Disorders of consciousness
• Consciousness = Alert, aware of time,
place, person
SEMICONSCIOUS
Forget an event or a person for a
while, later remembers the
situation
LETHARGY
Aware of stimuli, dull senses ,
slow responses
SOMNOLENCE
Rarely awake, hardly aware of
environmental stimuli, slurred
and muffled speech
STUPOR
Occasionally aware of
environmental stimuli needed to
bring awareness
COMA
Unconscious, variable response
to stimuli, incontinence of
faeces & urine
DISORDERS OF MOTOR ACTIVITY
PSYCHOMOTOR AGITATION OR OVER
ACTIVITY
Purposeful activity but objective
may not be reached. e.g. mania
PSYCHOMOTOR RETARDATION
Slowing down of initiation or
duration of activity
STEREO TYPE
Regular repeated body
movements without any
significance
NEGATIVISM
Refusing in obeying commands &
doing exactly opposite what has been
requested
ECHOPRAXIA
Imitation of another
person’s action or gestures
ECHOLALIA
Meaningless repetition of
others person’s words
POSTURING
Adoption of unusual bodily
position continuously for a
long time
COMPULSION
An irrational repetitive impulse to
perform an activity in a stereotype
manner
MANNERISM
Repeated movement of
functional significance
TICS
Irregular repeated body
movements of a particular group
of muscles
WAXY FLEXIBILITY
Clients limbs can be placed in
the same position for a longer
time, muscle tone will be
maintained in the same manner
CATAPLEXY
Waxy and semi rigid of muscles
consistently immobility of
muscles is observed
AUTOMATISM
Client is unable to judge whether it is
right or wrong ; cooperative and follow
suggestion compulsively and
automatically
VIOLENCE
Engaged in anti social
behaviour , exercising physical
force against law
RESTLESSNESS
Uneasiness, not able to take
rest
AGITATION
Consistently in motion,
continuous activity
EXCITEMENT
The action of the individual
resulting from stimulation
APHASIA
Inability to speak due to damage of
speech center( Broca’s area) in the
brain
MUTISM
Not talking at all
APHONIA
Inability to produce sound
because of paralysis of vocal cord
INCOHERENCE
No logical connection, thought
progress from one idea to another
PERSEVERATION
Clinging to a particular thought
persistent repetition in the
expression of an idea
CIRCUMSTANTIALITY
Finally reaches the answer but
only after explaining many
unnecessary details
BLOCKING
Patients thought and speech
proceed at an average rate but are
interrupted
VERBIGERATION
Repeating the same word,
phrases or sentence in which no
coherent thought is expressed
Tangentiality
Disorder of thought , the client
response to a question with a series
of related topics ,but will never really
answer the question
NEOLOGISM
Coining of new words or
phrases invented by himself to
describe his morbid
experiences
WORD SALAD
Jumbled words
ILLUSION
Misperception of an external stimuli
E.g. seeing a rope in a dark room
and misperceive it as a snake.
HALLUCINATION
False sensory perception without any
external stimuli (auditory , visual ,
olfactory , gustatory , tactile
hallucination)
Auditory hallucination
• Person will be able to hear words, sentences,
perceives others are speaking with him, but in
reality no one is speaking.
Visual hallucination
• Person experiences that he is seeing
someone/object/animal, but in reality no one is
present
Olfactory hallucination
• Person experiences unpleasant smell, but
in reality no smell is present
Gustatory hallucination
• Person experiences unpleasant taste, but
in reality taste is normal
Tactile hallucination
• Person experiences sensations of touch,
prick or movements just below the skin,
feels insects moving through the tissues.
DISORDERS OF THOUGHT
DELUSION
False , unshakeable beliefs and not
affected by rational or logical
arguments
Delusion of control= An individual
thought, impulses, feelings and
actions are not own, but they are
inserted on them by some external
force or person
Delusion of grandiosity=
Exaggerated concept of one’s own
importance, power, knowledge or
identity. E.g. individual thinking that
he is prime minister or GOD or a
famous actor or actress
Delusion of Jealousy or infidelity
False unshakable belief or doubt
about the spouse’s fidelity or trustfulness.
Delusion of persecution
Strong belief that he is being
threatened or mistreated or cheated or
attacked or damages his reputation
Delusion of control

Nihilistic delusion
• The client feels that nothing exists
either person or things. E.g. he feels
that his body, mind and world is not
existing.
Delusion of influence
• Client has a strong belief that enemies are
influencing him in various ways.
Delusion of sin, guilt and worthlessness
• Client has a strong belief that he has
committed some unforgivable sin or
mistake on others, feels that he is a evil
and worthless human being.
Thought block
Client experiences a break in the flow of
thoughts through his mind and missed
thought were taken by some outside
agencies
Thought insertion
Client’s thoughts are implanted or
inserted by other person or external force
Thought withdrawal
Client’s thoughts are taken out of
their mind by other person or external force
Thought broadcasting
Unspoken thoughts are known to
others through broadcasting like radio,
internet, telepathy
Pressure of thought
Large quantity and variety of thoughts
arises and changes frequently in mind
Poverty of thought
Small quantity of thoughts arising, lacks
variety and flow of thoughts
Mood
Disorders of mood and affect
• Mood = Sustained feeling lasts for large
period
• Affect = Dominant feeling connected
with thought process.
• Emotion = Strong feeling
• Blunt of ideas =Reduced emotion
• Labile emotions =Excessively rapid
sudden changing emotions
• Emotional incontinence = Marked mood
changes or mood swings
• Ecstasy =Client experiences great pleasure
or enthusiasm
• Exaltation =Feels about raise in rank or power
• Elation = Elevation of the mood above
normal range , client abnormally cheerful and
optimistic
• Depression =Persistent sadness of
• mood and interfere in daily routine of
• person’s adjustment
MANIA DEPRESSION
