This document provides an overview of holistic treatment options for neck and back pain authored by Dr. Shriniwas Janardan Kashalikar. It discusses the concept of holistic health and total stress management. It then provides remedies and techniques from Ayurveda, Allopathy, exercise, diet, posture, utilities, morning ablutions, body movements, yoga, massage, homeopathy, and osteopathy to treat neck and back pain holistically. The most important healing principle, according to the author, is to practice Namsmaran daily.
Refelctionsand Creations Dr. Shriniwas Kashalikarabanbana
Ā
The document discusses reflections and creations, and how they can provide both pleasure and horror. It notes that people commonly enjoy watching their mirror image and receiving compliments, and performers enjoy audiences' applause. Creations like successful surgeries or films can also boost creators. However, sometimes reflections and creations show horrifying failures, which abstract thinkers working to help the world face through lack of or distorted feedback and results. The document questions whether namasmÄran, or remembrance of the divine name, could help such people, suggesting it is worth trying.
Namasmran is remembering the name of God, one's guru, or great souls through chanting, singing, or silently. It is a way to reconnect one's physiological self with one's true spiritual self. Namasmran is considered the highest form of yoga and meditation as it is a culmination of all spiritual practices and aims to unify individual consciousness with objective cosmic consciousness. Regular practice of namasmran can help rise above commercial and material concerns and manifest higher purpose in one's life. It is an ocean of bliss that is beyond full description.
1) Prayer is meant to catalyze the culmination of any work in complete satisfaction and global welfare.
2) The essence of worshipping Lord Ganesha is that he represents the experience of enlightenment and cosmic oneness with one's inner and outer environment.
3) It is customary to pray to Lord Ganesha before starting any work because realizing truth is necessary for accurate vision, decision making, and successful action. These prayers provide holistic health or self-realization.

Massage has many health benefits. It improves circulation throughout the body and strengthens tissues, nerves, and metabolic activity. Regular massage, including self-massage of the head and feet, can significantly improve health and flexibility. Foot massage in particular is recommended two to three times per week to improve circulation in the feet and prevent common foot ailments, especially for those with jobs requiring long periods of standing or walking. Massage is most beneficial when done with care and can help soothe both body and mind.
The document discusses "my problem", which is a struggle within individuals between what is and what should be. This restlessness stems from sensory input not matching feelings of wants and deficiencies. "My problem" is a global issue, as it is difficult to voluntarily blossom into generous, loving, and benevolent qualities. Punishment and force cannot transform individuals, only creating a globally beneficial environment through tolerant policies and considering everyone can help with blossoming. However, even this is not possible through individual efforts alone in a sectarian environment. The only solution discussed is universal practice of remembrance of the divine name.
Unconditoned Happiness Dr. Shriniwas K Ashalikarabanbana
Ā
The document discusses the state of unconditioned happiness exemplified by saints. It clarifies that this state is not one of depression, surrender, or indifference, but rather a "videhi awasthaa" state where one has risen above perceptions and bodily influences. In this state, the individual appears unaffected but can interact with situations in subtle, trans-physical ways through articulation, action, or mystical means. The document emphasizes that cosmic consciousness and its manifestations are infinite and subtle, beyond the capacity of most to appreciate or experience, and that unconditioned happiness is a hallmark of evolved consciousness, not indolence or ignorance.

The biomechanics of running involves the study of the mechanical principles underlying running movements. It includes the analysis of the running gait cycle, which consists of the stance phase (foot contact to push-off) and the swing phase (foot lift-off to next contact). Key aspects include kinematics (joint angles and movements, stride length and frequency) and kinetics (forces involved in running, including ground reaction and muscle forces). Understanding these factors helps in improving running performance, optimizing technique, and preventing injuries.

Storyboard on Acne-Innovative Learning-M. pharm. (2nd sem.) CosmeticsMuskanShingari
Ā
Acne is a common skin condition that occurs when hair follicles become clogged with oil and dead skin cells. It typically manifests as pimples, blackheads, or whiteheads, often on the face, chest, shoulders, or back. Acne can range from mild to severe and may cause emotional distress and scarring in some cases.
**Causes:**
1. **Excess Oil Production:** Hormonal changes during adolescence or certain times in adulthood can increase sebum (oil) production, leading to clogged pores.
2. **Clogged Pores:** When dead skin cells and oil block hair follicles, bacteria (usually Propionibacterium acnes) can thrive, causing inflammation and acne lesions.
3. **Hormonal Factors:** Fluctuations in hormone levels, such as during puberty, menstrual cycles, pregnancy, or certain medical conditions, can contribute to acne.
4. **Genetics:** A family history of acne can increase the likelihood of developing the condition.
**Types of Acne:**
- **Whiteheads:** Closed plugged pores.
- **Blackheads:** Open plugged pores with a dark surface.
- **Papules:** Small red, tender bumps.
- **Pustules:** Pimples with pus at their tips.
- **Nodules:** Large, solid, painful lumps beneath the surface.
- **Cysts:** Painful, pus-filled lumps beneath the surface that can cause scarring.
**Treatment:**
Treatment depends on the severity and type of acne but may include:
- **Topical Treatments:** Such as benzoyl peroxide, salicylic acid, or retinoids to reduce bacteria and unclog pores.
- **Oral Medications:** Antibiotics or oral contraceptives for hormonal acne.
- **Procedures:** Such as chemical peels, extraction of comedones, or light therapy for more severe cases.
**Prevention and Management:**
- **Cleanse:** Regularly wash skin with a gentle cleanser.
- **Moisturize:** Use non-comedogenic moisturizers to keep skin hydrated without clogging pores.
- **Avoid Irritants:** Such as harsh cosmetics or excessive scrubbing.
- **Sun Protection:** Use sunscreen to prevent exacerbation of acne scars and inflammation.
Acne treatment can take time, and consistency in skincare routines and treatments is crucial. Consulting a dermatologist can help tailor a treatment plan that suits individual needs and reduces the risk of scarring or long-term skin damage.

CLASSIFICATION OF H1 ANTIHISTAMINICS-
FIRST GENERATION ANTIHISTAMINICS-
1)HIGHLY SEDATIVE-DIPHENHYDRAMINE,DIMENHYDRINATE,PROMETHAZINE,HYDROXYZINE 2)MODERATELY SEDATIVE- PHENARIMINE,CYPROHEPTADINE, MECLIZINE,CINNARIZINE
3)MILD SEDATIVE-CHLORPHENIRAMINE,DEXCHLORPHENIRAMINE
TRIPROLIDINE,CLEMASTINE
SECOND GENERATION ANTIHISTAMINICS-FEXOFENADINE,
LORATADINE,DESLORATADINE,CETIRIZINE,LEVOCETIRIZINE,
AZELASTINE,MIZOLASTINE,EBASTINE,RUPATADINE. Mechanism of action of 2nd generation antihistaminics-
These drugs competitively antagonize actions of
histamine at the H1 receptors.
Pharmacological actions-
Antagonism of histamine-The H1 antagonists effectively block histamine induced bronchoconstriction, contraction of intestinal and other smooth muscle and triple response especially wheal, flare and itch. Constriction of larger blood vessel by histamine is also antagonized.
2) Antiallergic actions-Many manifestations of immediate hypersensitivity (type I reactions)are suppressed. Urticaria, itching and angioedema are well controlled.3) CNS action-The older antihistamines produce variable degree of CNS depression. But in case of 2nd gen antihistaminics there is less CNS depressant property as these cross BBB to significantly lesser extent.
4) Anticholinergic action- many H1 blockers
in addition antagonize muscarinic actions of ACh. BUT IN 2ND gen histaminics there is Higher H1 selectivitiy : no anticholinergic side effects

Ā
GASTROINTESTINAL INFECTIONS AND GASTRITIS
Osvaldo Bernardo Muchanga
Gastrointestinal Infections
GASTROINTESTINAL INFECTIONS result from the ingestion of pathogens that cause infections at the level of this tract, generally being transmitted by food, water and hands contaminated by microorganisms such as E. coli, Salmonella, Shigella, Vibrio cholerae, Campylobacter, Staphylococcus, Rotavirus among others that are generally contained in feces, thus configuring a FECAL-ORAL type of transmission.
Among the factors that lead to the occurrence of gastrointestinal infections are the hygienic and sanitary deficiencies that characterize our markets and other places where raw or cooked food is sold, poor environmental sanitation in communities, deficiencies in water treatment (or in the process of its plumbing), risky hygienic-sanitary habits (not washing hands after major and/or minor needs), among others.
These are generally consequences (signs and symptoms) resulting from gastrointestinal infections: diarrhea, vomiting, fever and malaise, among others.
The treatment consists of replacing lost liquids and electrolytes (drinking drinking water and other recommended liquids, including consumption of juicy fruits such as papayas, apples, pears, among others that contain water in their composition).
To prevent this, it is necessary to promote health education, improve the hygienic-sanitary conditions of markets and communities in general as a way of promoting, preserving and prolonging PUBLIC HEALTH.
Gastritis and Gastric Health
Gastric Health is one of the most relevant concerns in human health, with gastrointestinal infections being among the main illnesses that affect humans.
Among gastric problems, we have GASTRITIS AND GASTRIC ULCERS as the main public health problems. Gastritis and gastric ulcers normally result from inflammation and corrosion of the walls of the stomach (gastric mucosa) and are generally associated (caused) by the bacterium Helicobacter pylor, which, according to the literature, this bacterium settles on these walls (of the stomach) and starts to release urease that ends up altering the normal pH of the stomach (acid), which leads to inflammation and corrosion of the mucous membranes and consequent gastritis or ulcers, respectively.
In addition to bacterial infections, gastritis and gastric ulcers are associated with several factors, with emphasis on prolonged fasting, chemical substances including drugs, alcohol, foods with strong seasonings including chilli, which ends up causing inflammation of the stomach walls and/or corrosion. of the same, resulting in the appearance of wounds and consequent gastritis or ulcers, respectively.
Among patients with gastritis and/or ulcers, one of the dilemmas is associated with the foods to consume in order to minimize the sensation of pain and discomfort.

Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Ā
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

Victory Over Neck And Back Pain Dr Shriniwas Kashalikar
1. VICTORY
OVER
NECK AND BACK
PAIN
BY
DR. SHRINIWAS
JANARDAN
KASHALIKAR
2. INTRODUCTION
I had following and many questions in
my mind, about ailments and healing.
Would I like to keep ailing like many
others are?
Would I like to go from doctor to
doctor belonging to different systems
of medicine as others are?
Would I like to go through a vicious
chain or cycle of expensive
investigations and subsequent
admonition [without any guarantee of
health and happiness] as many of us
are forced to go through?
3. Would I like to spend exorbitant
money on treatment, as many have to
spend?
Would I like to be eternally dependent
on medical professionals belonging to
some or other particular system of
medicine or a healer of some kind or
other as many of us depending?
Would I like to live in the threatening
shadow of disease and death forever as
many others are living?
The answer was a big NO.
This was probably why; I, as a medical
student and later as a
medical professional; kept on working
on this issue and realized;
that all medical systems of are part of
one single PATH OF HEALING. In
4. fact in the course of time I realized
that all the fields
of knowledge and art, could contribute
to the health or disease of an
individual and society. This is the
concept of HOLISTIC
MEDICINE.
But what was the HOLISTIC
MEDICINE expected to do?
Was it to impart only physical health?
No. So-called health, which
is determined by and endorsed by
laboratory and other
investigations, considers only physical
well being. But physical
well-being is not at all a true indicator
of health. A person cannot be judged
in the same way as we judge a statue.
A person is judged by his or her
5. actions. The actions in turn spring
from oneās
instincts, emotions, thoughts and
perspective.
HOLISTIC HEALTH therefore is
nothing else but a process of acquiring
the perspective of global welfare,
policies and plans and commitment to
these and
coordinated and concerted efforts of
experts in that direction from different
walks of life! Holistic Medicine is
understanding and practicing the
wisdom in their respective fields, for
the well being of one and all is the key
to HOLISTIC HEALTH which I call
TOTAL STRESS MANAGEMENT
also.
This small book is a humble effort in
that direction.
6. Precautions:
1. In this book you will find a variety
of remedies. It is important to be
aware of the legislations involved in
the use of the different
remedies.
2. It is quite possible that concurrent
use some of these remedies may not be
acceptable to some medical
practitioners.
3. It is also true that I do not have
clinical trials to quote;
for some of these remedies.
4. The quality of a particular
product is something I cannot vouch
on at all.
5. Some of the remedies may require
doctorās prescription.
7. 6. Some of the YOGASANAs or other
procedures/endeavors may require
expert guidance.
7. It is also important to note that we
are different individuals with
different constitutions. We are of
different age groups and genders.
We may vary in terms of different
physiological conditions such as
pregnancy, menstruation and
lactation. These variable factors alter
the efficacy and safety of medications
and practice of yogasanas or exercises.
8. The ailment may have
different causes and the stage of the
ailment may also vary.
9. We may have different histories of
past and family illnesses.
10. These different considerations may
limit the utility of this book.
8. However this book would surely
provide you, with new options for
substantial improvement in your
HOLISTIC HEALTH irrespective
of whether you belong to a non-
medical or a medical profession.

To succeed in this, practice

difficulties and the agonies produced
by them.
