I. PURPOSE
To establish a uniform procedure for the disposal of owner surrendered animals for the purpose
of euthanasia or deceased animals with regards to cremation or rendering.
II. POLICY
Animal owners surrendering their pet for the purpose of euthanasia or owners of deceased
animals shall be provided with information regarding the animal disposal methods available to
them. The Kennel Attendant assigned to Station I shall give the disposal information sheet to
the pet owner to review.

Applicable to all Kennel Services staff.

VII. PROCEDURE
A. Owner Surrendered Animals for Euthanasia, Rendering
1. Only those animals that have documented aggression or a medical condition that
would be considered by the Veterinarian on duty as either irremediably suffering
or can not be remedied or cured within a reasonable effort may be released by
the owner for immediate euthanasia.

2. The Kennel Attendant (KA) assigned to Station I shall enter the owner and
animal information into the Chameleon Kennel Window and collect applicable
fees.
3. The KA shall obtain the signature from the owner authorizing euthanasia in the
Animal Relinquishment Form book.
4. The KA shall affix a collar ID band with the animal’s identification number to
the transport cart.
5. The animal shall then be transported to the Station III.
6. The signed Animal Relinqishment Form shall be given to Veterinary Services
staff for processing or attached to the file box located in the clinic.
B. Owner Surrendered Animals for Euthanasia, Cremation
1. The KA assigned to Station I shall enter the owner and animal information into
the Chameleon Kennel Window and collect applicable fees. The fees are
determined by the weight of the animal, as stated on the provided fee schedule.
2. The scale located in Station III shall be used to determine the animal’s weight.
For safety reasons, an approximate weight for aggressive animals shall be
acceptable.
3. The KA shall obtain the signature from the owner authorizing euthanasia in the
Animal Relinquishment Form book.
4. The KA shall affix a collar ID band with the animal’s identification number to
the transport cart.
5. The KA shall complete the Cremation Identification Card with all requested
information. The Cremation Identification Card shall be attached to a manila tag
along with the signed Animal Relinquishment Form authorizing euthanasia.
6. Transport documents and animal to Station III.
7. The signed Animal Relinquishment Form shall be given to Veterinary Services
staff for processing or attached to the file box located in the Clinic.
8. After euthanasia, the Cremation Identification Card shall be attached to the
animal and placed in a designated container in the morgue.
9. The KA assigned to Station III shall enter all information in the Cremation Log
Book.
C. Deceased Animals, Rendering
1. The KA assigned to Station I shall enter the owner and animal information into
the Chameleon Kennel Window and collect applicable fees.
2. The animal shall then be transported to the Station III.

D. Deceased Animals, Cremation
1. The KA assigned to Station I shall enter the owner and animal information into
the Chameleon Kennel Window and collect applicable fees. The fees are
determined by the weight of the animal, as stated on the provided fee schedule.
The scale located in Station III shall be used to determine the animal’s weight.
2. The KA shall complete the Cremation Identification Card with all requested
information. This Cremation Identification Card shall be attached to a manila tag
and placed on the animal.
3. Then animal shall then be placed in a designated container in the morgue.
4. The KA assigned to Station III shall enter all information in the Cremation Log
Book.
E. Cremation Log Book
1. The KA assigned to Station III shall enter all applicable information into the
Cremation Log Book.
2. The KA assigned to Station III shall create a memo in the Chameleon Kennel
Window stating that the animal has been received by the cremation vendor.
3. The KA assigned to Station III shall review the Cremation Log at the end of
each day and identify those animal awaiting cremation.
