34. Oblique Direct
Hernia enters the inguinal canal through deep
inguinal ring
It enters the canal through the triangle of Hesselbach
The neck of the hernial sac lies lateral to the
inferior epigastric artery
The neck of the hernial sac lies medial to the inferior epigastric
artery
Unilateral and occurs in young subjects Usually bilateral and appears in old age
Directed downwards, forwards and medially Directed almost straight and forwards
Case is congenital, due to presence of
processus vaginalis of peritoneum
Case is acquired due to weakness of the conjoint tendon
It receives all the coverings of the spermatic
cord and the vas deferens usually lies behind
and below the sac
It pushes the contents of spermatic cord laterally and the
hernial sac intervenes between internal spermatic fascia and
cremasteric fascia in lateral direct hernia; between cremasteric
fascia and external spermatic fascia in medial direct hernia.
