Taking a thorough history is important for determining the patient's current ear problems and how factors like age, past illnesses or injuries, medications, lifestyle, and family history may be related.

2. HISTORY
 An otologic history include demographic data,
current clinical menifestations, past health
history, family health history, psychosocial
history and review of symptoms.
 The history interview is essential for
determining current problems related to the
ear.

3. DEMOGRAPHIC DATA
 It includes the client’s age. Hearing
impairment may occur as a result of the aging
process.

4. CURRENT HEALTH
 Hearing problems can affect a client’s ability
to communicate as well as limit their social
activities and job opportunities.
 Ear problems and hearing loss can lead to
isolation and depression.
 It is important to assess both verbal and non
verbal feedback and listen attentively. The
nurse should face the client as directly as
possible while speaking.

8. PAST MEDICAL HISTORY
 History of upper respiratory infections.
 History of head/ear trauma. History of
eardrum perforation.
 Immunized for Measles, mumps or Hib.
Childhood infections like acute otitis media ,
chronic otitis media.

9. PAST SURGICAL HISTORY
 History of ear surgery, History of tonsils,
adenoid, nose and throat surgery.

10. ALLERGIES
 Allergies to medications/food. Seasonal
allergies

11. MEDICATIONS
 Aspirin can cause tinnitus.
 Analgesics, diuretics, Salicylates,
chemotherapeutic agents,aminoglycosidse
antibiotics are ototoxic drugs.
 Ginger is known for its antinausea effect and
used for motion sickness.

12. DIETARY HABITS
 Inquire about dietary restrictions, use of Food
supplements.
 Meniere’s disease may be treated with low
salt diet.

13. FAMILY HEALTH HISTORY
 History of hearing loss in family members?

14. PSYCHOSOCIAL HISTORY
 Use of illicit drug/ Smoking history, Alcohol
intake.
 Occupation/Environmental exposure to loud
noise. Swimming ( Use of ear plugs).
 Ask about exposure to loud noises. Exposure
to levels greater than 85 to 90 db for months or
years cause cochlear change.
