human consumption
of antibiotics, the consumption of antibiotics used in food production, and the use of
medical and veterinary antimicrobial agents for applications other than human and
animal health
“ Develop the economic case for sustainable investment that
takes account of the needs of all countries, and increase
investment in new medicines, diagnostic tools, vaccines and
other interventions “
➢ Economic impact assessments are needed on the health and
broader socioeconomic burden of antimicrobial resistance, and
should compare the cost of doing nothing against the cost and
benefit of action
➢ Investment in the development of new antimicrobial medicines, as well as in
diagnostic tools and vaccines, is needed urgently.
➢ Antibiotics must also be supplemented by affordable, point-of-care
diagnostic tools to inform health practitioners and veterinarians of the
susceptibility of the pathogens to available antibiotics.
➢ Potential measure of effectiveness: extent of increase in sustainable
investment in capacity to counter antimicrobial resistance for all countries,
including investment in development of new medicines, diagnostics and
other interventions
What can you do as patient?
➢ Finish your pills- take the entire prescription exactly as
directed. Be compliant to the doctors orders.
➢ Get vaccinated
➢ Be hygienic and prevent infection in the first place.
➢ Stay safe in the hospital- antibiotic resistant bacteria are
commonly found in hospitals. Make sure you keep a
hygienic profile. Make sure your caregiver is hygienic.
What can you do as a doctor?
1. Timely and appropriate initiation of antibiotics
▪ Promptly identify patients who require antibiotics
▪ Obtain culture before starting antibiotics
▪ Determine and verify antibiotic allergies and tailor
therapy accordingly
▪ Start treatment promptly
▪ Specify expected duration of therapy based on
evidence and national guidelines
2. Appropriate administration and De-escalation
▪ Give antibiotics at the right dose and interval.
▪ Stop or de-escalate therapy promptly based on the culture and
sensitivity tests
▪ Monitor for toxicity reliably and adjust agent and dose promptly
3. Expertise at the point of care
▪ Develop and make available expertise in antibiotic use
▪ Cultivate local expertise among staff
▪ Develop a process for antibiotic formulary management.
4. Avoid unnecessary and risky procedures- if done aseptic precautions
must be take.
5. Doctors must comply with strict guidelines when prescribing
antibiotics.
6. Staff in healthcare institutions must comply with strict rules on hygiene
so that resistant bacteria cannot be transmitted from one patient to
another.
7. Promptly identify patients infected with multi-drug resistant organisms
and isolate them to prevent further infection.

How to tackle antibiotic resistence

  • 1. HOW TO TACKLE ANTIBIOTIC RESISTENCE
  • 2. ➢ Antimicrobial Resistance(AMR) is the ability of a microorganism (like bacteria, viruses, and some parasites) to stop an antimicrobial (such as antibiotics, antivirals and antimalarials) from working against it. As a result, standard treatments become ineffective, infections persist and may spread to others.
  • 3. ➢ Antibiotic resistance develops when bacteria adapt and grow in the presence of antibiotics ➢ The development of resistance is linked to how often antibiotics are used. ➢ When microbes become resistant to medicines, the options for treating the diseases are reduced. ➢ This resistance to antimicrobial medicines is happening in all parts of the world for a broad range of microorganisms with an increasing prevalence that threatens human and animal health.
  • 4. ➢ The direct consequences of infection with resistant microorganisms can be severe, including longer illnesses, increased mortality, prolonged stays in hospital, loss of protection for patients undergoing operations and other medical procedures, and increased costs. ➢ Antimicrobial resistance affects all areas of health, involves many sectors and has an impact on the whole of society
  • 5. ➢ The indirect impact of antimicrobial resistance, however, extends beyond increased health risks and has many public health consequences with wide implications, for instance on development. ➢ Antimicrobial resistance is a drain on the global economy with economic losses due to reduced productivity caused by sickness (of both human beings and animals) and higher costs of treatment.
  • 6. ➢ Antimicrobial resistance, therefore, threatens the very core of modern medicine and the sustainability of an effective, global public health response to the enduring threat from infectious diseases. ➢ Effective antimicrobial drugs are prerequisites for both preventive and curative measures, protecting patients from potentially fatal diseases and ensuring that complex procedures, such as surgery and chemotherapy, can be provided at low risk. Yet systematic misuse and overuse of these drugs in human medicine and food production have put every nation at risk. ➢ Few replacement products are in the pipeline. Without harmonized and immediate action on a global scale, the world is heading towards a post-antibiotic era in which common infections could once again kill.

The Way Forward.. ➢ Despite proposals and initiatives over many years to combat antimicrobial resistance, progress has been slow, in part because of inadequate monitoring and reporting at national, regional and global levels, and inadequate recognition by all stakeholders of the need for action in their respective areas. ➢ Therefore an action plan was required which had the following principles: (1) Whole-of-society engagement including a one health approach. (2) Prevention first. (3) Access. (4) Sustainability. (5) Incremental targets for implementation.

World Health Assembly adopted a global action plan on antimicrobial resistance, which outlines five objectives: ➢ to improve awareness and understanding of antimicrobial resistance through effective communication, education and training ➢ to strengthen the knowledge and evidence base through surveillance and research; ➢ to reduce the incidence of infection through effective sanitation, hygiene and infection prevention measures
  • 10. ➢ to optimize the use of antimicrobial medicines in human and animal health; ➢ to develop the economic case for sustainable investment that takes account of the needs of all countries and to increase investment in new medicines, diagnostic tools, vaccines and other interventions
  • 11. “ Improve awareness and understanding of antimicrobial resistance through effective communication, education and training “ ➢ Steps need to be taken immediately in order to raise awareness of antimicrobial resistance and promote behavioural change, through public communication programmes that target different audiences in human health, animal health and agricultural practice as well as consumers
  • 12. ➢ Making antimicrobial resistance a core component of professional education, training, certification, continuing education and development in the health and veterinary sectors and agricultural practice will help to ensure proper understanding and awareness among professionals ➢ Potential measures of effectiveness: extent of reduction in global human consumption of antibiotics and reduction in the volume of antibiotic use in food production

OBJECTIVE 2: “Strengthen the knowledge and evidence base through surveillance and research” ➢ Actions and investments to tackle antimicrobial resistance should be supported. ➢ National governments, intergovernmental organizations, agencies, professional organizations, nongovernmental organizations, industry and academia have important roles in generating such knowledge and translating it into practice. ➢ Potential measure of effectiveness: extent of reduction in the prevalence of antimicrobial resistance, based on data collected through integrated programmes for surveillance of antimicrobial resistance in all countries
  • 14. “Reduce the incidence of infection through effective sanitation, hygiene and infection prevention measures” ➢ Many of the most serious and difficult-to-treat antibiotic resistant infections occur in health care facilities, not only because that is where patients with serious infections are admitted but also because of the intensive use therein of antibiotics ➢ Better hygiene and infection prevention measures are essential to limit the development and spread of antimicrobial-resistant infections and multidrug-resistant bacteria ➢ Vaccination, where appropriate as an infection prevention measure, should be encouraged
  • 15. ➢ Existing vaccines can prevent infectious diseases whose treatment would require antimicrobial medicines ➢ Existing vaccines can reduce the prevalence of primary viral infections, which are often inappropriately treated with antibiotics ➢ Potential measure of effectiveness: extent of reduction in the prevalence of preventable infections, and in particular the incidence of drug-resistant infections in health care settings

OBJECTIVE 4: “ Optimize the use of antimicrobial medicines inhuman and animal health “ ➢ High antibiotic use may reflect over-prescription, easy access through over-the-counter sales, and more recently sales via the Internet which are widespread in many countries ➢ Despite measures taken by some Member States, antibiotic use in humans, animals and agriculture is still increasing globally
  • 17. ➢ More widespread recognition of antimicrobial medicines as a public good is needed in order to strengthen regulation of their distribution, quality and use, and encourage investment in research and development. ➢ Decisions to prescribe antibiotics are rarely based on definitive diagnoses. Effective, rapid, low-cost diagnostic tools are needed for guiding optimal use of antibiotics in human and animal medicine, and such tools should be easily integrated into clinical, pharmacy and veterinary practices. ➢ Potential measure of effectiveness: extent of reduction in global human consumption of antibiotics, the consumption of antibiotics used in food production, and the use of medical and veterinary antimicrobial agents for applications other than human and animal health
  • 18. “ Develop the economic case for sustainable investment that takes account of the needs of all countries, and increase investment in new medicines, diagnostic tools, vaccines and other interventions “ ➢ Economic impact assessments are needed on the health and broader socioeconomic burden of antimicrobial resistance, and should compare the cost of doing nothing against the cost and benefit of action
  • 19. ➢ Investment in the development of new antimicrobial medicines, as well as in diagnostic tools and vaccines, is needed urgently. ➢ Antibiotics must also be supplemented by affordable, point-of-care diagnostic tools to inform health practitioners and veterinarians of the susceptibility of the pathogens to available antibiotics. ➢ Potential measure of effectiveness: extent of increase in sustainable investment in capacity to counter antimicrobial resistance for all countries, including investment in development of new medicines, diagnostics and other interventions
  • 20. What can you do as patient? ➢ Finish your pills- take the entire prescription exactly as directed. Be compliant to the doctors orders. ➢ Get vaccinated ➢ Be hygienic and prevent infection in the first place. ➢ Stay safe in the hospital- antibiotic resistant bacteria are commonly found in hospitals. Make sure you keep a hygienic profile. Make sure your caregiver is hygienic.
  • 21. What can you do as a doctor? 1. Timely and appropriate initiation of antibiotics ▪ Promptly identify patients who require antibiotics ▪ Obtain culture before starting antibiotics ▪ Determine and verify antibiotic allergies and tailor therapy accordingly ▪ Start treatment promptly ▪ Specify expected duration of therapy based on evidence and national guidelines

Appropriate administration and De-escalation ▪ Give antibiotics at the right dose and interval. ▪ Stop or de-escalate therapy promptly based on the culture and sensitivity tests ▪ Monitor for toxicity reliably and adjust agent and dose promptly 3. Expertise at the point of care ▪ Develop and make available expertise in antibiotic use ▪ Cultivate local expertise among staff ▪ Develop a process for antibiotic formulary management.
  • 23. 4. Avoid unnecessary and risky procedures- if done aseptic precautions must be take. 5. Doctors must comply with strict guidelines when prescribing antibiotics. 6. Staff in healthcare institutions must comply with strict rules on hygiene so that resistant bacteria cannot be transmitted from one patient to another. 7. Promptly identify patients infected with multi-drug resistant organisms and isolate them to prevent further infection.
