Pt is 76 year old with pneumonia and malnutrition. Patient is on O2L per NC and breath sounds
decreased to right lower lobe with crackles. His breath sounds are 27 per min. Patient has not had
a bowel movement in 2 days and is constipated with decreased bowel sounds to LLQ. His HR is
regular, rate and rhythm with a noted murmur at 76 . He has a stage 1 decubitus to right hip with
dressing intact. He has noted melanoma to his right hand and is being evaluated for removal. He
currently has an Ut with a foley in place due to urinary retention. He uses a walker for ambulation,
and he is hard of hearing and wears a hearing aid to his left ear. He has dementia ands is
confused about where he is most of the time. He has an IV to his left-hand infusing 0.9%NS. The
site is intact with no redness noted. Do a nursing report..
