The School of Health and Social Development would like to find out how the student performed while
undertaking his/her work placement. This evaluation will contribute 20% towards their total mark for
the unit. Before the student’s last day of placement, please complete the following questions.
In order to facilitate student learning, it is expected that the field supervisor discuss the evaluation
results with the student before providing a copy to the university.

profile ID and data from Community Indicators Victoria
-Conducted a food retail audit in Brunswick West and Coburg North
-Mapped public transport routes in Brunswick West and Coburg North
-Used the Victorian Healthy Food Basket Survey to determine the cost of a standard basket
of food in Brunswick West and Coburg North
-Wrote a report highlighting the findings
-Sourced articles on puberty, sexuality and relationship education for "The Talk" newsletter
to be distributed to teachers working in Moreland schools

Angie was a pleasure to work with and fitted into the team perfectly. She contributed
positively to team meetings. Angie showed initiative by meeting with other staff
members in the team who were undertaking work in areas that she was interested in.
She showed great skills in planning and ensured she stepped out the tasks given to
her in a logical manner. Her writing skills were great and she was able to summarise
the data that she collected into a concise report. She was thorough and ensured that
the data collected was accurate. Angie was enthusiastic about the work she
undertook on placement and it was great to have her on board.
I don't have too many suggestions for improvement. As a student Angie was very
willing to learn and take it all in. With more experience, I have no doubt that she will
gain confidence in participating in meetings, and will excel in her future career.
Most definitely passed.
Angie has provided MCHS with a thorough report on the level of food insecurity in
the suburbs of Brunswick West and Coburg North, which will contribute to our
broader needs assessment.
She has showed initiative and made the most of all of the opportunities that were
provided to her. Angie got along really well with the team and we wish her all the
best in her final year of study.

I gained experience in supervision. We also were able to get data collected that we
may otherwise not have had the time to do.
