1. LM3S ā Medical Summary
Scope of Summary
This document will be used as the basis for the Provisional Patent application. It
contains overview summaries for each component of the LM3S. A further granulized
and detailed document will be submitted prior to the Non-Provisional Patent application
process.
Overview of the Invention
The Innovatio Group has created a process for hospitals and other medical
professionals to monitor, track, and report throughout the total life-cycle of the patient.
By combining existing technology with proprietary software code, collectively called the
LM3S, it is now possible for all parties of interest to become intimately involved with the
managed care of the patient as well as providing business reporting functionality that
increases overall effectiveness of the education of medical campaigns.
Elements of Invention
The foundations of the process involve the combination of existing technologies and
proprietary software. Below is a list of these attributes followed by a brief overview of
each.

1. Patient Acquisition and Data Gathering
Our process starts with a marketing campaign that raises the awareness of a
particular medical condition, disease, or preventative measure and provides a call to
action to either the hospital or their medical provider. This method involves
traditional marketing techniques such as: Direct mail, outside media, radio and TV
spots, websites, seminars, and health fairs.
The call to action involves the patient calling either a local or toll-free telephone
number where their name, address, and other pertinent information is obtained.
Each marketing initiative is coded with an information tracking device; therefore, as
each patient responds from their respective marketing medium, it can be individually
traced to its original source location, such as: which specific billboard, time of radio
ad, and search keywords to the website.
Patients can also be added to the system through an internal process by identifying
patients that qualify by certain medical criteria, such as an abnormal
Echocardiogram reading.
2. 2. Microsoft Customer Relationship Management System
The data obtained from the Patient Acquisition process is uploaded and merged into
the Microsoft Customer Relationship Management system (CRM). This software
package stores all of the patientās non-medical information, such as name, physical
and email address, contact phone number, etc. It also manages the data collected
from every marketing initiative as well as produces reports on the success rates of
the campaigns.
3. Microsoft HealthVault
HealthVault is a Personal Health Records (PHR) system developed by Microsoft. It
allows the patient to not only house their medical data electronically, but also utilize
the hundreds of companies connected to HealthVault, such as pharmacies, medical
devices, and lab reporting companies. By the use of Microsoftās HealthVault
platform, we capture snapshots of the patientās medical data from the hospitals and
their other medical providers. All of the patientās relevant medical data is stored
securely by Microsoft servers.
4. Microsoft Lync
Microsoft Lync is a unified communications software system. Similar to Skype and
GotoMeeting, Microsoft Lync has the ability to conduct āface-to-faceā video
conferencing and computer screen sharing. Hospital surgeons, physicians, and
patients can utilize the video conferencing feature to conduct virtual medical video
telephony with image and screen sharing functions. These videos are also archived
for viewing by non-attendees or to review for further diagnosis.
5. Personal Health Record
By layering our proprietary software code on top of Microsoft HealthVault, we create
for the patient a Personal Health Record (PHR) to visualize, add, and amend their
personal medical information. The patient can also share their data with their
medical providers, caregivers, and other family members.
6. HTML5 Website
The application utilizes portals (Administrator, Physician, and Patient [described in
further detail below]). These portals are accessed by user ID and Password on a
website written in HTML5 code. This will allow mobile users to have similar
experiences in relationship to desktop users.
Proprietary Software Solution
With the exception of the Personal Health Record software that is layered on top of
HealthVault, all of the platforms and programs described above can run independently
but not interact with each other. In order for the solution to be novel, they all have to
become one end-to-end process with information being gathered and transmitted
to and from each platform and disseminated out to the correct party.
3. This is accomplished by our Proprietary Software Solution, called The Linkage
Marketing Strategy Software System (LM3S). At its core, the LM3S combines the CRM,
Video Conferencing, and PHR modules into one unified entity. For the first time, a
patient can be tracked from patient acquisition through the medical procedure and
retention for future medical campaigns. Information is transferred seamlessly to the
patientās physicians, surgeons, hospital administrators and marketing staff. This
ensures that the patient is provided the most informed medical team in and out of the
hospital.
The software code, which runs the executable files to gather the information from the
marketing campaigns, CRM and PHR modules, along with the video conferencing
software, is stored on our own dedicated servers. Without this code, the LM3S would
not exist and neither would the seamless transfer of information between the entities.
Working Example from Combining Technologies
The Innovatio Group has developed an end-to-end solution for hospitals to acquire,
educate, manage, track, and report on patient (customer) activity. For this working
example, we will demonstrate how the process works for a hospital using our product
for their Heart Valve Initiative (HVI) program.
Patients are engaged in the HVI via external marketing campaign (billboards, radio, TV,
print, web, etc.). Each marketing initiative is assigned separate local or toll-free
telephone numbers where the LM3S manages the acceptance rate of each marketing
tool. Patients can also be added to the HVI through physician review of an existing
echocardiograph (ECHO). The hospital administrator will tag the ECHO and upload the
patientsā medical information into the LM3S.
A patientās medical information is made available through the Personal Health Records
(PHR) tool that is linked to Microsoft HealthVault. By signing up for HealthVault, a
patientās medical information is uploaded to the LM3S and is now made available to
those participating in the HVI including the patientās primary care physician and
cardiologist.
Patients designated to have ECHOās that need further review are then put into
āConferenceā. Conference is the direct collaboration of physician, surgeon and hospital
administrator. These conferences take place on-line via the LM3S system. Physicians
managing a particular patient are given access to engage the āconferenceā via e-mail
submission to register with a user name and password from the participating hospital
website. Once logged, the physicianās involved with a patient up for a āconferenceā event
can now review in real time a conference event. Similar to the physician, hospital
administrators can also engage a āconferenceā event and subsequent patient
information once they log in via the hospital website with their designated username and
password.
A āconferenceā event is a āliveā streaming video conference where all parties involved in
the health management of the patient engage on-line. Patient information is made
4. available to review by all parties, medical images (echocardiographs, x-rays) can be
reviewed, notes from conference participants can be uploaded and e-mails can be sent
relating to conference outcome. After the event, an archived version is made available
on the LM3S for participants not available for the live event and also for review for
further diagnosis.
The journey the patient takes through the HVI is constantly tracked by hospital
administrators so that this information can be immediately disseminated to all parties
involved with the health management of the patient. This feature is available to the
hospital administrators by the LM3Sā Administrator Dashboard. Other benefits
include:

patientās health.
ļ· Master scheduling of therapeutic events and live Conferences for the patient
ļ· Reporting functionality to assess the success rates of the initial marketing
campaigns, show the workflow of patients throughout the HVI process, identify
which outside physicians are the most engaged with their patients in the HVI, and
financial reports such as Return on Investment for each marketing initiative.

The LM3S also includes a Physician Portal that is accessed by the patientās
physicians. This tool allows the physician outside of the hospital to become intimately
involved with the progress of their patient through the HVI process at the hospital. It also
includes features such as:

patientās health.
ļ· Patient File ā where people can access a patientās medical files

The LM3S completes the circle of health management with a Patient Portal that houses
the medical information of the patient. By utilizing HealthVault, the patient has the ability
to access current medical records imported by their physician or through HVI. Tools are
available for patients to upload and track medical progress, follow physicians medical
6. orders, track weight, blood pressure, abbreviated notes from a HVI conference, and
other pertinent medical information as described earlier via the PHR program that is
linked to Health Vault.
Additionally, patients will receive through their personal portal (web page) marketing
messages about programs, events, medical trails, medical news and other specific to
their medical condition. They will also have the ability to utilize the live video
conferencing feature to conduct telemedicine āoffice callsā with their primary care
physician or cardiologist.
