The document is a 3-page evaluation form for mentors in the Volunteers for Youth SMART Mentor program. It asks mentors to provide feedback on various aspects of the program, their mentoring experience, and their mentoring relationship. Mentors are asked to rate different program components, their satisfaction with the match and experience, their effectiveness, and the impact mentoring had on them personally. They are also asked open-ended questions about how the relationship affected their attitudes, what has been easy and hard about mentoring, beneficial activities done with their mentee, and recommendations for future mentor-mentee pairs.
